J Pain Res. 2023 Sep 13;16:3101-3117. doi: 10.2147/JPR.S424589. eCollection 2023.ABSTRACTThe need to be competent in neuromodulation is and should be a prerequisite prior to completing a fellowship in interventional pain medicine. Unfortunately, many programs lack acceptable candidates for these advanced therapies, and fellows may not receive adequate exposure to neuromodulation procedures. The American Society of Pain and Neuroscience (ASPN) desires to create a consensus of experts to set a minimum standard of competence for neurostimulation procedures, including spinal cord stimulation (SCS), dorsal root ganglion stimula...

As cannabis laws relax, neuroscientist warns of its dangers for developing brain
One morning in June, barely 5 months after the first dispensary for recreational cannabis opened in New York state, neuroscientist Yasmin Hurd spoke via Zoom to an audience of educators and specialists who work with or run programs for children. The session’s organizers, alarmed by how many children in their South Bronx community were now getting their hands on cannabis, had sought Hurd’s expertise on the drug’s effects. Hurd put up a slide of the human brain, its bumps and grooves tinged blue, green, yellow, and red to indicate the distribution of the receptors to which tetrahydrocannabinol (THC), the psychoact...

Emergencies in cancer immunotherapy
CONCLUSIONS: Even emergencies are manageable if we know the most important side effects and the therapeutic options. Immune-mediated side effects are of particular importance because they can affect any organ system. However, as long as we consider the possibility of toxicity from checkpoint inhibitors when the patient presents with symptoms, most side effects are easy to treat and therefore manageable.PMID:37607581 | DOI:10.1055/a-2128-4849

Acute Renal Failure with Severe Loin Pain and  Patchy Renal Ischemia after Anaerobic Exercise in Active Duty Marines
We present three cases of ALPE following participation in the Marine Combat Fitness Test (CFT), an anaerobic evolution. Medical workup demonstrated no additional etiology for acute renal failure. The average peak creatinine in these patients was 2.9 mg/dL, and all demonstrated return to normal renal function, without hemodialysis. One patient experienced recurrent ALPE, after short-interval CFT participation. Military medical providers should be aware of this diagnosis when evaluating service members with acute renal injury after exercise. The clinical course is benign, and affected service members are at increased risk of...

Outpatient Vascular Care: Good, bad or ugly?
BY ANISH KOKA Filling in the holes of recent stories in the New York Times, and Propublica on the outpatient care of patients with peripheral arterial disease Most have gotten used to egregiously bad coverage of current events that fills the pages of today’s New York Times, but even by their now very low standards a recent telling of a story about peripheral artery disease was very bad. The scintillating allegation by Katie Thomas, Jessica Silver-Greenberg and Robert Gebeloff is that “medical device makers are bankrolling doctors to perform artery clearing procedures that can lead to amputationsâ€...
