Idiopathic intracranial hypertension from benign to fulminant: Diagnostic and management issues
Meagan D Seay, Kathleen B DigreAnnals of Indian Academy of Neurology 2022 25(8):59-64 Idiopathic intracranial hypertension (IIH) primarrily affects obese women of childbearing years and is commonly associated with headaches, pulsatile tinnitus, and vision changes. Though most patients have a “benign” course, it can lead to significant morbidity, including blindness. The treatment approach is based on severity of visual impact and includes weight loss, intracranial pressure lowering medications, and sometimes surgery, such as spinal fluid diversion, optic nerve sheath fenestration, or venous sinus...

Pleomorphic adenoma of the parotid gland
Salivary gland tumors are rare, accounting for 6% to 8% of head and neck tumors. Detecting them in primary care is important so that adequate treatment can be provided. This case report describes a 58-year-old woman whose unilateral pulsatile tinnitus and ear fullness led to the diagnosis of a salivary gland tumor, specifically a pleomorphic adenoma of the parotid gland.

Ototoxicity in cystic fibrosis patients receiving intravenous tobramycin for acute pulmonary exacerbation
Aminoglycoside (AG) antibiotics are commonly used to treat cystic fibrosis (CF) patients with acute pulmonary exacerbations, despite known potential for dose cumulative nephrotoxicity and ototoxicity [1]. Ototoxicity, which includes cochleotoxicity (hearing loss and tinnitus) and vestibulotoxicity (dizziness and vertigo), is a major side effect of all AG, including tobramycin [2]. Monitoring for the early detection of cochleotoxicity using audiometry has been defined and is recommended by the American-Speech-Language-Hearing-Association (ASHA) [3] and the American Academy of Audiology (AAA) [4].

Trigeminal trophic syndrome in a C2 nerve distribution
A 98-year-old woman with longstanding cardiovascular disease and noninsulin-dependent diabetes mellitus was admitted to the hospital for evaluation of a chronic, nonhealing, postauricular ulceration. History revealed an episode of right-sided unilateral tinnitus approximately two years prior prompting otolaryngology evaluation and external auditory canal debridement. Within weeks of the procedure, a small, pruritic, hypoalgesic ulcer developed over the right posterolateral scalp which rapidly expanded in size over the proceeding months before eventually stabilizing.

Crystal Structure of the Monomeric Extracellular Domain of α9 Nicotinic Receptor Subunit in Complex With α-Conotoxin RgIA: Molecular Dynamics Insights Into RgIA Binding to α9α10 Nicotinic Receptors
Conclusion The first crystal structure of a human nAChR domain with an α-Ctx is presented. The structure revealed the interactions between α-Ctx RgIA and the (+) side of neuronal nAChR α9-ECD in high detail. Based on the structure of this complex, models of human α9α10 nAChR ECD with fully formed binding sites were constructed with RgIA bound to each of them. Our MD simulations suggest that the favorable binding site of RgIA in the human α9α10 nAChR ECD consists of either α9 or α10 subunits as the (+) side and of an adjacent α9 rather than α10 su...

Clinical Imaging Findings of Vestibular Aqueduct Trauma in a Patient With Posttraumatic Meniere's Syndrome
In conclusion, for the radiological assessment of TB fractures, the entire VA should be regarded as a part of the otic capsule, and delayed inner ear sequelae should be anticipated for retrolabyrinthine fracture lines that course into or through the VA. When considering treatment options in cases similar to the present, our findings suggest that surgical interventions targeting the eES (ES shunting/decompression procedures (31–33), which are used in MD with the intention to drain the hydropic endolymphatic fluid space, or to improve the fluid resorptive functions of the eES, respectively, most likely cannot work as...

Severity of Tinnitus Distress Negatively Impacts Quality of Life in Patients With Vestibular Schwannoma and Mimics Primary Tinnitus
Conclusions: To our knowledge, this is the first study to investigate VS-related tinnitus with respect to both patients' hearing status and psychological condition. Our results suggest that tinnitus distress strongly affects VS patients' QoL and that its characteristics are similar to primary tinnitus. An intervention for VS-related tinnitus, therefore, should assess to what extent tinnitus bothers patients, and it should reduce any unpleasant emotions that may exacerbate symptoms. This approach should improve their QoL. Introduction Vestibular schwannoma (VS)—or acoustic neuroma—is ...

Milk Consumption Across Life Periods in Relation to Lower Risk of Nasopharyngeal Carcinoma: A Multicentre Case-Control Study
Conclusion: Consumption of milk across life periods was associated with lower risks of NPC. If confirmed to be causal, this has important implications for dairy product consumption and prevention of NPC. Background The etiology of nasopharyngeal carcinoma (NPC) is unclear, and its male predominance has been linked to sex hormones (1). Dairy products are a source of steroid hormones (2) and contain numerous potential antitumor substances. Using an ecological study design, we found that increasing consumption of dairy products might explain the declining NPC incidence in 48 countries/regions (3, 4). Six case-control s...

Cerumen Impaction: Diagnosis and Management.
Abstract Cerumen production is a normal and protective process for the ear canal. However, cerumen should be removed when it causes symptoms (e.g., hearing loss, itching, pain, tinnitus) or prevents assessment of the external auditory canal, the tympanic membrane, or audiovestibular system. Cerumen should also be removed when it limits examination in patients who cannot communicate their symptoms, such as those with dementia or developmental delay, nonverbal patients with behavioral changes, and young children with fever, speech delay, or parental concerns. Patients with coagulopathies, hepatic failure, thrombocyt...
