This post-approval, open-label trial (n = 1266) assessed the efficacy of efalizumab, administered in accordance with the European label at that time, in patients with concomitant nail, scalp or palmoplantar psoriasis. Patients received subcutaneous efalizumab 1.0 mg/kg weekly for up to 20 weeks. By Week 12, an improvement from baseline of 50% or more was observed in 21.4% (181/844) of patients with nail psoriasis, 62.4% (718/1150) of patients with scalp psoriasis, and 51.4% (127/247) of patients with palmoplantar psoriasis. To the Editor: Balanitis is an inflammatory condition of the glans penis that may be caused by a wide range of dermatologic conditions, including psoriasis. Psoriatic balanitis is clinically characterized by erythematous, nonscaling plaques most commonly located proximally on the glans and under the prepuce. Most often, genital psoriasis is part of a more generalized disease process, but in the case of exclusively penile involvement, the correct diagnosis may be difficult to make, and several investigations, including a skin biopsy, are often necessary. CONCLUSIONS: Moderate-to-severe psoriasis is associated with a significant impact on the quality of life and at least 4-fold higher costs than mild psoriasis, indicating the need for efficient control of the disease. This cost-of-illness study provides specific health economic data for future healthcare decision making, particularly with the advent of new therapeutic agents for effective psoriasis control.

Increased levels of the proinflammatory cytokine TNF play a major role in this phenotype. Here we define the underlying molecular mechanism using genetic mouse models. Palmoplantar pustular psoriasis (PPP) is an uncommon form of chronic psoriasis. Characterized by sterile, intraepidermal pustules located on the palms and soles, it is highly resistant to treatment. Patients with AS have an increased risk of clinical vertebral fracture but not of non-vertebral fractures, while the risk of any clinical fracture is increased in patients with concomitant inflammatory bowel disease. Conclusions. The above data confirm previous reports that treatment with infliximab is an efficacious and safe option for patients with moderate to severe plaque psoriasis (and/or arthritis). Long-term follow-up, continued pharmacovigilance, and controlled comparative studies will be required to fully evaluate its use in psoriasis.

A 33-year-old man presented with verrucous, red-brown papules, which coalesced into linear plaques on the right buttock, waist, thigh, and genitals. A skin biopsy was consistent with psoriasis, and the unilateral distribution in a linear pattern led to a diagnosis of linear psoriasis, which is a rare variant of psoriasis. Although histopathologically it can be difficult to distinguish from inflammatory linear verrucous epidermal nevus (ILVEN), linear psoriasis presents in adulthood and responds to conventional topical antipsoriatic therapies.
