Emergence of Marburg virus: a global perspective on fatal outbreaks and clinical challenges
The Marburg virus (MV), identified in 1967, has caused deadly outbreaks worldwide, the mortality rate of Marburg virus disease (MVD) varies depending on the outbreak and virus strain, but the average case fatality rate is around 50%. However, case fatality rates have varied from 24 to 88% in past outbreaks depending on virus strain and case management. Designated a priority pathogen by the National Institute of Allergy and Infectious Diseases (NIAID), MV induces hemorrhagic fever, organ failure, and coagulation issues in both humans and non-human primates. This review presents an extensive exploration of MVD outbreak evolu...

Changing with the times —learning to pivot as an academic allergist
In 535 BCE, Heraclitus said “Change is the only constant in life.” As changes continue in allergy and immunology (AI) with respect to practice types, models and patient complexity, it is critical that trainees are provided the skills needed to navigate this ever-changing environment. The article by Dr Zafra and Dr Chiu ill ustrates the changes in recruitment, cost of medical education, salaries, and ACGME standards and wellness.1 We like to take this a step further focusing on the many domains of wellness: emotional, spiritual, intellectual, social, physical, environmental, financial, and occupational.

Everything you wanted to know about anaphylaxis, but were afraid to ask
This issue of the Annals poses the challenge of “Everything you wanted to know about anaphylaxis, but were afraid to ask.” The featured content takes a provocative look at anaphylaxis, challenging the paradigm of how this is treated, and how we can evolve care. Anaphylaxis is the one true medical emergency that we are all trained, from the fi rst day of starting fellowship, to learn to manage, master, and respond to as an automatic reflex. However, as automatic as the response is to inject intramuscular epinephrine, there is inherent variation in practice, and still much left to be desired in our understanding of how a...

Arerugi. 2023;72(5):453-462. doi: 10.15036/arerugi.72.453.ABSTRACTBACKGROUND/PURPOSE: Anaphylactic shock is a serious and life-threatening condition, and affected patients should be quickly and effectively treated with an EpiPen. Although the correct use of an EpiPen is greatly affected by a user's proficiency level and the instructions accompanying the EpiPen, there has been almost no investigation into the knowledge of the EpiPen and the actual situation of the accompanying instructions for use. Therefore, we conducted this nationwide survey to elucidate these issues.METHODS: A questionnaire survey was conducted among ph...
