Recurrence after neoadjuvant chemoradiation and surgery for esophageal cancer: Does the pattern of recurrence differ for patients with complete response and those with partial or no response?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusions: Most esophageal cancer recurrences after neoadjuvant therapy and surgery are distant, and survival time after recurrence is short regardless of pathologic response. Fewer patients achieving complete response had recurrences, and distant recurrences in these patients manifest later than in patients showing partial response and those showing no response. Only pathologic response is significantly associated with disease recurrence, suggesting that tumor biology and chemosensitivity are critical in long-term patient outcome. Accurate assessment of primary tumor size is an essential part of management in breast cancer patients. Tumor size is part of the TNM system used to stage breast cancer and provide prognostic information. Adaptation of the principles of breast conservation surgery requires evaluation of tumor size to judge appropriateness and extent of surgery. Precise measurement of tumor size is also important to monitor response to neoadjuvant chemotherapy for locally advanced tumors. (Source: Cancercompass News: Breast Cancer)

Minimally invasive resection has emerged as a surgical technique for gastric cancer, and there has been continued investigation to determine the appropriate extent of lymphadenectomy in gastric cancer patients. There has also been significant progress in evaluating the role of chemotherapeutic regimens used in the neoadjuvant and adjuvant settings for patients with resectable disease. We also summarize a selection of RCT trials focused on the perioperative care of the gastric cancer patient. (Source: Surgical Oncology Clinics of North America)

During the past eight years, there have been significant advances in the treatment and outcome of patients with metastatic colorectal cancer. This is likely the result of improved imaging, staging, chemotherapy, and surgery. After conducting a standardized MEDLINE literature search, this article reviews the prospective randomized controlled trials in advanced colorectal cancer published between May 2001 and October 2008. There are a total of 96 studies, all chemotherapy-related trials, divided into five categories based on trial focus: (1) types of chemotherapy; (2) administration of adjuvant or neoadjuvant chemotherapy fo...

The treatment of esophageal cancer with curative intent remains highly controversial, with advocates of surgery alone, chemoradiotherapy alone, surgery with adjuvant therapy (including neoadjuvant and postoperative), and trimodality therapy each contributing prospective randomized controlled trials (PRCTs) to the body of scientific publications between 2000 and 2008. Any improvements in survival have been small in absolute percentage terms, and as such PRCTs published over the last decade have met the same primary obstacle encountered by the studies from the two prior decades, namely lack of power to detect small differenc...

Resection of hepatocellular carcinoma (HCC) offers the only hope for cure. However, in patients undergoing resection, recurrences, in particular, intrahepatic recurrence are common. The effectiveness of transarterial chemoembolization (TACE) as a neoadjuvant therapy for unresectable HCC was exploited by numerous liver units and employed preoperatively in the setting of resectable HCC with an aim to prevent recurrence and prolong survival. A systematic literature search of databases (Medline and PubMed) to identify published studies of TACE administered preoperatively as a neoadjuvant treatment for resectable HCC was undert...

To assess whether the response to primary androgen-deprivation therapy (PADT) and radiotherapy (RT) plus adjuvant ADT would be muted in older men, as their tumours might already be relatively androgen insensitive, because serum testosterone levels decline with increasing age. Patients with invasive upper tract transitional cell carcinoma (UTTCC) pose a difficult management problem with prognosis dependent on the degree of invasion, the relationship, if any, to previous superficial or invasive bladder cancer, and the presence of metastatic nodes. Similar to invasive bladder cancer, preoperative staging can often be inaccurate and distinguishing superficial invasion from deep invasion may not always be possible. Unlike invasive bladder cancer, strategies to improve survival rates using neoadjuvant systemic chemotherapy before nephroureterectomy have not been subjected to clinical trials. However,...

Altered expression of microRNAs (miRNA), an abundant class of small nonprotein-coding RNAs that mostly function as negative regulators of protein-coding gene expression, is common in cancer. Here, we analyze the regulation of miRNA expression in response to estrogen, a steroid hormone that is involved in the development and progression of breast carcinomas and that is acting via the estrogen receptors (ER) transcription factors. We set out to thoroughly describe miRNA expression, by using miRNA microarrays and real-time reverse transcription-PCR (RT-PCR) experiments, in various breast tumor cell lines in which estrogen sig...

