Air leak in Covid 19: experience from a DGH in the UK
Conclusions: Inpatient incidence of PTX and PM is still approximately 1%. Survival is better as overall Covid19 survival improves(direct mortality from air leak approx. 21 %) with mortality due to other factors rather than the air leak.

WNK1 signaling after loss of CFTR function in pneumonia-induced lung barrier failure
In severe pneumonia, pulmonary endothelial inflammation and hyperpermeability can lead to life-threatening respiratory failure. The endothelial cystic fibrosis conductance regulator (CFTR) is associated with barrier regulation, and infection with S. pneumoniae is associated with loss of CFTR function, resulting in increased intracellular chloride concentration ([Cl-]i). We hypothesized that S. pneumoniae infection increases [Cl-]i that will inhibit Cl--sensitive with-no-lysine kinase 1 (WNK1), one of the downstream actors of CFTR, leading to decreased barrier integrity, and that WNK1 activation can partly prevent barrier f...

Retrospective analysis of chest drain size and complications in spontaneous secondary pneumothorax in a District General Hospital, UK.
Conclusions: If a drain is indicated for SST we should consider using an 18Fr rather than 12Fr drain initially. Prospective multi-centre data are needed to examine this question.

Povidone Iodine use as effective pleurodesis treatment for prolonged air leak ?
Conclusions: PI is an effective and safe solution for pleurodesis, with low complication rates that remains acceptable. It could be proposed as treatment of PPAL after all types of lung resections including for lung cancer.

Prevalence of pleural effusion in dengue:systematic review and meta-analysis
Conclusion: PE was present in 1/3 of patients with dengue and children more often had PE compared to adults. USG more frequently identified PE compared to CXR. PE may serve as a useful guide in dengue case assessment and clinical management.

Development of scoring system for the prediction of pulmonary complications following curative resection in lung cancer with idiopathic pulmonary
Conclusions: PPCs frequently developed in patients with IPF who received curative resection for NSCLC. Old age, diabetes, high ASA score, low DLco, and extent of surgery were predictors of PPC development.

A multimodal MRI protocol for structure and functional lung, gut and liver imaging in cystic fibrosis (CF) for children aged 6 and above.
Conclusion: This feasibility study has encouraged us to study children aged 6-11 before and after commencement of Kaftrio, to assess its effect on lung health, gut function and liver toxicity.Figure: Representative scan A) Lung (i) UTE image and (ii) PREFUL FV map; (B) Liver MRE (i) base, (ii) stiffness (C) (i) Abdominal dual echo (ii) organ water content.

The interest of telemonitoring on treatment abandonment in patients with home noninvasive ventilation: the Otelo observation study
Conclusion: The implementation of remote monitoring in patients newly treated with nocturnal NIV at home improves compliance and leakage from the first weeks, and significantly decreases the dropout rate at 12 months compared to patients not remotely monitored.

Ambulatory management of bronchopleural fistulae secondary to empyema
We present a case series of 2 patients who received ambulatory management who were not suitable for surgical intervention.Case 1. 69 year-old man admitted with a right hydropneumothorax and mediastinal shift. An empyema was confirmed on chest drain insertion. CT showed a bronchopleural fistula from abscess rupture. After a 4 week hospital stay, an ambulatory chest drain valve was fitted. He received outpatient managment for 2 weeks prior to chest drain removal. Follow up x-rays showed lung re-expansion with small residual pleural effusion.Case 2. 75 year-old man presented with right lower zone consolida...

The accuracy of the Cuff Leak Test in neurocritical patients
Conclusion: The CLT, as demonstrated in the literature, has low sensitivity and should not be used alone to guide decision making regarding extubation, being necessary to associate it with physical examination and other pre-extubation tests.
