Patterns of pharmacologic and non-pharmacologic treatment, treatment satisfaction and perceived tolerability in patients with fibromyalgia: a patients' survey.
CONCLUSIONS: Patients with FM in Spain are overtreated with a combination of non-pharmacologic and pharmacologic therapies. Several of these therapies lack adequate support from randomised clinical trials and/or clinical practice guidelines. This overtreatment is not associated with relevant clinical benefits or patient satisfaction and, in the case of pharmacologic treatments, poses tolerability and safety issues. Revisiting Tramadol: A Multi-Modal Agent for Pain Management
AbstractTramadol —an atypical opioid analgesic—has a unique pharmacokinetic and pharmacodynamic profile, with opioidergic, noradrenergic, and serotonergic actions. Tramadol has long been used as a well-tolerated alternative to other drugs in moderate pain because of its opioidergic and monoaminergic activities. However, cumulative evidence has been gathered over the last few years that supports other likely mechanisms and uses of tramadol in pain management. Tramadol has modulatory effects on several mediators involved in pain signaling, such as voltage-gated sodium ion channels, transient receptor potenti al V1 channe...

Combination pharmacotherapy for the treatment of fibromyalgia in adults.
CONCLUSIONS: There are few, large, high-quality trials comparing combination pharmacotherapy with monotherapy for fibromyalgia, consequently limiting evidence to support or refute the use of combination pharmacotherapy for fibromyalgia. PMID: 29457627 [PubMed - as supplied by publisher]

Pregabalin Misuse and Abuse in Jordan: a Qualitative Study of User Experiences
AbstractPregabalin is currently approved for the treatment of epilepsy, generalized anxiety disorder, neuropathic pain, and fibromyalgia. A qualitative study was undertaken in Jordan, where concerns have been raised about its unprescribed availability in community pharmacies and thereby its abuse. Semi-structured interviews were conducted with all patients with a history of pregabalin use in two Jordanian addiction treatment centers. All were male patients aged 21 –30 years (n = 11). The majority was poly-drug abusers and had a previous history of substance abuse (tramadol, Captagon, synthetic cannabinoids, and mariju...

Changing Trends in Opioid Use Among Patients With Rheumatoid Arthritis in the United States
ConclusionIn the US, opioid use in older patients with RA peaked in 2010 and is now declining slightly. Withdrawal of propoxyphene from the US market in 2010 had minimal effect on overall opioid use, because use of propoxyphene was replaced by increased use of other opioids.

The role of tramadol in pain management in Latin America: a report by the Change Pain Latin America Advisory Panel.
CONCLUSIONS: In Latin America, tramadol is a valuable and frequently used medication for treating moderate to severe pain. More stringent regulations would have significant impact on its availability, especially for outpatients. This could cause regression to older and frequently inadequate pain management methods, resulting in unnecessary suffering for many Latin American patients. Impact of conflicts of interest on guideline recommendations : Empirical study within the second update of the German interdisciplinary S3 guidelines on fibromyalgia syndrome.
CONCLUSION: Academic COI were more frequent than financial COI in the second update of the German interdisciplinary guidelines group on fibromyalgia syndrome. The impact of COI on guideline recommendations was low. The inclusion of patients and of all relevant healthcare professionals into a guidelines group is a protective factor against the influence of COI on guideline recommendations. Fibromyalgia Outcomes Over Time: Results from a Prospective Observational Study in the United States.
CONCLUSION: Fibromyalgia patients reported high levels of burden at both time points, with few significant changes observed over time. Outcomes were variable among patients over time and were better among those with greater pain improvement. PMID: 28077978 [PubMed - in process]

Management of fibromyalgia: practical guides from recent evidence-based guidelines.
Authors: Häuser W, Ablin J, Perrot S, Fitzcharles MA Abstract Fibromyalgia (FM) is a prevalent and costly condition worldwide, affecting approximately 2% of the general population. Recent evidence- and consensus-based guidelines from Canada, Germany, Israel and the European League Against Rheumatism aim to support physicians in achieving a comprehensive diagnostic work-up of patients with chronic widespread pain (CWP) and to assist patients and physicians in shared decision making on treament options. Every patient with CWP requires at the first medical evaluation a complete history, medical examination and some l...

Effect of intravenous lidocaine combined with amitriptyline on pain intensity, clinical manifestations and the concentrations of IL‐1, IL‐6 and IL‐8 in patients with fibromyalgia: A randomized double‐blind study
ConclusionsThe combination of 240 mg of intravenous lidocaine (once a week for 4 weeks) with 25 mg of amitriptyline for 8 weeks had no meaningful impact in fibromyalgia patients.

Treatment Initiation Timing and Healthcare Costs in Newly Diagnosed Patients with Fibromyalgia (P6.276)
Conclusions:Total healthcare costs were significantly lower among early initiation patients, suggesting early use as a potential cost-saving strategy. FM patients had substantial comorbidity and most did not receive ACR recommended medications. Study Supported by: Daiichi Sankyo, Inc.Disclosure: Dr. Frech-Tamas has nothing to disclose. Long-term Application of Opioids for Non-Tumor Associated Pain - Impact of the S3 Guidelines for the Field of Orthopaedic Surgery and Trauma Surgery.
Discussion: Opioid analgetics are a treatment option for particular pain syndromes (arthritis, back pain). However, the latest S3 guidelines emphasise patient education and therapy. The indication, effects, side effects and therapeutic objectives should be regularly assessed, both before and during treatment. Once therapeutic goals have been achieved, measures such as physical therapy, physiotherapy or psychotherapy should be continued, whereas opioid dosage should be gradually reduced. Opioid-induced hyperalgesia and dopamine-induced augmentation in an intractable and refractory case of RLS
Dopamine-induced augmentation (DIA) has been reported in many patients with restless legs syndrome/Willis-Ekbom disease (RLS/WED) [1] but there are only two reports [2,3] of DIA-like features in RLS patients on long-term tramadol (a synthetic analog of codeine) but not in RLS patients on other opioids. Herein I report an intractable and refractory RLS/WED patient showing augmentation-like features after long-term dopamine agonist and multiple opioids. A 62-year-old woman fulfilling the diagnostic criteria [4] of RLS for 23 years and comorbid fibromyalgia, diabetes mellitus, hypothyroidism, obstructive sleep apnea, and morb...
