Tumor Thrombus
A 48-year-old nonsmoking woman presented with acute epigastric pain after several months of nocturnal sweats. Computed tomography revealed a 9-cm right renal mass with venous tumor thrombus extending from the right renal vein to the right atrium, with no pulmonary embolism and no evidence of distant metastasis. Intraoperative transesophageal echocardiogram demonstrated an 8- × 4-cm echogenic mass (TT) recoiling into the right atrium in systole (A, Online Video 1) and prolapsing into the right ventricle (RV) in diastole (B), with an intact tricuspid valve (Online Video 2). The mass prolapsed into the right ventricular ou...

Whole exome sequencing of adenoid cystic carcinoma
Adenoid cystic carcinoma (ACC) is a rare malignancy that can occur in multiple organ sites and is primarily found in the salivary gland. While the identification of recurrent fusions of the MYB-NFIB genes have begun to shed light on the molecular underpinnings, little else is known about the molecular genetics of this frequently fatal cancer. We have undertaken exome sequencing in a series of 24 ACC to further delineate the genetics of the disease. We identified multiple mutated genes that, combined, implicate chromatin deregulation in half of cases. Further, mutations were identified in known cancer genes, including PIK3C...

Mutation signature of adenoid cystic carcinoma: evidence for transcriptional and epigenetic reprogramming
Adenoid cystic carcinoma (ACC), a relatively rare malignancy usually of salivary gland origin, has a signature v-myb avian myeloblastosis viral oncogene homolog–nuclear factor I/B (MYB-NFIB) gene fusion that activates MYB transcriptional regulatory activity. A new study in this issue by Stephens et al. is a comprehensive genomic mutation profiling analysis of this neoplasm and documents a common theme of alteration in chromatin regulatory genes. Also, mutations in SPEN (split ends, homolog of Drosophila), which encodes an RNA-binding coregulatory protein, suggest that other changes in transcriptional regulation may invol...

Lymphadenectomy combined with locoregional treatment for multiple advanced hepatocellular carcinoma with lymph node metastases.
Abstract
Lymphadenectomy of lymph node metastasis (LNM) from hepatocellular carcinoma (HCC) may potentially improve survival of patients with intrahepatic tumors controllable by means of locolegional treatment. However, the treatment strategy has not gained wide clinical acceptance, especially in patients with multiple advanced HCC. Thus, the purpose of this study is to evaluate the role of lymphadenectomy combined with locoregional treatment for the management of multiple advanced HCC with LNM. Between January 1998 and August 2012, 15 patients underwent a selective lymphadenectomy either concurrently or sequential...

Non-detection of Epstein-Barr virus and human papillomavirus in a region of high gastric cancer risk indicates a lack of a role for these viruses in gastric carcinomas
Gastric mucosa tissue was collected from patients with gastroduodenal diseases in a region of norrteastern China showing a high risk of gastric cancer incidence. The presence of EBV and HPV were assayed to investigate the relationship between gastric carcinomas and virus infection. Neither EBV nor HPV DNA was detected in tissue from the patients. The role of EBV and HPV in gastric cancer is not well understood and still needs to be clarified. (Source: Genetics and Molecular Biology)

Bioinformatics analysis of the gene expression profile in Bladder carcinoma
Bladder carcinoma, which has the ninth highest incidence among malignant tumors in the world, is a complex, multifactorial disease. The malignant transformation of bladder cells results from DNA mutations and alterations in gene expression levels. In this work, we used a bioinformatics approach to investigate the molecular mechanisms of bladder carcinoma. Biochips downloaded from the Gene Expression Omnibus (GEO) were used to analyze the gene expression profile in urinary bladder cells from individuals with carcinoma. The gene expression profile of normal genomes was used as a control. The analysis of gene expression revea...

Hereditary Syndromes With Associated Renal Neoplasia: A Practical Guide to Histologic Recognition in Renal Tumor Resection Specimens
Many hereditary tumor syndromes are associated with neoplasms of the kidney. It is becoming increasingly well recognized that a given familial tumor syndrome may be very heterogenous in clinical appearance and that unrecognized patients may present initially for the treatment of a renal mass. It is therefore important for surgical pathologists to be aware of the specific gross and microscopic findings in the kidney that suggest a possible syndromic association. In this review, we detail the histologic features of syndromic-associated renal neoplasms, describe the presence of characteristic changes in the background renal p...

