Development and Current State of Digital Therapeutics for Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) is a common, debilitating disorder characterized by abdominal pain and disordered bowel habits. Current pharmacological treatments often provide incomplete symptom relief and may be poorly tolerated. Furthermore, alleviation of gastrointestinal symptoms does not always translate into improved quality of life (QOL) for IBS patients.Current treatment guidelines recommend brain-gut behavior therapy (BGBT) in conjunction with other IBS therapies, and in randomized controlled trials, BGBT has been shown to improve symptoms, patient satisfaction, functioning, and QOL.

An unusual cause of abdominal pain in an older woman
We report an elderly woman who was diagnosed as toothpick penetrated the jejunal wall and successfully retrieval under DBE. To our best knowledge, there is currently only one report of removing toothpick from the small intestine using DBE. We figured that DBE is a feasible, safe, and effective treatment when toothpicks penetrate the small intestinal wall without other adverse events based on our presentation.PMID:37732347 | DOI:10.17235/reed.2023.9898/2023

Coexistence of small bowel gastrointestinal stromal tumor, small bowel adenocarcinoma, and ganglioneuroma in a patient with neurofibromatosis type I and caused intussusception
We report a synchronous case of small bowel gastrointestinal stromal tumor, small bowel adenocarcinoma, and ganglioneuroma complicated by intussusception in an old woman with NF-I. The patient presented with a 2-month history of abdominal pain and multiple neurofibromas and café au lait macules was found all over the patient's body. The enhanced computed tomography revealed a terminal ileal tumor which was significantly enhanced with secondary intussusception. The tumor of the ileum was considered to be NF-I-associated intestinal neurofibroma.Postoperative pathology and immunohistochemistry showed that the terminal ileoce...

Sizzling Fat —Curative Endoscopic Resection of a Giant Lipoma Causing Colo-Colic Intussusception
We report the case of a 78  years-old woman with abdominal pain, constipation, and bleeding due to a giant lipoma of the sigmoid colon causing intussusception. After an initial diagnostic colonoscopy, the patient underwent an endoscopic mucosal resection (EMR) without complications. Even if surgery is traditionally the prima ry therapeutic approach for giant colonic lipomas, selected cases can be successfully treated with EMR.

A single-center prospective study on pain alleviation during peroral upper endoscopy with an ultrathin endoscope
The efficacy of transnasal endoscopy using an ultrathin endoscope has been reported in several studies. However, few studies regarding peroral endoscopy with ultrathin endoscopes with high resolution have been...

Moderately severe acute pancreatitis after snake bite: a case report from Southern China
In this report, we presented a rare case of moderately severe acute pancreatitis caused by snake venom even after prompt antivenom treatment. The patient was relieved, obviously, with effective treatment of acute pancreatitis and was discharged without severe complications. Although acute pancreatitis after snake bite is a rarity, its serious complications and lethality still deserve our utmost attention, and timely and standardized treatment of acute pancreatitis is needed in addition to antivenom treatment.PMID:37729941 | DOI:10.1055/a-2150-2291

G-POEM for refractory gastroparesis after gastric pacer placement: moving things forward
Gastroparesis remains a devastating diagnosis. The inability to properly empty the stomach after eating can lead to chronic symptoms such as nausea, vomiting, abdominal pain, bloating, difficulty controlling serum blood sugar levels (in patients with diabetes), and other troubles.1 For literally decades, the treatment of patients with gastroparesis has focused on attempts to improve gastric motility with prokinetic medications such as metoclopramide, erythromycin, domperidone, and other agents. Diabetic patients are often also told to lower their hemoglobin A1C to achieve better glycemic control in hopes of producing an im...

Advances in the Management of Pain in Chronic Pancreatitis
AbstractPurpose of ReviewThe data on recent advances in managing chronic pancreatitis (CP) pain is limited. This review highlights the role of endotherapy and the advances in the overall management of pain in CP.Recent FindingsOf late, pancreatic biodegradable stents have been used in endotherapy with appreciable success. These include slow, medium, and fast degrading stents, which optimize the overall management of CP and could prevent the need for multiple procedures. Endoscopic ultrasound-guided celiac plexus block is reserved in selected patients to treat debilitating pain. Total pancreatectomy with islet autotransplan...

Rev Esp Enferm Dig. 2023 Sep 14. doi: 10.17235/reed.2023.9793/2023. Online ahead of print.ABSTRACT59-year-old man, smoker, diabetic and hypertensive. He went to the ER due to fixed abdominal pain in the epigastrium, diaphoresis, dizziness, nausea, and "coffee grounds" vomiting. On examination he presented abdominal distension and pain on palpation in the epigastrium, without peritonism. He had a BP of 235/100 mmHg and in the blood-tests, leukocytosis with neutrophilia and normal hemoglobin. An urgent abdominal CT scan was performed, identifying a 5x6 cm nodular lesion of homogeneous density attached to the wall of the seco...

Chronic follicular pancreatitis: a space occupying lesion with spontaneous resolution
Rev Esp Enferm Dig. 2023 Sep 14. doi: 10.17235/reed.2023.9848/2023. Online ahead of print.ABSTRACTFollicular pancreatitis (FP) is an extremely odd entity characterized by the presence of a pseudotumor that histologically presents lymphoid aggregates and germinal centers. The authors present the case of a 67-year-old woman with epigastric pain and jaundice. Endoscopic ultrasonography (EUS) was performed, revealing a 14x15 mm hypoechoic area with irregular edges in the head of the pancreas. The in situ cytological study showed polymorphic lymphoid cellularity, without atypia suggesting neoplasia. The cell block and the immun...
