Advance care planning for cancer patients in primary care: a feasibility study.

Conclusion A clear divide was found between UK policy directives and delivery of end-of-life care in the community that educational interventions targeting primary care professionals are unlikely to address. Advance care planning has the potential to promote autonomy and shared decision making about end-of-life care, but this will require a significant shift in attitudes.

Abstract: Five laboratories in the Chronic Lymphocytic Leukemia (CLL) Research Consortium (CRC) investigated standardizing and pooling of fluorescence in situ hybridization (FISH) results as a collaborative research project. This investigation used fixed bone marrow and blood cells available from previous conventional cytogenetic or FISH studies in two pilot studies, a one-day workshop, and proficiency test. Multiple FISH probe strategies were used to detect 6q-, 11q-, +12, 13q-, 17p-, and IGH rearrangements. Ten specimens were studied by participants who used their own probes (pilot study 1). Of 312 FISH interpretations, ...

The number of treatment options available to patients with breast cancer is larger and more complex than ever before. This is due in part to increased understanding of breast cancer tumor biology and the signaling pathways involved in tumor development and progression, which drives new areas of breast cancer research and the development of novel agents. Therapies targeting HER2 signaling, angiogenesis, DNA repair, and many other essential cellular processes that are dysregulated in cancer have produced significant improvements in disease outcome, although careful patient selection and toxicity management are required t...

The purpose of this study was to describe the effect of an intervention program on nurses’ HIV-related knowledge and attitudes in Lithuania. The program focused on HIV epidemiology, transmission, coinfections, treatment and care, risk contacts, and moral and ethical dilemmas. It was designed to increase nurses’ knowledge and positive attitudes concerning HIV. The study used a randomized controlled trial design with two experimental groups ([EG]; EG1, n = 63, EG2, n = 63) and one control group (n = 59) in three Lithuanian hospitals. Data were collected using a questionnaire developed by Held (1993). The questionnaire ...

Conclusions Teaching evidence‐based practice skills and concepts to allied health students within a multi‐professional university curriculum improved confidence and perceived and actual knowledge regarding evidence‐based practice. Further research is needed to determine if these changes result in long‐term behaviour change once students graduate, and to consider optimal methods for multi‐ and interprofessional delivery of evidence‐based practice training.

I recently delivered a sex-education workshop to a freshman class at a college in New York City. During the workshop, I handed out blank pieces of paper and asked the students to write down any questions they may have about sex. In an attempt to elicit honest questions without fear of embarrassment, all questions were anonymous. As I walked around the room to collect the folded up papers, I noticed that approximately three quarters of the male students had not written anything down, while every female student had written down at least one question. This contrast was so stark the class addressed it without my prompting. One...

Agreement about the terminology and descriptions of fetal heart rate (FHR) patterns (nomenclature) is now well established, largely based on the report of the National Institute of Child Health and Human Development (NICHD) workshop of 1997, but consensus on FHR interpretation and management has been extraordinarily difficult to achieve in US obstetrics. Interpretation deals with the significance for the fetus in terms of risk of potentially damaging metabolic acidemia. It is also now understood that part of this interpretation is recognizing or projecting the probability of a pattern of lower risk of acidemia evolving int...

AbstractPenalized likelihood methods have become increasingly popular in recent years for evaluating haplotype‐phenotype association in case‐control studies. Although a retrospective likelihood is dictated by the sampling scheme, these penalized methods are typically built on prospective likelihoods due to their modeling simplicity and computational feasibility. It has been well documented that for unpenalized methods, prospective analyses of case‐control data can be valid but less efficient than their retrospective counterparts when testing for association, and result in substantial bias when estimating the haplotyp...

"The PEN is mightier than the sword" - Edward Bulwer-Lytton, playwright (1839)If your family is like mine, the holidays are a time for stretching our capacities: for eating, of course, but also for patience, love, and harmonious co-existence.That's because, from politics to sports to child-rearing, there is so much potential for both disagreement and discovery - for both gaining and losing connection. If we are lucky, we emerge on the other side of December feeling warmer and closer to each other, our hearts filled with bounty. If unlucky, we stagger into the New Year with our emoti...

Lily Asquith: Experiment vs Theory at an LHC workshop. Now it's serious.I've spent the last 48 hours enthralled by the goings on at the Argonne "Analysis Jamboree". I'm not really much of a one for these things... my more senior colleagues doze and my more junior colleagues tap loudly on their laptops; facebooking, writing code, who knows. Sometimes I wonder what the senior ones dream about - particularly in the weekly lunchtime seminars, where the average age of an attendee is about 2,609.I thought I would have to make an effort to concentrate on the endless hours of men reading off slides projected onto the end wall; pro...

I'm filled with such gratitude for him and what he has awakened in me that the words from the hymn now symbolize what has happened in my heart. My scarlet letter glitters with what I take to be long-forgotten joy. When I call, I know I need not worry about disturbing his...
