A case of emphysematous pyelonephritis in an older man with poorly controlled type 2 diabetes mellitus
We present the case of a 75-year-old man who was diagnosed early with EPN and received multidisciplinary treatment. He had poorly controlled type 2 diabetes mellitus (DM) and chronic kidney disease (CKD), and was treated with oral hypoglycemic drugs, including a sodium –glucose co-transporter-2 inhibitor. He experienced the onset of back pain in the midsection of his back, tenderness in the costovertebral angle, and a high fever (>  39 °C), accompanied by tachycardia, hypotension, and tachypnea. The patient was diagnosed with pyelonephritis and septic shock. Immediate measures encompassing empirical antibiotic the...

How To Avoid China ’ s Medicine Monopoly
I want to share a shocking statistic with you… Around 80% of all the pharmaceuticals sold in America — both prescription and over-the-counter — are manufactured in China. I’m talking about drugs for Parkinson’s and Alzheimer’s, blood pressure and blood thinners, diuretics, aspirin, antibiotics, and a big chunk of the world’s insulin and diabetes drugs — just to name a few.1 We don’t even make penicillin anymore. The last penicillin plant in the U.S. closed its doors in 2004. Americans who rely on medicine are now almost entirely at the mercy of a country whose relations with the U.S. have become more ...

Multimodality approach to treat calciphylaxis in end-stage kidney disease patients
Ren Fail. 2023;45(2):2256413. doi: 10.1080/0886022X.2023.2256413. Epub 2023 Sep 19.ABSTRACTA multimodality approach has been proposed as an effective treatment for calciphylaxis in patients with end-stage kidney disease. In this retrospective study, we report the cases of 12 end-stage kidney disease patients from l'Hôtel-Dieu de Québec hospital (Canada) who were diagnosed with calciphylaxis between 2004 and 2012 and treated with a multimodality clinical approach including sodium thiosulfate (STS). Statistical analyses were performed to evaluate the impacts of patients characteristics, the different interventions as well ...

Plateau pika fecal microbiota transplantation ameliorates inflammatory bowel disease manifestations in a mouse model of colitis
In conclusion, Plateau pika feces was found to protect mice against colitis induced by dextran sodium sulfate by reducing inflammation and regulating microbial dysbiosis. These findings suggest the potential of Plateau pika feces as an alternative therapy for IBD.

Peripheral temperature dysregulation associated with functionally altered Na < sub > V < /sub > 1.8 channels
Pflugers Arch. 2023 Sep 11. doi: 10.1007/s00424-023-02856-2. Online ahead of print.ABSTRACTThe voltage-gated sodium channel NaV1.8 is prominently expressed in the soma and axons of small-caliber sensory neurons, and pathogenic variants of the corresponding gene SCN10A are associated with peripheral pain and autonomic dysfunction. While most disease-associated SCN10A variants confer gain-of-function properties to NaV1.8, resulting in hyperexcitability of sensory neurons, a few affect afferent excitability through a loss-of-function mechanism. Using whole-exome sequencing, we here identify a rare heterozygous SCN10A missense...

Employing the sustained-release properties of poly(lactic-co-glycolic acid) nanoparticles to reveal a novel mechanism of sodium-hydrogen exchanger 1 in neuropathic pain
Transl Res. 2023 Sep 5:S1931-5244(23)00143-3. doi: 10.1016/j.trsl.2023.09.003. Online ahead of print.ABSTRACTNeuropathic pain is caused by injury or disease of the somatosensory system, and its course is usually chronic. Several studies have been dedicated to investigating neuropathic pain-related targets; however, little attention has been paid to the persistent alterations that these targets, some of which may be crucial to the pathophysiology of neuropathic pain. The present study aimed to identify potential targets that may play a crucial role in neuropathic pain and validate their long-term impact. Through bioinformat...

Pharmacological and Nonpharmacological Treatments for Painful Diabetic Peripheral Neuropathy
Diabetes Metab J. 2023 Sep 6. doi: 10.4093/dmj.2023.0018. Online ahead of print.ABSTRACTDiabetic peripheral neuropathy (DPN) is one of the most prevalent chronic complications of diabetes. The lifetime prevalence of DPN is thought to be >50%, and 15%-25% of patients with diabetes experience neuropathic pain, referred to as "painful DPN." Appropriate treatment of painful DPN is important because this pain contributes to a poor quality of life by causing sleep disturbance, anxiety, and depression. The basic principle for the management of painful DPN is to control hyperglycemia and other modifiable risk factors, but these...
