Chameleon ™ PTA balloon catheter: A single device in managing thrombosed AV access
J Vasc Access. 2021 Jun 23:11297298211027477. doi: 10.1177/11297298211027477. Online ahead of print.ABSTRACTThrombectomy is a common procedure for maintenance of arteriovenous (AV) access and is critical to prolong access life. Techniques for performing thrombectomy are incredibly diverse, ranging from open surgical procedures to percutaneous interventions. Percutaneous interventions include a combination of thrombectomy devices to clear the thrombus and balloon angioplasty to treat the underlying lesion. In this case report we describe a novel technique using a single device, the Chameleon™ PTA balloon catheter (Medtron...

Guide catheter extension lock enables the strongest backup force during the antegrade approach in percutaneous coronary intervention
AbstractSufficient guide catheter backup is crucial for successful percutaneous coronary intervention (PCI). We have developed a new technique for locking a small child catheter to the vessel wall by balloon dilatation outside the child catheter at the proximal site of the main branch. We call this the “guide catheter extension lock” (“Kiwami® lock ”, Kiwami® child catheter, Terumo Corp. Tokyo, Japan). The guide catheter extension lock backup force was evaluated in an experimental model, and we report a case of chronic total occlusion treated with PCI using this technique. The guide catheter backup force was meas...

An athletic 30-something woman with acute substernal chest pressure
Post by Smith, with short article by Angie Lobo (https://twitter.com/ALoboMD), a third year intermal medicine resident at Abbott Northwestern Hospital CaseA 30-something woman with no past history, who is very fit and athletic, presented with 1.5 hours of substernal chest pressure.  It was non-radiating and without other associated symptoms except for nausea.  She had zero CAD risk factors. Here was her ECG at time zero:What do you think? There is ST elevation in V2 with large fat T-wave.  There is ST depression in II, III, and aVF, and V3 to V6.I saw this before any other information and k...

Chest pain, Ventricular Paced Rhythm, and a Completely Normal Angiogram 3 Months Prior.
One of our graduates, Rochelle Zarzar, who is now an education fellow, sent me this from one of the hospitals she works at now:An elderly woman presented with chest pain.  She had been nauseous the night before and did not feel well, then awoke 2 hours prior with chest pain. She had had a completely normal angiogram 3 months prior. Here is that angiogram report:The left main coronary artery is normal.Left anterior descending is a type 3 vessel and is normal.Left circumflex is nondominant and normal. The right coronary artery is dominant and normal. The nurses immediately recorded an ECG.  This was 2 hours after the o...

Cavoiliac Thrombectomy with Use of Snare  Retrieval Technique
A 47-year-old woman with bilateral lower-extremity swelling and pain presented for evaluation 2 months after placement of a G ünther Tulip inferior vena cava (IVC) filter (Cook, Bloomington, Indiana) for the treatment of right lower-extremity deep vein thrombosis. Anticoagulation was contraindicated as a result of concurrent gastrointestinal bleeding. Venography revealed extensive subocclusive thrombus within the IVC and bilateral common iliac veins (Fig 1). Balloon maceration of the thrombus in the IVC and iliac veins with 10-mm semicompliant balloon angioplasty (Evercross; Medtronic, Minneapolis, Minnesota) was unsucces...

Sex-based Differences in Quality of Care and Outcomes in a Health System Using a Standardized STEMI Protocol
Conclusions Previously reported treatment disparities between men and women are diminished in a regional PCI-based STEMI system using a standardized STEMI protocol. No sex differences in short-term or long-term age-adjusted mortality are present in this registry despite some treatment disparities. These results suggest that STEMI healthcare disparities and mortality in women can be improved using STEMI protocols and systems.

Rupture of Third Dorsal Metatarsal Artery after Balloon Angioplasty and Its Management
An 80-year-old man with pain and discoloration of the left third toe was referred for lower extremity arteriography. Endovascular recanalization of a completely occluded dorsalis pedis artery had been performed 5 months ago to improve a nonhealing ulcer. The ulcer healed; however, he presented again with the current symptoms. Arteriography demonstrated diffuse stenosis of the third dorsal metatarsal artery proximal to bifurcation of the third and fourth digital arteries (Fig 1a, b, arrows). Angioplasty of the distal dorsalis pedis and third dorsal metatarsal artery was initially performed with a semicompliant 2 × 80 mm ba...

A Randomized Study of SheathLess vs Standard Guiding Catheters for Transradial Percutaneous Coronary Interventions
Conclusions In selected coronary lesions requiring large-bore catheters in men and in all lesions in women, the SheathLess GC was superior to the standard GC for successful transradial PCI with the designated GC. The SheathLess GC was also associated with easier arm navigation and less patient discomfort.

Efficacy and Safety of the GuideLiner Mother ‐in‐Child Guide Catheter Extension in Percutaneous Coronary Intervention
ConclusionsUse of the GuideLiner catheter facilitated successful completion of PCI procedures in a majority of patients with complex lesions. Operators should be aware of the potential complications associated with GuideLiner use.

Forward and back aspiration during ST-elevation myocardial infarction: a feasibility study.
CONCLUSIONS: The strategy of forward and back aspiration to facilitate stent implantation and high-pressure post-dilatation during STEMI appears to be safe and effective. Randomised controlled trials are required to confirm the safety and efficacy of this newly devised angiographic strategy. PMID: 27056124 [PubMed - in process]