To identify if thymidine phosphorylase (TP), dihydropyrimidine dehydrogenase (DPD), and ratio TP/DPD levels in tumor tissues are potential predictive factors for response to combined preoperative chemoradiation with capecitabine, in patients with locally advanced rectal cancer (LARC).Between 2004 and 2006, 28 patients with LARC (cT2-T4, N0-N2) were treated with neoadjuvant chemoradiation. Total radiation dose was 50.4 Gy and daily dose was 1.8 Gy in 5.5 weeks. Capecitabine was administrated 1,650 mg/m2/day, 7 days/week. Preoperative staging was based on combined computer tomography and endorectal ultrasound. Tissue samples...

This study is aimed to estimate the predictive role of PET CT and other imaging modalities (ultrasound, MRI) through NAC.PET CT was acquired before and after NAC from 41 patients. Pathologic results were classified as pathological complete response (pCR) and non-pCR. The results of clinical responses were assessed with imaging indexes (postTx, postchemotherapy size or peak standardized uptake values (pSUV); delta, the size difference between treatment; RR, reduction rate of tumor size or pSUV), and they were compared with pathologic results.Seven patients (17.1%) showed pCR. As a result of comparison of the image index, al...

Extended bile duct resection liver and transplantation in patients with hilar cholangiocarcinoma: Long-term resultsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
In conclusion, the overall long-term survival was relatively low in our inhomogeneous cohort but favorable in patients without metastases. However, because of the increased perioperative mortality, EBDR is not recommended as a standard procedure for hilar cholangiocarcinoma instead of LT alone. To further improve the results, other approaches such as (neo)adjuvant therapy have to be increasingly investigated. Liver Transpl 15:1499-1507, 2009. © 2009 AASLD. (Source: Liver Transplantation)

Long-term results of sleeve lobectomy in the management of non-small cell lung carcinoma and low-grade neoplasms.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSIONS: Sleeve lobectomy can be performed with minimal bronchial anastomotic complications and low postoperative mortality. In our study, neoadjuvant therapy for non-small cell lung cancer adversely influenced the rate of anastomotic complications. Performing sleeve lobectomy for patients with N1 disease was not associated with decreased overall survival rates. PMID: 19853115 [PubMed - in process] (Source: The Annals of Thoracic Surgery)

Successful treatment of neoadjuvant therapy for papillary tumor of the pineal regionemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We report a boy who in 1994, at the age of 11, presented with headache and vomiting. The fi nal diagnosis was papillary tumor of the pineal region (PTPR). Magnetic resonance imaging (MRI) revealed a heterogeneous mass. Hydrocephalus was addressed by immediate ventricle drainage; subsequently, we attempted tumor removal. As the intraoperative diagnosis of the hemorrhagic tumor was primitive neuroectodermal tumor (PNET), we did not proceed to total removal. After the delivery of radiotherapy (50.4 Gy) and one course of Nimustine hydrochloride (ACNU) chemotherapy, the residual tumor was completely resected. The diagnosis ...

Chemoradiotherapy for cancer of the esophagus: contribution of the leucovorin, 5-fluorouracil bolus, and infusion–cisplatin–radiotherapy schedule starting with two neoadjuvant chemotherapy cycles: results from a pilot studyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
To assess feasibility and tolerance of a modification in the usual radiochemotherapy regimen for esophageal cancer by using a leucovorin, 5-fluorouracil bolus, and infusion[ndash]cisplatin regimen (six cycles), beginning with two cycles of chemotherapy before conventional radiotherapy (50 Gy), 33 patients, 30 were men, 62.8 ± 9.5 years, were treated for an esophageal carcinoma (29 squamous cell), 27 of these were in stage III (based on computed tomography scan). Neoadjuvant chemotherapy was well tolerated; concomitant radiochemotherapy was associated with severe adverse events mostly hematological in 23 patients. Complete...

To evaluate the influence of the pathological Gleason score on the predictive value of the prostate-specific antigen (PSA) doubling time (DT), as this variable predicts a patient's risk of disease progression both before and after definitive therapy for prostate cancer, and there is an inverse correlation between the Gleason score and PSA production. We evaluated all men treated with radical prostatectomy (RP) between 1990 and 1999 who did not receive neoadjuvant or adjuvant therapy. We identified 2296 patients who had multiple PSA values available before RP, and 1323 who had biochemical recurrence after RP and had at leas...

Data from the Austrian Breast and Colorectal Cancer Study Group (ABCSG)-24 study presented at the joint 15th ECCO and 34th ESMO congress in Berlin, show that adding capecitabine (Xeloda) to anthracycline- and taxane-containing regimens prior to surgery completely eradicated the tumor in 24% of women with HER2-positive or HER2-negative early breast cancer. This is an impressive finding since the proportion of women achieving total tumor eradication with standard chemotherapy for HER2-positive or HER2-negative early breast cancer is less than 20% (range = 6%–18%).
