In this commentary, the author discusses different approaches to the primary prevention of cardiovascular disease (CVD), now that the cost of statins has decreased and there is a large amount of data available on their efficacy and safety. They ask whether "for an inexpensive treatment, known to be safe and effective, should the eligibility be widened?" They also ask what the consequences of the new vascular checks - a 'systematic risk-based approach' - are, and how they compare with alternatives.
They conclude that "scant information is available on the cost and the public a...

Abstract There is presently no vaccine to combat African swine fever (ASF), a viral hemorrhagic fever of domestic pigs that causes
up to 100% morbidity and mortality in naive, commercial pig populations. In its endemic setting, ASF virus cycles between
asymptomatic warthogs and soft ticks, with persistence in exotic locations being ascribed to the almost global distribution
of susceptible soft tick and suid hosts. An understanding of the role played by diverse hosts in the epidemiology of this
multi-host disease is crucial for effective disease control. Unlike the intensively studied Ornithodoros tick vector...

A Monte Carlo-based procedure has been developed to assess the shielded fetal doses from 6 MV external photon beam radiation treatments and improve upon existing techniques that are based on AAPM Task Group Report 36 (TG-36). Anatomically realistic models of the pregnant patient representing 3- and 6-mo gestational stages were implemented into the MCNPX code together with a detailed accelerator model that is capable of simulating scattered and leakage radiation from the accelerator head. The phantom was shielded using suggested lead and Cerrobend in different locations and with different thicknesses. Absorbed doses to ...

Approaches for the risk assessment of carcinogens in food have evolved as scientific knowledge has advanced. Early methods allowed little more than hazard identification and an indication of carcinogenic potency. Evaluation of the modes of action of carcinogens and their broad division into genotoxic and epigenetic (non-genotoxic, non-DNA reactive) carcinogens have played an increasing role in determining the approach followed and provide possibilities for more detailed risk characterisation, including provision of quantitative estimates of risk. Reliance on experimental animal data for the majority of risk assessments...

Abstract Therapeutic monoclonal antibodies are potent new tools for a molecular targeted approach to modify the course of multiple

in the treatment of MS. It is well tolerated, although severe side effects have been reported that have conditioned its use
as a second-line drug for the treatment of MS. The clinical benefit of natalizumab should be weighed carefully against the
potential risk of serious adverse events. Therefore, risk management plans have already been developed in order t...

Integration of complementary therapies in maternity practice is an ever growing field. Midwives have burgeoning interest in and are increasingly using complementary therapies within their practice. Issues regarding knowledge and safety remain essential within midwifery care. It is possible that the use of complementary therapies during pregnancy is not always based on a foundation of adequate knowledge and therefore may not be completely safe. Risk management is essential, both within conventional maternity care and where there is integration of new practices, to ensure that client safety is paramount. In order for int...

Novel biomarkers of kidney toxicity are powerful tools not only with respect to their clinical applications but also because of their impact on drug development. These biomarkers can influence the assessment of efficacy of new drugs for kidney diseases as well as the risk management for new drugs. The science behind these novel biomarkers reflects the evolution over the past decade of genomic and proteomic platforms that have transformed the discovery and development of new biomarkers for preclinical and clinical applications in drug development. Several of these biomarkers are in use as transcriptomic biomarkers in an...

Population mobility is a main factor in globalization of public health threats and risks, specifically distribution of antimicrobial drug-resistant organisms. Drug resistance is a major risk in healthcare settings and is emerging as a problem in community-acquired infections. Traditional health policy approaches have focused on diseases of global public health significance such as tuberculosis, yellow fever, and cholera; however, new diseases and resistant organisms challenge existing approaches. Clinical implications and health policy challenges associated with movement of persons across barriers permeable to products...

DISCUSSION: Pandemic planning and preparedness in general practice is ultimately a crucial risk management exercise, the cornerstone of which is sound infection control. As planning will be significantly aided by, and should extend to, other respiratory outbreaks, we propose a framework for managing outbreaks of respiratory infections with a focus on planned, practised and habitual infection control measures, and a stepwise response according to the extent and severity of the outbreak.

Evidence regarding the effectiveness and efficiency of safety analysis is scarce. No studies on mitigation were included. Abstract Since its advent, predictive DNA testing has been perceived as a technology that may have considerable impact on the quality
of people’s life. The decision whether or not to use this technology is up to the individual client. However, to enable well
considered decision making both the negative as well as the positive freedom of the individual should be supported. In this
paper, we argue that current professional and public discourse on predictive DNA-testing is lacking when it comes to supporting
positive freedom, because it is usually framed in terms of risk and risk management. We show how this ...

Abstract: The PREM Bank has been providing pasteurised donor human milk (PDHM) to very preterm for the past 3 years. It is the first human milk bank (HMB) to operate in Australia in over 20 years. Our community has rapidly embraced the concept of human milk banking, with both donations and demand for PDHM exceeding expectations. Providing PDHM in ‘exceptional circumstances’ where a mothers’ own milk is unavailable is supported by the WHO and UNICEF. We submit that neonatal intensive care is an exceptional circumstance. Although evidence supporting PDHM use from randomised control trial (RCT) is limited, the latest ...

CONCLUSIONS: Emerging patterns imply that pharmacy staff response may vary according to the medicine requested. It may be that pharmacists and their staff prioritize drugs or behaviors that they consider critically relevant as part of a risk management approach. This has possible implications for future scheduling policy. Further research using a range of scenarios would more fully explore the breadth of such behavior. This would aid identification of medicines that trigger professional intervention and inform development of targeted training programs.

Electronic health records (EHRs) are here to stay, and their use is expected to grow significantly in the future. It is incumbent on risk managers to ensure their use does not inhibit good provider-to-patient communication. Providing education and strategies on how to make the patient encounter meaningful while the provider uses an EHR may increase patient satisfaction and may contribute to a favorable professional liability loss experience. Risk managers also need to have an understanding of the capabilities of each specific EHR system in the facility, the implications of the ability to cut-and-paste information from one ...

Exposure to neurotoxic agents represents a concern of high priority in modern society. The nervous system can compensate for the toxic effects caused by low doses, but prolonged and lifetime exposure can also lead to delayed neurodegenerative effects for very low levels. The reported frequency of neurodegenerative diseases is constantly increasing, which results in serious negative impacts on the health of the population worldwide. Future estimates indicate that Parkinson's Disease will increase mostly in industrialized countries during the next few years. Exposure to neurotoxic agents can take place during the various...
