No brain, no problem. Jellyfish learn just fine
When it comes to learned behavior, even the simplest minds are capable of advanced thought. The Caribbean box jellyfish ( Tripedalia cystophora ), which doesn’t even have a brain, can alter its behavior based on past experiences, new research reveals. Scientists believe the creature uses this learning ability along with its astoundingly complex visual system to navigate the murky mangrove swamps it calls home. Scientists have known for some time that animals in the phylum Cnidaria—which includes jellyfish, corals, and sea anemones—are capable of basic forms of learning when repeatedly presented with a stim...

Risk Stratification in a Tertiary Care Spine Centre: Comparison Between STarTBack and OSPRO-YF Screening Tools
CONCLUSIONS: STarTBack and OSPRO-YF are short screening tools with acceptable convergent and known-group construct validity and no floor or ceiling effect of their total score. Both tools could assist with the identification, evaluation, and management of psychological distress in patients presenting to tertiary care spine centres.PMID:37736380 | PMC:PMC10510560 | DOI:10.3138/ptc-2021-0026

Opioid Prescribing Habits of Emergency Department Providers in Response to an Educational Intervention
CONCLUSION: Overall, the educational intervention on the topic of opioid prescribing guidelines did not have the expected impact on amounts of opioids administered or prescribed during the study period. Total opioids utilized and opioids prescribed did not vary between the pre- and post-intervention groups overall, by subset diagnosis, or by provider. There was, however, an increase in utilization of non-opioid treatments that could be indicative of a partial response to the educational intervention. This study showed that long-term effects of education regarding recommended opioid utilization might be less effective than ...

A case of emphysematous pyelonephritis in an older man with poorly controlled type 2 diabetes mellitus
We present the case of a 75-year-old man who was diagnosed early with EPN and received multidisciplinary treatment. He had poorly controlled type 2 diabetes mellitus (DM) and chronic kidney disease (CKD), and was treated with oral hypoglycemic drugs, including a sodium –glucose co-transporter-2 inhibitor. He experienced the onset of back pain in the midsection of his back, tenderness in the costovertebral angle, and a high fever (>  39 °C), accompanied by tachycardia, hypotension, and tachypnea. The patient was diagnosed with pyelonephritis and septic shock. Immediate measures encompassing empirical antibiotic the...

Strong association of depression and anxiety with the presence of back pain while impact of spinal imaging findings is limited: analysis of an MRI cohort study
Development of back pain is multifactorial, and it is not well understood which factors are the main drivers of the disease. We therefore applied a machine-learning approach to an existing large cohort study data set and sought to identify and rank the most important contributors to the presence of back pain amongst the documented parameters of the cohort. Data from 399 participants in the KORA-MRI (Cooperative Health Research in the Region Augsburg) study was analyzed. The data set included MRI images of the whole body, including the spine, metabolic, sociodemographic, anthropometric and cardiovascular data.

Development and temporal validation of clinical prediction models for 1-year disability and pain after lumbar decompressive surgery. The Norwich Lumbar Surgery Predictor (development version)
ConclusionImportant PROM predictors are baseline PROMs, specific co-morbidities, work status, condition duration, previous lumbar operation, female, and smoking status. The ODI model predicted the likelihood of achieving a satisfactory state of both disability and pain.

Urgent Juxtarenal Aortic Aneurysm Repair With Modified Off-the-shelf Endograft to Preserve Intercostal Arteries and Branch Incorporation by Transfemoral Access
CONCLUSION: t-Branch-modified endograft to limit aortic coverage is an alternative option in urgent cases to diminish the risk of spinal cord injury, and association of steerable sheaths with branch incorporation is leading complex aortic repairs to an even less invasive procedure.CLINICAL IMPACT: This paper highlights the possibility of performing complex endovascular aortic repair in an urgent setting with modification of an off-the-shelf multi branched endograft limiting intercostal arteries coverage, thereby diminishing the risk of spinal cord ischemia. This technical innovation provides an alternative for clinicians w...
