Thrombosed brainstem developmental venous anomaly causing hydrocephalus
We present a case of thrombosed brainstem DVA leading to venous infarction and oedema within the posterior fossa. A 49 year old, previously fit gentleman presented to the local hospital with a one day history of headache, slurred speech and incoordination. Examination demonstrated GCS of 13/15, cerebellar dysarthria, horizontal nystagmus to the left, mild right sided pyramidal weakness, right sided cerebellar ataxia and bilateral extensor plantars. CT venogram revealed a cerebellar DVA with thrombosis of one of the veins. There was surrounding venous infarction and oedema within the posterior fossa, causing compression of ...

Pineal cyst haemorrhage in an anticoagulated patient
A 72 year old Caucasian female presented to the acute medicine unit with an episode of loss of consciousness and reports of daytime sleepiness with descriptions of an apathetic facade and somnolence from her family. Her past medical history includes mechanical aortic valve replacement and paroxysmal AF for which she was anticoagulated with warfarin, an aneurysmal subarachnoid haemorrhage complicated by hydrocephalus for which a ventriculo-peritoneal shunt had been inserted, and two previous TIAs. Examination showed a GCS of 15, bilateral superior gaze paresis, and an Epworth Sleepiness Scale score of 15/24. A non-contrast ...

Warfarin and Antiplatelet Medication Use Are Independently Associated with Poor Functional Outcomes Following Spontaneous Intracerebral Hemorrhage (P3.096)
Conclusion: Warfarin use is associated with a higher incidence of hydrocephalus, IVH and hematoma expansion, but lesser relative edema volume. While both warfarin and antiplatelet use are independently associated with worse functional outcomes, only warfarin use correlates with higher mortality.Disclosure: Dr. Murthy has nothing to disclose. Predictors and Outcome of Hematoma Expansion in Patients with Intracerebral Hemorrhage (P01.245)
CONCLUSIONS: Patients with congestive heart failure, prior Coumadin use, and intraventricular hemorrhages are at risk of hematoma expansion. Aggressive measures to monitor for and prevent hematoma growth in these patients are important. Larger studies are needed to confirm these results.Disclosure: Dr. Hinduja has nothing to disclose. Cerebral Venous Thrombosis: Current and Newer Anticoagulant Treatment Options
Conclusions: CVT is a rare neurological disease with potentially serious implications and high neurological morbidity and mortality rates. Understanding the role of risk factors—such as genetic or acquired thrombophilia, pregnancy, use of oral contraceptives, and hyperhomocysteinemia—in CVT development is important. Although heparin and warfarin have been used for more than 50 years, newer oral anticoagulants (eg, dabigatran, rivaroxaban, apixaban) might offer an alternative to traditional therapy.

Risk of Ventriculostomy-Associated Hemorrhage in Patients with Aneurysmal Subarachnoid Hemorrhage Treated with Anticoagulant Thromboprophylaxis
Conclusion In our cohort, the risk of VTE was reduced by more than half in patients receiving chemoprophylaxis. Ventriculostomy-associated hemorrhages were rare and minor. Anticoagulant thromboprophylaxis is mostly safe and required in aneurysmal SAH.

Hospital to Hospital Transfers of Cerebral Hemorrhage: Characteristics of Early Withdrawal of Life-Sustaining Treatment
ConclusionsEarly death or WOLST after ICH within 24  h of presentation was most associated with DNR/DNI code status, warfarin use, ICH score, and lower level of consciousness at presentation. These characteristics may be used by clinicians to guide conversations prior to transfer to tertiary care centers.

Extensive cerebral venous thrombosis after resection of third ventricle colloid cyst
CONCLUSION: A preoperative venous study should be performed in all cases to gain a better understanding of the venous anatomy. We advocate meticulous microsurgical techniques to protect the venous system surrounding the foramen of Monro and reduce the amount of retraction during surgery.PMID:37404486 | PMC:PMC10316143 | DOI:10.25259/SNI_348_2023
