Recognition and treatment of freezing and nonfreezing cold injuries.
This article reviews recent medical literature to provide an overview of the recognition and treatment of the two broad categories of cold injuries, freezing and nonfreezing. Frostbite, a freezing cold injury, is treated traditionally with rapid rewarming followed by tissue care and surgical debridement of necrotic tissue. Recently, newer therapies aimed at prevention of tissue necrosis have shown improved outcomes compared with more traditional therapies. These newer treatment regimens for frostbite include the use of various drugs such as ibuprofen, aspirin, warfarin, tissue plasminogen activator, and prostacyclin. The u...

The Participatory Approach to Personalized Medicine
Realizing personalized medicine’s full benefits will require active patient involvement. This is the first in a series of articles defining personalized medicine in the context of participatory medicine. The series will explore how current best practices can contribute to improved medical outcomes, reduced medication adverse reactions, increased patient satisfaction and overall value. What is “personalized medicine?” Personalized medicine, which is also called precision or individualized medicine, is an evolving field in which physicians use certain diagnostic tests to determine which medical treatments w...

Applied Pharmacogenomics in Cardiovascular Medicine.
Abstract Interindividual heterogeneity in drug response is a central feature of all drug therapies. Studies in individual patients, families, and populations over the past several decades have identified variants in genes encoding drug elimination or drug target pathways that in some cases contribute substantially to variable efficacy and toxicity. Important applications of pharmacogenomics in cardiovascular medicine include clopidogrel and risk for in-stent thrombosis, steady-state warfarin dose, myotoxicity with simvastatin, and certain drug-induced arrhythmias. This review describes methods used to accumulate a...

Efficacy and safety of traditional Chinese medicine on thromboembolic events in patients with atrial fibrillation: A systematic review and meta-analysis.
CONCLUSIONS: This meta-analysis suggests that TCM combined with warfarin is superior to warfarin alone for the prevention of total thromboembolic events in patients with AF, with equal risk of bleeding as warfarin alone. PMID: 28619293 [PubMed - in process]

An analysis of the interactions between warfarin and Korean traditional herbal medicine in a Korean oriental hospital
Conclusion: This study identified several cases of potential interactions between warfarin and herbal medicine had been reported. Patients should be encouraged and educated to report adverse events and clinicians should measure PT/INR (prothrombin time/international normalized ratio) when herbal medicines are prescribed to patients taking warfarin. This article belongs to the Special Issue: “Ensuring and Improving Patients’ Safety in Integrative Health Care”.

Review of potential drug interaction between Oseltamivir and Warfarin and why it is important for emergency medicine physicians
Oseltamivir is a very commonly prescribed anti-viral medication by the Emergency Medicine (EM) physicians for the prophylactic and therapeutic treatment of Influenza infection. While the drug interaction of Warfarin with various antibiotics is known, the drug interaction between Oseltamivir and Warfarin is not common. Through our case, we aim to highlight the importance of close monitoring of INR in patients started on Oseltamivir. Also, we feel that conveying the need for close INR monitoring by the Emergency Medicine physicians to the Primary Care Physicians can help prevent a potential bleeding complication.

Visual diagnosis in emergency medicine: retrobulbar haemorrhage
An elderly patient on Coumadin presented to the emergency department one day after left eye surgery. She complained of orbital pain and vision loss. On examination there was eyelid ecchymoses, chemosis and proptosis (figure 1). There was complete vision loss with preservation of light sense. The patient went to the operating room for emergent decompressive surgery. She regained full vision in her eye. Diagnosis: retrobulbar haemorrhage Retrobulbar haemorrhage is a vision-threatening emergency. It is typically secondary to facial trauma or a surgical procedure. Without prompt recognition and treatment, the haemorrhage can e...

Infected Pseudoaneurysm of the Superficial Femoral Artery
A 41-year-old, previously healthy male presented to the emergency department (ED) 4 weeks after accidentally falling onto his hunting knife during a hike. Two weeks after the accident, the patient noticed some swelling and bloody drainage from his thigh wound. At an outside hospital, he was given pain medicine and instructed to use wet to dry dressings on his wound. He began to have worsening pain and swelling. On the day of his presentation to the ED, he noticed foul-smelling, ruddy brown discharge and blood from his wound (). Patient was pale and tired appearing. His hemoglobin was found to be 5.1 g/dL. Computed tomograp...

Literature Update in Hospital Medicine
Publication date: April 2014 Source:Hospital Medicine Clinics, Volume 3, Issue 2 Author(s): William Southern , Bradley A. Sharpe , Romsai Tony BoonyasaiTeaser Perioperative statins reduce perioperative myocardial infarctions, atrial fibrillation, and length of stay. Statins do not increase the risk of intracranial hemorrhage. New oral anticoagulants are superior to warfarin in preventing stroke in patients with atrial fibrillation, and may have a better safety profile. In patients who have an acute gastrointestinal bleed while on warfarin, restarting warfarin after a week may lead to fewer thromboses and lower mortality ...

Practical and clinical considerations in assessing patients with atrial fibrillation for switching to non-vitamin K antagonist oral anticoagulants in primary care
Patrícia O Guimarães,1 Scott Kaatz,2 Renato D Lopes11Duke Clinical Research Institute, Duke Medicine, Durham, NC, 2Hurley Medical Center, Flint, MI, USAAbstract: Atrial fibrillation (AF) is an important risk factor for thromboembolic events, and anticoagulation therapy can reduce this risk. Vitamin K antagonists (VKAs), such as warfarin, have been used for decades in patients with AF for stroke prevention. Currently, non-VKA oral anticoagulants (NOACs) are approved and available for non-valvular AF patients who are at increased risk of stroke. These agents are safe and effective and have important advantages ...

Pharmacogenetics in Cardiovascular Medicine
Abstract Purpose of Review Pharmacogenetics is an important component of precision medicine. Even within the genomic era, several challenges lie ahead in the road towards clinical implementation of pharmacogenetics in the clinic. This review will summarize the current state of knowledge regarding pharmacogenetics of cardiovascular drugs, focusing on those with the most evidence supporting clinical implementation—clopidogrel, warfarin, and simvastatin. Recent Findings There is limited translation of pharmacogenetics into clinical practice primarily due ...
