Frequency of HLA-DR+CD38hi T cells identifies and quantifies T-cell activation in hemophagocytic lymphohistiocytosis, hyperinflammation, and immune regulatory disorders
Evaluation of T-cell activation is essential for the diagnosis of numerous hyperinflammatory and immune regulatory disorders, including hemophagocytic lymphohistiocytosis (HLH).1-6 Currently, plasma levels of soluble IL-2 receptor (sIL-2R) or sCD25/ αchain of the IL-2R complex are extensively used as a biomarker for measuring T-cell activation.7 Its role in the assessment of disease severity and monitoring has been established in a spectrum of hyperinflammatory disorders (HLH, multisystem inflammatory syndrome in children [MIS-C]),4,8,9 autoim mune disorders,10 infections (eg, HIV, EBV, coronavirus disease 2019 [COVID-19]...

Autosomal Dominant STAT6 Gain of Function Causes Severe Atopy Associated with Lymphoma
AbstractThe transcription factor STAT6 (Signal Transducer and Activator of Transcription 6) is a key regulator of Th2 (T-helper 2) mediated allergic inflammation via the IL-4 (interleukin-4) JAK (Janus kinase)/STAT signalling pathway. We identified a novel heterozygous germline mutationSTAT6 c.1255G  >  C, p.D419H leading to overactivity of IL-4 JAK/STAT signalling pathway, in a kindred affected by early-onset atopic dermatitis, food allergy, eosinophilic asthma, anaphylaxis and follicular lymphoma. STAT6 D419H expression and functional activity were compared with wild type STAT6 in transduced H EK293T cells and to ...

Neurosarcoidosis: Diagnostic Challenges and Mimics A Review
AbstractPurpose of ReviewNeurosarcoidosis is a rare manifestation of sarcoidosis that is challenging to diagnose. Biopsy confirmation of granulomas is not sufficient, as other granulomatous diseases can present similarly. This review is intended to guide the clinician in identifying key conditions to exclude prior to concluding a diagnosis of neurosarcoidosis.Recent FindingsAlthough new biomarkers are being studied, there are no reliable tests for neurosarcoidosis. Advances in serum testing and imaging have improved the diagnosis for key mimics of neurosarcoidosis in certain clinical scenarios, but biopsy remains an import...

The Role of Positron Emission Tomography for the Management of Sinonasal Malignancies: A Systematic Review
CONCLUSIONS: In general, and across all types of sinonasal malignancies, PET scan yielded positive results regarding detection and initial staging. It was also considered as the modality of choice for detection of distant metastases, except in the case of sinonasal lymphoma. PET scan's main limit resides in its inability to detect lesions in or close to the metabolic activity of the brain.PMID:37229633 | DOI:10.1177/19458924231177854

New Presentation of CD27 Deficiency; Coronary Ectasia and COVID-19
Iran J Allergy Asthma Immunol. 2023 Feb 20;22(1):110-118. doi: 10.18502/ijaai.v22i1.12013.ABSTRACTCD27 is a costimulatory receptor involved in the maturation of the innate and adaptive immunity. CD27, through interaction with CD70, plays a role in the control of Epstein-Barr virus (EBV) infection. CD27 deficiency leads to an immune dysregulation disease characterized by EBV susceptibility. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) might put patients with primary immunodeficiency at risk for adverse outcomes. Chromogenic in situ hybridization (CISH) study was performed to detect EBV in the lymphoma tissue...
