Extrinsic lipoid pneumonia due to chronic polyethylene glycol consumption: A case report
Respir Investig. 2023 Sep 14;61(6):768-772. doi: 10.1016/j.resinv.2023.08.002. Online ahead of print.ABSTRACTExtrinsic lipoid pneumonia (ELP) results from the aspiration of lipid-containing substances. Tissue or cell histopathology after Oil-Red-O staining can confirm the diagnosis, which requires proper tissue handling and preparation during bronchoscopy. Here, we report a case of ELP in a quadriplegic patient with a long history of dysphagia and polyethylene glycol consumption. Computed tomography (CT) of the chest revealed multiple, progressively enlarging, fat-attenuated, nodular pulmonary lesions. Bronchoscopy with br...

Epidemiology Of Postoperative Complications After Esophagectomy: Implications for Management
CONCLUSIONS: Maintaining a high index of suspicion for early postoperative complications is crucial for rescuing patients after esophagectomy. Early postoperative pneumonia and atrial fibrillation may be sentinel complications for an anastomotic leak, and their occurrence may be used to prompt further clinical investigation. Early recovery protocols should consider the development of early complications into postoperative feeding and imaging algorithms.PMID:37704003 | DOI:10.1016/j.athoracsur.2023.09.004

Application of overlap anastomosis in digestive tract reconstruction during minimally invasive Ivor-Lewis esophagectomy
This study aims to assess the feasibility of the Overlap anastomosis technique in minimally invasive Ivor-Lewis esophagectomy. An accompanying video presentation elucidates our surgical procedures. A retrospective review of 46 patients diagnosed with middle and lower esophageal cancer was conducted. These patients underwent minimally invasive Ivor-Lewis esophagectomy with Overlap anastomosis between January 2019 and December 2020. A consistent team of surgeons performed all procedures. The initial phase involved laparoscopic stomach mobilization, intra-abdominal lymphadenectomies, and preparation of the tubular stomach. Su...

Delayed esophageal anastomotic complication and ramucirumab therapy: A case report
In conclusion, this is the only known report of delayed esophageal anastomotic complication associated with ramuciru mab. VEGF inhibitor therapies such as bevacizumab have been associated with late (greater than 3 months postoperative) colorectal anastomotic complications including fistulas and leaks. Risk factors that have been associated are perioperative radiotherapy and history of early postoperative leak. The se findings raise concern whether VEGF inhibitor therapy should be used in post-esophagectomy patients with recurrence if these rare but catastrophic events are likely to be terminal.

Fast Heartburn Relief Without Deadly Drugs
Since gastroesophageal reflux disease (GERD) was first identified in the early 1930s, the number of people experiencing heartburn has increased substantially. In fact, one study shows that in just 10 years, the number of people experiencing:1 Any GERD symptoms increased by 30% Symptoms at least once a week increased by 47% Severe GERD increased by 24% While these numbers are concerning, I’ll admit they’re not surprising considering the typical American diet. Our nutrition-less, grain-based diet of carbohydrates and starches has wrecked our health and made our bodies behave in ways nature never intended. This has led ...

Effect of Early Versus Late Oral Feeding on Postoperative Complications and Recovery Outcomes for Patients with Esophageal Cancer: A Systematic Evaluation and Meta-Analysis
CONCLUSION: Early oral feeding after esophageal cancer surgery can shorten the time to the first flatus and the first defecation postoperatively, shorten the hospital stay, and promote the recovery of patients. Moreover, it has no significant effect on the incidence of postoperative complications.PMID:37610489 | DOI:10.1245/s10434-023-14139-2

Subcutaneous emphysema, pneumomediastinum and pneumothorax in COVID-19 pneumonia —independent prognostic factors
AbstractThe ongoing coronavirus disease 2019 pandemic has created a substantial disease burden and morbidity. However, the development of subcutaneous emphysema, pneumomediastinum, and pneumothorax have been of rare occurrence and their significance in mortality has not been studied. In a retrospective single-institution observational study at a tertiary care centre in the northern part of India, we evaluated the occurrence of these complications and their relationship with mortality from 1 June 2020 to 30 November 2020. All coronavirus disease 2019 (COVID-19) patients developing subcutaneous emphysema, pneumomediastinum, ...

Liver Transplant After Severe Liver Trauma: The First Report in a Korean Adult
Exp Clin Transplant. 2023 Jul;21(7):619-622. doi: 10.6002/ect.2023.0144.ABSTRACTFollowing a motor-vehicle accident, a 57-year-old man was diagnosed with a grade 4 liver injury (American Association for the Surgery of Trauma organ injury scale) with multiple contrast extravasations. He initially underwent nonoperative management, which included transcatheter arterial embolization. However, he experienced a hemorrhage after the first embo-lization procedure, and so the procedure was repeated. Thereafter, he was diagnosed with liver failure based on findings from computed tomography and liver function tests. On day 28 of hosp...
