Outpatient Vascular Care: Good, bad or ugly?
BY ANISH KOKA Filling in the holes of recent stories in the New York Times, and Propublica on the outpatient care of patients with peripheral arterial disease Most have gotten used to egregiously bad coverage of current events that fills the pages of today’s New York Times, but even by their now very low standards a recent telling of a story about peripheral artery disease was very bad. The scintillating allegation by Katie Thomas, Jessica Silver-Greenberg and Robert Gebeloff is that “medical device makers are bankrolling doctors to perform artery clearing procedures that can lead to amputationsâ€...

This case was provided by Steven Souchtchenko, a recent graduate of our Hennepin Healthcare EM/IM residency (i.e., a former trainee of mine).CaseA 45 yo man with no previous cardiac history presented to an ED not associated with a cath lab.  He complained of jaw pain radiating to left shoulder for 6 hours.  He stated he had had a brief similar episode the evening prior. Here is his ED ECG.What do you think? When this was shown to me I immediately said " Proximal LAD OMI " .  Then I ran it through theQueen of Hearts AI app and she said " OMI with high confidence " .I showed it to Pendell, who said: " Clear...

Foobaw and more
Let me start with Damar Hamlin. His physicians haven ' t said anything publicly about what happened to him, but there are basically two possibilities. First, it is obviously uncommon but not unheard of for apparently healthy athletes to suffer cardiac arrest during exertion. This happened to Boston Celtics star Reggie Lewis. I happened to be in Boston Garden watching the first round playoff game against the Charlotte Hornets on April 29, 1993 when Lewis collapsed. All of the spectators were baffled about what  had happened.  Doctors at New England Baptist Hospital later diagnosed him with a heart abnormality...

The duality of being a female physician
I am a female cardiologist that graduated from medical school almost 20 years ago. Although my core personality has remained constant, I have been viewed through multiple different optics. What I find most interesting is that while most of my patients would unanimously agree that I am a smart, talented, and dedicated professional, I have Read more… The duality of being a female physician originally appeared in KevinMD.com.

What are the signs of blood clots in the leg? Cardiology Basics
Signs of blood clots in the leg depend on whether the clot is in the artery or the vein. When the artery is blocked, it produces pain, pallor, coldness of the leg, loss of sensation and paralysis. If the occlusion of the artery persists for a long period, the limb will become black with death of cells, called as gangrene. This occurs when the block occurs abruptly. In a gradually progressing obstruction, one first notes pain in the legs on walking (claudication). This pain subsides with rest and recurs on walking. Distance at which pain starts is an important indication of the severity of obstruction. More severe obstruct...

A Middle Aged Male diagnosed with Gastroesophageal Reflux
This middle aged male with h/o GERD but also h/o stents presented to the ED with chest pain.  He had been at a clinic that day where he had complained of worsening GERD.  An EKG was recorded and interpreted as normal by the computer, the clinician, and by the overreading cardiologist. He had an ECG recorded in triage (I am not certain whether the patient had active pain at this time; I believe he didnot):What do you think? Here is the patient ' s ECG from several hours ago (which was essentially the same):This shows minimal inferior ST Elevation that is howeverall but diagnostic of inferior ischemia. There is the o...

Interventionalist at the Receiving Hospital: " No STEMI, no cath. I do not accept the transfer. "
Are Some Cardiologists Really Limited by Strict Adherence to STEMI millimeter criteria? Yes. We don ' t know how many though.I was texted these ECGs by a recent residency graduate after they had all been recorded, along with the following clinical information:A 50-something with no cardiac history, but with h/o Diabetes, was doing physical work when he collapsed. He was found in ventricular fibrillation and defibrillated, then brought to a local ED which does not have a cath lab. Here is the initial ED ECG:This is pretty obviously and inferior posterior OMI, right? There is slight inferior ST Elevation, with reciprocal ST de...

What ’ s the relationship between JVP and PCWP/LVEDP?
This study elegantly shows a correlation (or lack of it) in different subsets of heart failure. It tells us very clearly If JVP(RAP) is not correlating or disproportionate to PCWP, it implies RV dysfunction. 2. This paper suggests a really useful scheme to classify heart failure as concordant and discordant with reference to right and left heart. It throws some interesting facts. I guess it will help us guide diuretic management and prognosticate chronic heart failure.

A Vigilante in Statistical Badlands
This study, for instance, attributes a patient’s waitlist/transplant outcome to the very last dialysis facility the patient was associated with.  In epidemiology speak, this means the causal inference authors are trying to draw between for-profit status and good transplant outcomes is subject to time-varying confounding. As an example, if one is seeking an association between testosterone levels and risk of a heart attack, using the last testosterone level available would be a poor way of doing this study because testosterone levels are known to vary over time. The same applies to dialysis facilities. Patients chan...

60-something with 2 days of intermittent epigastric pain. Why does the cardiologist disagree?
Conclusion An invasive strategy based on coronary stenting with adjunctive use of abciximab reduces infarct size in patients with acute STEMI without persistent symptoms presenting 12 to 48 hours after symptom onset.===================================MY Comment by KEN GRAUER, MD (10/20/2019):===================================I find it difficult to believe that the Cardiologist Attending On Call did not allow for the possibility that the OMI in this case was either acute and/or ongoing.MY Question: Was this the opinion of the Attending Cardiologist  — or a Resid...
