Association of mechanical temporal summation of pain with muscle co-contraction during walking in people with knee osteoarthritis
Central pain sensitization, assessed as mechanical temporal summation at the knee, is related to greater knee muscle co-contraction during fast walking in people with knee osteoarthritis. Thus, mitigating central sensitization may be an interventional target to reduce muscle co-contraction for people with knee osteoarthritis.

Management of urachal anomalies in pediatric patients: A comparison of treatment strategies between pediatric urology and general surgery
Persistence of embryonic urachal structures due to a failure of the urachus to involute into the median umbilical ligament is known as a urachal anomaly (UA). UAs may remain asymptomatic or lead to abdominal pain and recurrent infections. Management of UAs in pediatric patients has historically lacked a clear consensus between conservative and surgical management. While both urologists and general surgeons manage this pathology, a comparison of management style and outcomes between these specialties has not been published to our knowledge.

A case of emphysematous pyelonephritis in an older man with poorly controlled type 2 diabetes mellitus
We present the case of a 75-year-old man who was diagnosed early with EPN and received multidisciplinary treatment. He had poorly controlled type 2 diabetes mellitus (DM) and chronic kidney disease (CKD), and was treated with oral hypoglycemic drugs, including a sodium –glucose co-transporter-2 inhibitor. He experienced the onset of back pain in the midsection of his back, tenderness in the costovertebral angle, and a high fever (>  39 °C), accompanied by tachycardia, hypotension, and tachypnea. The patient was diagnosed with pyelonephritis and septic shock. Immediate measures encompassing empirical antibiotic the...

Sclerosing angiomatoid nodular transformation of the spleen in a child with anemia: a case report and  review of the literature
ConclusionAlthough sclerosing angiomatoid nodular transformation is exceedingly rare in children, it should be considered a differential diagnosis in pediatric splenic neoplasms with concurrent hematologic manifestations, such as anemia.

“Buprenorphine, It Works so Differently”: Adults with Sickle Cell Disease Describe Transitioning to Buprenorphine for Treatment of Chronic Pain
A growing body of literature describes the use of buprenorphine for the treatment of chronic pain in people with sickle cell disease. The experiences of people with sickle cell disease who have tried buprenorphine are not yet reported. This qualitative descriptive study was conducted to explore perspectives on buprenorphine for chronic pain in sickle cell disease. We interviewed 13 participants with sickle cell disease who had been prescribed buprenorphine and had a clinic visit between December 1, 2020 and April, 2022 in our Sickle Cell Center for Adults.

Characterisation of common genetic variants in P2RX7 and their contribution to chronic pain conditions
In this study, we first systematically characterized all 17 common nonsynonymous variants using whole-cell patch clamp electrophysiology. Then, we analyzed these variants for statistical association with chronic pain phenotypes using both individual P2RX7 variants as predictors and cumulative allele counts of same-direction cellular effect in univariate models.

Strong association of depression and anxiety with the presence of back pain while impact of spinal imaging findings is limited: analysis of an MRI cohort study
Development of back pain is multifactorial, and it is not well understood which factors are the main drivers of the disease. We therefore applied a machine-learning approach to an existing large cohort study data set and sought to identify and rank the most important contributors to the presence of back pain amongst the documented parameters of the cohort. Data from 399 participants in the KORA-MRI (Cooperative Health Research in the Region Augsburg) study was analyzed. The data set included MRI images of the whole body, including the spine, metabolic, sociodemographic, anthropometric and cardiovascular data.

Soreness Reminds Me of Grief: Patients with Chronic Pain Show Less Differentiated Representations of Emotional Feelings and Bodily States
People experience similarities between emotional feelings and bodily states on a daily basis, but both the magnitude and pervasiveness of this experiential similarity varies across individuals. Inspired by previous findings that chronic pain (CP) is characterized by strengthened pain-affect coupling and a reduced interoceptive accuracy, we conducted two cross-sectional studies to examine whether patients with CP would exhibit less differentiated perception and mental representation of emotional feelings and bodily states.

Development and Current State of Digital Therapeutics for Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) is a common, debilitating disorder characterized by abdominal pain and disordered bowel habits. Current pharmacological treatments often provide incomplete symptom relief and may be poorly tolerated. Furthermore, alleviation of gastrointestinal symptoms does not always translate into improved quality of life (QOL) for IBS patients.Current treatment guidelines recommend brain-gut behavior therapy (BGBT) in conjunction with other IBS therapies, and in randomized controlled trials, BGBT has been shown to improve symptoms, patient satisfaction, functioning, and QOL.

Effective and clinically relevant optimisation of cushioning stiffness to maximise the offloading capacity of diabetic footwear
People with diabetes can gradually lose the protective sensation of pain in their feet due to peripheral neuropathy. As a result, they tend to repeatedly overload and seriously injure their feet, causing the development of diabetic foot ulceration (DFU) [1]. DFU is an open wound with limited capacity for healing, it can get infected and even lead to amputation. It is estimated that worldwide there is one amputation every twenty seconds due to diabetes (85% of all lower-limb amputations) [2]. The severity of this fact becomes even more pronounced considering that eight out of ten people die within five years of having an am...

This ECG was texted to me: normal variant early repolarization, or LAD Occlusion MI (OMI)?
This ECG was texted to me with no other information.  I assumed the presentation was consistent with acute MI.What did I say?" OMI.  Activate the cath lab. "  The T-waves in V2-V6 are diagnostic.  It does, in fact, the STE meets STEMI criteria since there is 1 mm of in V4 and V5.  There is also some non-diagnostic STE in inferior leads.  There is zero reciprocal ST depression. 50% of LAD OMI have zero reciprocal ST depression! But it looks a lot like normal variant STE (also known as " early repolarization " .Then I sent it to the Queen of Hearts PMCardio AI Bot and she gave this...

Urticaria in childhood —what’s new?
AbstractUrticaria can manifest at any age, including infants and young children. Urticaria is one of the most prevalent skin diseases in childhood. As in adults, a  distinction is made between acute and chronic urticaria, with chronic urticaria further classified into chronic spontaneous urticaria and inducible urticaria. According to the current German S3 guideline for classification, diagnosis, and treatment of urticaria, existing literature suggests that the prevalence, disease characteristics, causes, and also the response to treatment are very similar in children and adults. The clinical hallmark of urticaria is the...
