A Case of Thunderclap Headache and Neurological Deficits
Dr. Christiana Prucnal: Today's case is that of a 62-year-old woman who presented to the emergency department (ED) with sudden onset headache. For the three days prior to presentation, she noted episodic, momentary, unprovoked visual symptoms, described as “lines and dots” that would obscure part of her visual field, but otherwise felt well and in her usual state of health. Forty-five minutes prior to ED arrival, she reached to grasp an object from atop her refrigerator and experienced acute right-sided stabbing headache with associated dizziness. Structural equation modeling for identifying the drivers of health-related quality of life improvement experienced by patients with migraine receiving eptinezumab
ConclusionsImprovements in HRQoL experienced by patients treated with eptinezumab can be substantially explained by its effect on migraine frequency and PI-MBS. Therefore, in addition to MMD reduction, healthcare providers should discuss PI-MBS improvements, since this may impact HRQoL. Health technology policymakers should consider implications of these findings in economic evaluation, as they point to alternative measurement of quality-adjusted life years to capture fully treatment benefits in cost-utility analyses.Trial registration ClinicalTrials.gov (Identifier:NCT04418765; EudraCT (Identifier: 2019 –004497-25; URL:...

OnabotulinumtoxinA as a promising treatment for primary trochlear headache: A retrospective case series
CONCLUSIONS: Our case series presents the first evidence of the potential of BoNTA as a safe and effective treatment option for PRTH. From a clinical standpoint, having a safer alternative is of paramount significance for patients with limited treatment options, such as those with PRTH. Further research is warranted to validate these findings and explore the long-term efficacy of BoNTA in PRTH management.PMID:38533675 | DOI:10.1111/head.14699 (Source: Headache)

Lumbar percutaneous transforaminal endoscopic discectomy: a retrospective survey on the first 172 adult patients treated in Denmark
ConclusionClinical improvements after lumbar PTED performed by surgeons novel to the technique are satisfactory, although the reoperation rate is high, severe complications may occur, and the learning curve can be longer than expected. (Source: Acta Neurochirurgica)

The effects of upper cervical spine manual therapy on central sensitization and brainstem auditory evoked potentials in subjects with migraine and neck pain
Migraine is an important disease due to its impact on global health. Due to association of neck pain with migraine, manual therapy (MT) often used in management. The aim of this study was to evaluate the effects of upper cervical spine MT on specific mechanisms including central sensitization (CS) and brainstem auditory evoked potentials (BAEPs) in subjects with migraine and neck pain. Case report: A novel case of paraneoplastic voltage gated calcium channel antibodies secondary to appendiceal adenocarcinoma
Voltage gated calcium channels (VGCCs) play a critical role in neural transmission. Antibodies that target these ion channels can disrupt cellular signal transmission resulting in various clinical presentations. VGCC antibodies are most commonly associated with paraneoplastic syndromes such as Lambert-Eatons myasthenic syndrome. Here, we report a 47-year-old female with Stage IV appendiceal adenocarcinoma status post appendectomy and right hemicolectomy, who presented with progressive memory impairment, aphasia, ataxia, weakness, and headache. Neurologic exam was notable for right-sided parietal drift, decreased right arm ...

Anal Biochem. 2024 Mar 24:115513. doi: 10.1016/j.ab.2024.115513. Online ahead of print.ABSTRACTNaproxen is a nonsteroidal anti-inflammatory drug used to treat nonrheumatic inflammation, migraine, and gout. Therefore, the determination of naproxen in pharmaceutical and biological samples is of particular importance. In the present work, SrTi0.7Fe0.3O3 perovskite/Chitosan nanosheets were used to modify the surface of a glassy carbon electrode (GCE) for highly sensitive determination of naproxen. To ensure the successful synthesis of the perovskite nanosheets, morphological studies including scanning electron microscopy (SEM)...

Associations between daily home blood pressure measurements and self-reports of lifestyle and symptoms in primary care: the PERHIT study
CONCLUSION: In hypertension management, it may be important to identify patients with high-stress levels and low wellbeing. The association between medication intake and BP was obvious, thus stressing the importance of medication adherence for patients with hypertension.PMID:38529930 | DOI:10.1080/02813432.2024.2332745 (Source: Primary Care)

Transitional Pain Service: An Update
AbstractPurpose of ReviewChronic Postsurgical Pain (CPSP) and the risk for long-term opioid dependency are known complications following major surgery. The idea of  Transitional Pain Service (TPS) has been introduced as an interdisciplinary setting to manage pain in the perioperative continuum. We expand on the basic framework and principles of TPS and summarize the current evidence of the TPS and possible interventions to adress postoperative pain. Areas o f future work in TPS-related research are discussed.Recent FindingsSeveral studies support the effectiveness of TPS in reducing opioid consumption in the perioperati...
