Is low-dose naltrexone effective in chronic pain management?
J Fam Pract. 2023 Sep;72(7):320-321. doi: 10.12788/jfp.0654.ABSTRACTYES. Low-dose naltrexone is as effective as amitriptyline in the treatment of painful diabetic neuropathy and has a superior safety profile (strength of recommendation [SOR], B; single randomized controlled trial [RCT]). Low-dose naltrexone significantly reduced pain by 32% in inflammatory conditions and 44% in neuropathic conditions (SOR, B; single retrospective cohort study). Doses as low as 5.4 mg were found to reduce pain in 95% of patients with fibromyalgia (SOR, B; single prospective dose-response study).PMID:37729143 | DOI:10.12788/jfp.0654

A Randomized, Controlled Neuroimaging Trial of Cognitive ‐Behavioral Therapy for Fibromyalgia Pain
ConclusionOur results suggest clinically important and CBT-specific associations between somatosensory/motor- and salience-processing brain regions and the default mode network in chronic pain. These patterns of connectivity may contribute to individual differences (and treatment-related changes) in somatic self-awareness. CBT appears to provide clinical benefits at least partially by reducing pain-related catastrophizing and producing adaptive alterations in default mode network functional connectivity.

The Relationship between Pain Catastrophizing and Cognitive Function in Chronic Musculoskeletal Pain: A Scoping Review
CONCLUSION: Although evidence exists for the relationship between cognitive function and PC, there is a lack of rigorous research to indicate the strength of this relationship and the specific cognitive functions affected. The literature lacks appropriate populations needed to investigate clinically relevant PC and is limited by heterogeneous neuropsychological test batteries. The effect of vitamin D treatment on quality of life in patients with fibromyalgia
ConclusionWe determined that vitamin D deficiency is significantly more prevalent in patients diagnosed with fibromyalgia. Vitamin D supplementation was observed to have a positive effect on quality of life and reduction of pain.

Symptoms of complex pelvic pain: A survey in three cohorts of women
ConclusionMany women in our study reported complex pelvic pain, and overlap with other pain-related conditions, low mental health and past sexual assault was associated with high symptom burden. Those findings support taking a biopsychosocial approach to treating women who present with such complaints.

Reduced Cortico ‐Cortical Resting‐State Connectivity in Sensory Systems is related to Bodily Pain in Juvenile Fibromyalgia
ConclusionReduced cortico-cortical sensory integration involving PCL/S1 and spanning the sensory systems may underly critical pain sensory features in youth with JFM. Reduced sensory integration is paralleled by augmented crosstalk between sensory and affective/salience-processing regions, potentially indicating a shift towards more affectively colored sensory experiences to the detriment of specific sensory discrimination.This article is protected by copyright. Psychological therapies delivered remotely for the management of chronic pain (excluding headache) in adults
CONCLUSIONS: Currently, evidence about remotely-delivered psychological therapies is largely limited to Internet-based delivery of CBT. We found evidence that remotely-delivered CBT has small benefits for pain intensity (moderate certainty) and functional disability (moderate to low certainty) in adults experiencing chronic pain. Benefits were not maintained at follow-up. Our appraisal of quality of life and adverse events outcomes post-treatment were limited by study numbers, evidence certainty, or both.
