The boys are always prepared
I live in eastern Connecticut, not far from the Coast Guard Academy. For those who don ' t know it ' s in New London, actually across the street from Connecticut College, which my cousin attended and a friend of mind taught at, so I ' ve been in the area a few times. The Academy is a source of pride to our state and one of a few important symbols of our maritime tradition. Actually I drive right past it whenever I ' m headed to Mystic and points nearby. The Mystic Seaport museum is another of those important symbols -- home, among other rare artifacts, to the only surviving wooden whaling ship, the Charles W. Morgan. ...

Like Don Quixote Chasing Windmills, I Aim to See Financial Remediation for Patients Still Suffering in a Predatory U.S Healthcare System
Sometimes, I feel (if you pardon the metaphor) a bit like Don Quixote chasing windmills with my efforts to see true financial remediation introduced in the predatory U.S. healthcare " system " (and I am glad to see that the term " predatory " is now being used by a growing number of medical doctors, which I think is great). I envision a type of financial remediation whereby patients will be able to get access to the care they need without having to face financial predators trying to pick their pockets with illegal discount bribes standing in the way of their getting access to things they need like insulin, CGM senors ...

Slideshow: Breathtaking Brains
The average human brain is only about 3 pounds, but this complex organ punches well above its weight, acting as the control center for the whole body. Many of the brain’s intricacies still aren’t fully understood. To gain more insight into brain processes, scientists often peer into the brains of research organisms such as fruit flies and mice. These organisms have shed light on how our brains maintain circadian rhythms, how neuropsychiatric disorders develop, and more. Here, we feature images and videos that NIGMS-supported researchers have created while investigating the secrets of the brain in research organi...

Betting Against Progress Turns Out Poorly, But Can Work in the Short Term in a Slow Field
Setting oneself up as a spokesperson for "we will not achieve this goal", as the fellow noted here is choosing to do, is a bet against technological progress. A glance at any few decade period in the past two hundred years suggests that such a bet will almost certainly fail in time, sometimes quite rapidly. In highly regulated fields that move as slowly as is the case for medicine, however, one can profitably continue to be a skeptic for quite some time. While progress is rapid and impressive in the lab and in animal studies, a skeptic can continue to shrug and point to the lack of human therapies. This is the resul...

We are Still Learning the Lesson Charles Babbage Taught Us in 1821
Back in 1821 when Charles Babbage introduced the world to his Difference Engine, one of the world’s first mechanical computers, he taught us that bad input = bad output. This is a lesson we are still learning today in healthcare. As we leap into the world of artificial intelligence (AI), machine learning (ML), and large language models, we would do well to remember this lesson before relying too heavily on the output of these fantastical technologies. At the recent HIMSS23 Conference in Chicago, Charlie Harp, CEO of Clinical Architecture – a company that provides solutions for healthcare data quality, interoperabil...

Electric Cars: Policy Beyond Capability?
Peter Van DorenThe Environmental Protection Agency (EPA) recentlyannounced proposed emission standards that would mandate a  large increase in the sale of new zero‐emission vehicles from model years2027 through 2032. Compliance with the proposed rule is estimated to require 67 percent of new vehicles to be electric in 2032 compared to 5.8 percent in 2022.Informed analysts claim that the rule is extremely ambitious: “The new rule will effectively try to shove electric vehicles down the throats of the public at a faster rate than it has shown a willingness to swallow them.” The Energy Information Administration i...

Rethinking Medication and Information Technology
Previous articles in this series looked at barriers to taking medication and possible solutions, including special conditions that produce challenges. This final article in the series turns the question on its head. Can patients get better without the medications? Dr. Omar Manejwala, CMO of DarioHealth, goes so far as to use the terms “paternalistic” and “infantilizing” to label claims that people fail to take medication solely out of ignorance or forgetfulness. To all the other factors that hold people back from taking their meds, he adds social and religious factors, concerns about side effects an...

Helping People Take Their Meds
Lack of adherence to prescribed medication is a major failing of our healthcare system. (Dr. Omar Manejwala, CMO of DarioHealth, recommends using the term “adherence” instead of the more condemnatory term “compliance.”) A previous article listed some alarming statistics about the number of patients who don’t take their medications as advised, and the dire effects on life expectancy as well as costs to the health care system. That previous article focused on ways to solve problems of cost and access, which in the United States and many other countries are big contributors to lack of adherence. ...
