Understanding wheelchair use in older adults from the National Health and Aging Trends Study
CONCLUSIONS: Current wheelchair users reported more physical difficulties and were much less likely to go outside. This lower outdoor mobility could be due to physical difficulties or potential barriers in physical and socio-cultural environments. In addition, older adults who do not use wheelchairs showed increasing physical problems over time (including balance or coordination problems). Clinicians should consider older wheelchair users' health and physical limitations when prescribing wheelchairs.PMID:37734645 | DOI:10.1016/j.apmr.2023.09.006

Signal detection of adverse events associated with gabapentinoid use for chronic pain
CONCLUSIONS: Our study identified expected and unexpected ADE signals in GABA users. Neurological signals likely related to indications for GABA use. Signals for immunity, mental/behavior, and skin disorders were found in the FDA adverse event reporting system database. Unexpected signals of stroke and cancer require further confirmatory analyses to verify.PMID:37640024 | DOI:10.1002/pds.5685

The Prevalence of Cognitive Impairment among Medicare Beneficiaries Who Use Outpatient Physical Therapy
CONCLUSION: One in five older adults who use outpatient physical therapy services have cognitive impairment. Further, cognitive impairment is more common in older physical therapy patients who report worse physical function and less physical activity.IMPACT: Physical therapists should consider cognitive screening for vulnerable older adults to inform tailoring of clinical practice towards a patient's ability to remember and process rehabilitation recommendations.PMID:37615482 | DOI:10.1093/ptj/pzad115

The Impact of High- Versus Low-Dose Home Rehabilitation for Functional Independence After Hip or Knee Replacement
Surgical pain is predominantly treated with prescription opioids, rather than nonpharmacologic treatments1 like occupational therapy and/or physical therapy (OT/PT).2 Evidence on the effectiveness of nonpharmacologic treatments is limited because national surgical registries lack data on OT/PT.3 This evidence gap has limited the inclusion of OT/PT in postoperative protocols4 and opioid prescribing guidelines5,6 for total hip arthroplasty (THA) and total knee arthroplasty (TKA). We hypothesized that a higher dose of OT/PT would increase the likelihood of overcoming pain and achieving functional independence among Medicare b...

Concurrent Gabapentin and Opioid Use and Risk of Mortality in Medicare Recipients with Non-Cancer Pain
Clin Pharmacol Ther. 2023 Aug 7. doi: 10.1002/cpt.3019. Online ahead of print.ABSTRACTGabapentin is prescribed for pain and is perceived as safe generally. However, gabapentin can cause respiratory depression, exacerbated by concomitant central nervous system depressants (e.g., opioids), a concern for vulnerable populations. We compared mortality rates among new users of either gabapentin or duloxetine with or without concurrent opioids in the 20% Medicare sample. We conducted a new-user design retrospective cohort study, in Medicare enrollees ages 65-89 with non-cancer chronic pain and no severe illness who filled prescri...