Successful single-cycle rituximab treatment in a patient with pemphigus vulgaris and squamous cell carcinoma of the tongue and IgG antibodies to desmocollins
To the Editor: Pemphigus can be triggered by many insults, including drugs, infections, and neoplasms. Radiation-induced pemphigus vulgaris accompanied by an internal malignancy has been rarely reported, but the link between pemphigus vulgaris and malignancy still needs to be elucidated. Moreover, the treatment of pemphigus in this condition is often challenging, as the patient’s immune system is already suppressed by radiation and chemotherapy. Is living donor liver transplantation really equivalent to deceased donor liver transplantation?
Living donor liver transplantation (LDLT) has developed as an alternative to deceased donor liver transplantation (DDLT) in order to overcome the critical shortage of deceased organ donations. Particularly in regions with low deceased donation rates, like Asian, LDLT for end stage liver disease significantly reduces the risk of death or drop‐off the wait list without compromising posttransplant survival. A preference for LDLT to DDLT may depend on the original disease representing the indication for liver transplantation (LT). LDLT offers a timely alternative to DDLT for patients with hepatocellular carcinoma (HCC). (Sou...

High HMGA2 expression and high body mass index negatively affect the prognosis of patients with ovarian cancer
Abstract
HMGA2 is a small, non‐histone, chromatin‐associated protein with a key role in tumorigenesis and adipogenesis. Indeed, HMGA2 overexpression has been frequently detected in several malignant neoplasms and inhibition of its expression prevents thyroid cell transformation. Moreover, HMGA2 null mice show a pigmy phenotype with a great reduction in fat tissue. To investigate whether HMGA2 expression correlates with clinico‐pathological parameters and patient outcome, immunohistochemical analysis of HMGA2 expression was performed in ovarian cancer specimens from 117 patients.HMGA2 overexpression was found in 39% o...

A Human Thyroid Cancer Cell Line, DH-14-3, Newly Established from Poorly Differentiated Thyroid Carcinoma.
Abstract
Poorly differentiated thyroid carcinoma (PDTC) is a newly recognized histological type of malignant thyroid tumor, accounting for about 2 - 13% of all thyroid carcinomas. PDTC is considered as a morphologically and biologically intermediate stage between well-differentiated thyroid carcinoma and anaplastic thyroid carcinoma. PDTC preferentially manifests bone metastases. We here established a cell line from a resected tumor specimen from a 70-year-old male patient with PDTC who presented with multiple bone metastases. This new thyroid tumor cell line was designated as DH-14-3 and was subsequently grown in ...

Biochemical and biological characterization of exosomes containing prominin-1/CD133
Exosomes can be viewed as complex "messages" packaged to survive trips to other cells in the local microenvironment and, through body fluids, to distant sites. A large body of evidence indicates a pro-metastatic role for certain types of cancer exosomes. We previously reported that prominin-1 had a pro-metastatic role in melanoma cells and that microvesicles released from metastatic melanoma cells expressed high levels of prominin-1. With the goal to explore the mechanisms that govern proteo-lipidic-microRNA sorting in cancer exosomes and their potential contribution(s) to the metastatic phenotype, we here employed promini...

The Paracrine Effect of Cancer‐associated Fibroblast‐induced Interleukin‐33 Regulates the Invasiveness of Head and Neck Squamous Cell Carcinoma
Abstract
Head and neck squamous cell carcinoma (HNSCC) is one of the leading causes of cancer‐related death worldwide. The prognosis of HNSCC is usually poor because of its propensity for extensive invasion, local recurrence and frequent regional lymph node metastasis even at initial diagnosis. Carcinoma‐associated fibroblasts (CAFs), a major type of tumour‐surrounding stromal cell, generate mediators through which they interact with tumours and contribute to cancer progression. The orchestration between CAFs and cancer cells is complex. Despite recent studies demonstrating the paracrine effect of stromal cells in th...
