Genetic Disorders in Pediatric Rheumatology Clinic: When to Suspect, and Why?
Indian J Pediatr. 2023 Sep 22. doi: 10.1007/s12098-023-04845-w. Online ahead of print.ABSTRACTJust under a decade ago, most children with genetic disorders received a phenotypic diagnosis, often by atlas matching. With advances in genomics (decoding of human genome, easy availability of genetic testing, and reduction in cost of tests), genotypic diagnosis is now a reality. Genetic diseases can lead to non-inflammatory arthritis that can mimic juvenile idiopathic arthritis (JIA). A small but growing number (as newer genes are discovered) of genetic diseases are being diagnosed in children with a seemingly inflammatory muscu...

Effects of Savoring Meditation on Positive Emotions and Pain-Related Brain Function: A Mechanistic Randomized Controlled Trial in People With Rheumatoid Arthritis
medRxiv. 2023 Sep 8:2023.09.07.23294949. doi: 10.1101/2023.09.07.23294949. Preprint.ABSTRACTPositive emotions are a promising target for intervention in chronic pain, but mixed findings across trials to date suggest that existing interventions may not be optimized to efficiently engage the target. The aim of the current mechanistic randomized controlled trial was to test the effects of a single skill positive emotion-enhancing intervention called Savoring Meditation on pain-related neural and behavioral targets in patients with rheumatoid arthritis (RA). Participants included 44 patients with a physician-confirmed diagnosi...

Factors associated with sarcopenia in Japanese patients with rheumatoid arthritis: results from the IORRA cohort study
Clin Rheumatol. 2023 Sep 21. doi: 10.1007/s10067-023-06779-2. Online ahead of print.ABSTRACTTo investigate the prevalence of and factors associated with sarcopenia in Japanese patients with rheumatoid arthritis (RA). We analyzed a cross-section of patients with RA participating in the Institute of Rheumatology Rheumatoid Arthritis cohort survey in 2021. Participants completed self-administered questionnaires, including a 5-item sarcopenia screening index (SARC-F). Patients with a SARC-F score of 4 or higher were categorized as having sarcopenia. Among 2416 Japanese patients with RA (2113 women and 303 men; mean age 63.9 ye...

A cross-sectional study of multidimensional fatigue in biologic-treated rheumatoid arthritis: which variables play a role?
CONCLUSIONS: Our findings indicate that effectively addressing fatigue in RA patients requires an individualized approach. This approach should acknowledge the varying degrees of fatigue across different fatigue dimensions (physical or mental), while also taking into account the patient's mental health problems, pain levels, and disability levels.PMID:37731384 | DOI:10.1080/09638288.2023.2258333

Intermediate and Long-Term Outcomes of the Universal 2 Total Wrist Replacement
Conclusion The Universal 2 poses a reasonable TWR implant choice for patients with debilitating arthritis. However, despite excellent short-term follow-up outcomes, longer term follow-up shows the design does continue to be plagued by implant failure and instability. A Randomized, Controlled Neuroimaging Trial of Cognitive ‐Behavioral Therapy for Fibromyalgia Pain
ConclusionOur results suggest clinically important and CBT-specific associations between somatosensory/motor- and salience-processing brain regions and the default mode network in chronic pain. These patterns of connectivity may contribute to individual differences (and treatment-related changes) in somatic self-awareness. CBT appears to provide clinical benefits at least partially by reducing pain-related catastrophizing and producing adaptive alterations in default mode network functional connectivity.

A clinician ’s perspective on the role of imaging in knee osteoarthritis in clinical practice
AbstractKnee osteoarthritis (OA) is a highly prevalent and disabling disease. Most persons age 45 and over with chronic knee pain have OA and with characteristic history and physical findings, diagnostic imaging is usually not necessary. Further, treatment of chronic knee pain with or without evidence of OA is similar, so imaging does not usually alter therapy. The exception is atypical presentations, such as sudden onset of pain perhaps after trauma or evidence of arthritis in atypical locations elsewhere in the body. Imaging is also unnecessary to follow patients. Given the absence of treatments that slow progression, th...

A Real-World Effectiveness Study Using a Mobile Application to Evaluate Early Outcomes with Upadacitinib in Rheumatoid Arthritis
ConclusionsIn this real-world cohort of patients with RA, treatment with upadacitinib was associated with early and significant improvement in RAPID3, pain, morning stiffness, and fatigue regardless of prior TNFi experience. Clinically meaningful improvement in RAPID3 patient-reported disease activity was observed as early as week 1, with continued improvement reported through week 12.

How To Avoid China ’ s Medicine Monopoly
I want to share a shocking statistic with you… Around 80% of all the pharmaceuticals sold in America — both prescription and over-the-counter — are manufactured in China. I’m talking about drugs for Parkinson’s and Alzheimer’s, blood pressure and blood thinners, diuretics, aspirin, antibiotics, and a big chunk of the world’s insulin and diabetes drugs — just to name a few.1 We don’t even make penicillin anymore. The last penicillin plant in the U.S. closed its doors in 2004. Americans who rely on medicine are now almost entirely at the mercy of a country whose relations with the U.S. have become more ...
