Malignant Melanoma of the Middle Ear: Case Report
We present a case of malignant melanoma in the middle ear in a 63-year-old woman who presented with severe pain on the right side of her face and head, tinnitus, photophobia, and hemorrhagic discharge from the right ear, as well as right facial nerve paralysis. After more than two years of conservative treatment for otitis media and sclerotic mastoiditis, a pathologically enlarged lymph node on the right side of the neck appears. It is removed, and the histopathological examination reveals that it is metastatic malignant melanoma. Imaging examinations reveal a tumoral lesion in the middle ear with bone infiltration. The ri...

1117PDPembrolizumab and radiotherapy for locally advanced head and neck squamous cell carcinoma (LA-HNSCC) with contraindication to cisplatin therapy
ConclusionsThis is the first study to report acute toxicities of combining pembrolizumab with radiation in LA-HNSCC. Pembrolizumab does not potentiate typical XRT associated toxicities and no unexpected immune related adverse events were observed. Further, lower rates of feeding tube placement and grade 3/4 dermatitis, xerostomia, and mucositis were noted when compared to RTOG1016 (concurrent XRT with cisplatin or cetuximab in LA-HNSCC).Clinical trial identificationNCT02609503.Legal entity responsible for the studyThe authors.FundingMerck Sharp& Dohme Corp.DisclosureAll authors have declared no conflicts of interest.

Sensorineural Hearing Loss After Adoptive Cell Immunotherapy for Melanoma Using MART-1 Specific T Cells: A Case Report and Its Pathophysiology
Conclusion: Based on our presented case and a vast amount of literature there is circumstantial evidence that TCR gene therapy for melanoma targets the perivascular macrophage-like melanocytes in the stria vascularis, resulting in SNHL. We suggest that SNHL after TCR gene therapy may be caused by a disruption of the blood–labyrinth–barrier and the endolymphatic potential and/or a sterile inflammation of the stria vascularis. In severe cases like our subject, we posit that endolymphatic hydrops or hair cell loss may cause irreversible and asymmetrical deafness. Steroid prophylaxis via transtympanic application is debatable.

Exceptional response and multisystem autoimmune-like toxicities associated with the same T cell clone in a patient with uveal melanoma treated with immune checkpoint inhibitors
AbstractBalancing the potential for durable remissions with autoimmune-like toxicities is a key clinical challenge in the use of immune checkpoint inhibitors (ICI). Certain toxicities are associated with an increased response rate; however, the molecular underpinnings of this association are poorly understood. Here, we report a patient with wide spread uveal melanoma who had an exceptional response to treatment with ipilimumab and nivolumab, but suffered severe immune-related sequelae, including central serous retinopathy with retinal detachment, tinnitus, and vitiligo resembling Vogt-Koyanagi-Harada disease, and refractor...

Clear Cell Variant Squamous Cell Carcinoma of Temporal Bone: Case Report and Literature Review
ConclusionThis case showed a very destructive and bizarre clinical presentation but more report of cases is needed to have a better characterization of the clinical presentation and prognosis of this variant of SCC of temporal bone.

Healing the Patient of Tomorrow
Products on well-understood commercialization tracks often take an incremental approach rather than trying to create paradigm-shifting technologies. In this blog, I’ll take a look at medical devices that are on a commercialization track to heal the patient of tomorrow in a way that is substantially better than previous offerings. They are generally broken into three broad categories based on their advantages over existing medical devices: either healing the patient at home, fixing issues with existing devices, or creating non-invasive diagnostic tools. Connected Home Use Devices Connected home-use devices allow a patient...

Eagle Syndrome Secondary to Osteoradionecrosis of the Styloid Process.
CONCLUSION: Radiation is a common component of treatment for head and neck cancers. The diagnosis of Eagle syndrome secondary to osteoradionecrosis of the styloid process is an elusive, but important, diagnosis to consider because the condition can be treated successfully. Benefits, pitfalls and risks of phytotherapy in clinical practice in otorhinolaryngology
Conclusion Scientific proof of the benefit of phytotherapy in otorhinolaryngology remains to be established but, given its widespread use and the reported data, knowledge of this form of treatment needs to be developed.

Personalized Medicine: The Way Forward?
This article will look at some of the strategies already available to help healthcare professionals meet individual patient needs, in the multifaceted field of personalized medicine. Personalizing drug therapy for depression Research suggests that around 50 percent of patients with depression do not respond to first-line antidepressants. What can explain this, and how can it be solved? Current treatment is often a case of trial and error. A patient may take one medication after another, often for 12 weeks or more each time, while symptoms remain the same, or worsen. A team from King's College London in the United Kingd...

Exacerbation of audiovestibular symptoms in a patient with Ménière's disease and choroidal melanoma
We report the case of a man affected by a unilateral Ménière's disease who, after being free from vertigo spells and tinnitus for more than ten years, manifested an exacerbation of symptoms just before the diagnosis of a choroidal melanoma. Melanoma is an aggressive form of cancer derived from neuroectodermal melanocytes. Melanocytes are present in the inner ear where they contribute to the production of endolymphatic potentials and to the maintenance of normal volumes of the inner ear fluids. A possible autoimmune connection between the exacerbation of audiovestibular symptoms and melanoma is discussed.
