As technology makes the world smaller, monitoring clinical trials no longer requires traveling to a destination to check up on the investigators. Much of this work can be done remotely and the FDA, in fact, wants to encourage sponsors to undertake more of what the agency calls risk-based monitoring in order to match international standards articulated by the International Conference on Harmonisation.

A little while back I ran into a friend, Susan O’Connell (Zen Master and Vice President of the San Francisco Zen Center), and she did something old fashioned.
When I said we should have tea sometime, she immediately went to her bag and got out her paper calendar, and suggested we make a date right then.
I said, “No, you’re busy, we can set a date later.”
She said she tries to only deal with something once.

Last week, the FDA announced that any clinical tests conducted between April 2005 and June 2010 by a contract research organization called Cetero Research may have to be reevaluated because two FDA inspections and an outside audit found falsified data and manipulated samples. In explaining its move, the agency maintained there were “significant instances” of misconduct.
The agency says Cetero failed to conduct an adequate internal investigation to determine the extent and impact of the violations, and did not take sufficient steps to assure data integrity during those five years. And so drugmakers must check their data...

Three years after a dispute erupted over the seizure by EU authorities of Indian-made generics as they were being shipped through Europe, India and the European Commission have struck an interim deal that restricts the 27 member EU states from seizing meds unless there is evidence that drugs will be diverted in the European Union, according to a statement from the Prime Minister’s office in India.

As drugmakers scramble to replenish their pipelines, they are encountering all sorts of difficulties, including rising costs for clinical trials. And this is happening across all phases. Why? There is increasing competition for trial sites and clinical research organizations that can yield reliable, high quality data, according to a recent survey.

Several months after a curious link was observed between the GlaxoSmithKline pandemic flu vaccine known as Pandremix and unexplained cases of narcolepsy emerged in Finland and Sweden, the European Medicines Agency has decided to restrict usage.
Specifically, those younger than 20 years old should use Pandemrix only in the absence of seasonal trivalent flu vaccines, and if immunization against H1N1 swine flu is still needed. An example would be those at risk of complications from infection, according to the EMA (read here). However, the EMA notes that, for now, the benefits continue to outweigh the risks.

Is Medicaid getting good value for its money? A new study suggests the federal healthcare program may be paying more than is necessary for drugs and not offering the most effective options for patients. Why? The authors suggest Medicaid is apparently ignoring the World Health Organization’s 2009 Essential Medicines List, which offers biannual updates on meds that are safe and effective.

To settle a bitter dispute over seizures of generic meds at a Dutch airport, Eli Lilly is about to make an undisclosed payment to several Indian generic drugmakers, The Financial Times writes. Several big drugmakers, in fact, benefited when the lower-cost drugs were impounded, since the move thwarted distribution of the generics throughout the European Union.

A doctor and a clinical research coordinator working for a contract research organization hired by the former Schering-Plough were indicted by the feds for falsifying study data. The clinical trial was designed to test a tablet the drugmaker was developing to treat allergies.

A shift among drugmakers to prefer existing patients, rather than healthy volunteers, for early stage trials, is forcing Parexel, one of the largest contract research organizations, to close offices and eliminate jobs. Specifically, the CRO is cutting up to 30 per cent of Phase I capacity and around 300 jobs, or about 3 per cent of its 10,350 workforce, according to a filing with the Securities and Exchange Commission.

I’ve been working with a book editor for the last few weeks to put together an outline and proposal for a new book based on this blog. At the suggestion of Michael Hyatt, I decided to focus on one specific area, and research the material extensively. This has been a fun endeavor and the outline is taking shape. For those who had any doubt about where R&D money is headed…look east. As in the Far East. A recent survey of 241 execs from drugmakers and biotechs finds that R&D budgets are rising just 1 percent overall this year. Meanwhile, outsourcing is expected to rise 11 percent, to 41 percent of the R&D undertaken, and most companies favor investing more of their clinical work in China.

I just came away from an intense encounter with a group of creatives. Ideas were flowing, presentations were given, and real change is in the wind. After spending a week on the Re:create cruise, I’ve come back home to plot a course for success.
For me, success may be a published book, a speaking opportunity, or an increased focus on my blog. For others it may be a new recording, painting, or fiction based novel. All 93 of us on the cruise witnessed a plethora of ideas and strategies to transform our ideas into reality.
