The aim of the study was to describe our experience with bi-level positive airway pressure (BiPAP) ventilation in oncology children with acute respiratory failure, hospitalized in a single tertiary pediatric tertiary center. This was a retrospective cohort study of all pediatric oncology patients in our center admitted to the intensive care unit with acute hypoxemic or hypercarbic respiratory failure from January 1999 through May 2006, who required mechanical ventilation with BiPAP. Fourteen patients met the inclusion criteria with a total of 16 events of respiratory failure or impending failure: 12 events were hypoxemic, ...

Conclusion: Survey data highlight marked cancer service deficiencies in rural and regional Australia. It is not unreasonable to conclude that these deficiencies might contribute to poorer outcomes for cancer patients living in these areas. The results suggest the need for short- and long-term measures to improve access to best-practice cancer services for patients living in regional, rural and remote areas of Australia. White patients with advanced cancer are more likely than black patients with advanced cancer to receive the EOL care they initially prefer. A recently filed lawsuit by Cleveland J. Savoy is claiming 38 different companies are responsible for his development of an asbestos-related disease.
This is the second suit he has filed. In a previous suit issued against the companies, Savoy claimed a different asbestos-related injury in contrast to his most recent complaint.
Savoy believes he was exposed to asbestos-containing products while working as a carpenter. He also stated in the lawsuit that the companies negligently continued to manufacture asbestos-containing products where he worked even after knowing the hazards associated with asbestos.

(World Journal of Gastroenterology) A research team from Taiwan identified factors that were related to the short term effectiveness of percutaneous transhepatic biliary drainage in cholangiocarcinoma patients and evaluated the impact of palliative drainage on their survival. They found the short term effectiveness of percutaneous biliary drainage was related to patient's prothrombin time or the extent of tumor involvement. It, however, had no impact on survival. Abstract Rhabdomyosarcoma (RMS) is the most common soft tissue tumour in children. This is the first time a facial RMS metastatic to
the pancreas is reported. The patient was a 3-year-old boy with a symptomatic solitary pancreatic metastasis of a primary
facial RMS, with already one relapse in a submental lymph node. Since this was the second relapse of the disease and due to
the extensive nature of the mass, the patient was considered for palliative care and a bypass procedure with a choledochojejunostomy,
a gastrojejunostomy and a cholecystectomy was performed. The patient expired 6 months later.

Methadone is an effective and inexpensive opioid for cancer pain treatment. It has been reported as difficult to use in the outpatient setting because of its variable relative potency and long half-life. The purpose of this study was to determine the outcome of methadone initiation or rotation for cancer pain treatment in outpatient settings.Chart review was done of 189 consecutive patients who underwent methadone initiation or rotation at the authors' palliative care outpatient center. Data were collected regarding demographic and clinical characteristics, symptoms, and opioid side effects at baseline and for 2 follow-up ...

Abstract Chondrosarcoma (CSA) of the skull base (SB) is an uncommon, slowly growing, neoplasm comprising approximately 0.1% of all
intracranial tumors and 6% of SB lesions. Even though its growth is slow, CSA is a potentially lethal tumor. The therapeutic
approach to CSA of the SB is still controversial and clinical experience is limited because of the relative rarity of this
tumor. The use of proton therapy (PT) after maximal surgery is widely accepted, but there are no controlled studies demonstrating
the need of PT and its superiority in comparison to radiotherapy with photons. We conducted a systematic r...

Conclusion. GPs caring for patients dying at home encountered complex clinical dilemmas in end-of-life care (including palliative sedation therapy and euthanasia). They valued practical advice from, and open discussion with, GP advisors. The advice often helped the GP find solutions to the patient's problems that did not require deliberately foreshortening life. (Source: Family Practice)

Palliation of symptoms to optimize QOL is the foundation of cancer care regardless of stage of disease or level of anticancer treatment. Patients commonly experience pain, constipation, nausea, vomiting, dyspnea, fatigue, and delirium. Many valid clinical tools are available to the primary care clinician to screen for symptoms, assess severity, measure treatment response, and elicit the patient's subjective symptom experience. Although there is limited evidence regarding the relative efficacy of symptom interventions from randomized controlled trials, clinical practice guidelines are available.

This literature also documents widespread lack of understanding about dying and palliative care and fears of abandonment by health care providers. Despite advances in the care of infants, there remain many newborns whose medical conditions are incompatible with sustained life. At times, healthcare providers and parents may agree that prolonging life is not an appropriate goal of care, and they may redirect treatment to alleviate suffering. While pediatric palliative treatment protocols are gaining greater acceptance, there remain some children whose suffering is unrelenting despite maximal efforts. Due to the realization that some infants suffer unbearably (ie, the burdens of suffering outweigh the benefits of life), the Dutch have developed a protocol for euthanizin...

Conclusions Patients do not always think that their suffering is unbearable, even if they have a lasting wish to die. Physicians seem to have a narrower perspective on unbearable suffering than patients and than case law suggests. In an attempt to solve the problem of different perspectives, physicians should take into account the different aspects of suffering as described in the literature and a framework for assessing the suffering of patients who ask for euthanasia. Discussion
Health care providers often view the patient through the medical lens and may not see the full view of all aspects of patients and families lives. Even many who work in community centered locations may not be aware of the breadth of services wanted and needed by families. Medical and community services are diverse in every location and often change as funding becomes available or dries up.
One of the most difficult tasks for a health care provider (and patients and families) is to move to a new location and need to locate and access various services. Health care providers who take care of patients and families ...
