Exercise, Spinal Microglia and Neuropathic Pain: Potential Molecular Mechanisms
Neurochem Res. 2023 Sep 19. doi: 10.1007/s11064-023-04025-4. Online ahead of print.ABSTRACTAs one of the most common neuropathic disorders, neuropathic pain often has a negative impact on patients with persistent pain, mood disorders and sleep disturbances. Currently, neuropathic pain is not treated with any specific drug, instead, drugs for other diseases are used as replacements in clinics, but most have adverse effects. In recent years, the role of spinal cord microglia in the pathogenesis of neuropathic pain has been widely recognized, and they are being explored as potential therapeutic targets. Spinal microglia are k...

The influence of explainable vs non-explainable clinical decision support systems on rapid triage decisions: a mixed methods study
CONCLUSIONS: Although overall trust in the models was low, the median (IQR) weight-on-advice was high (0.33 (0.0-0.56)) and in line with published literature on expert advice. In contrast to the hypothesis, weight-on-advice was comparable between the explaining and non-explaining systems. In 30% of cases, weight-on-advice was 0, meaning the physician did not change their rating. The median of the remaining weight-on-advice values was 50%, suggesting that physicians either dismissed the recommendation or employed a "meeting halfway" approach. Newer technologies, such as clinical reasoning systems, may be able to augment the...

Effect of electrical and chemical (activation versus inactivation) stimulation of the infralimbic division of the medial prefrontal cortex in rats with chronic neuropathic pain
This study aimed to investigate the participation of the infralimbic division (IFL) of the mPFC in chronic NP, as well as the role of the N-methyl-D-aspartic acid receptor (NMDAr) in the elaboration of chronic NP. Male Wistar rats were submitted to the von Frey and acetone tests to assess mechanical and cold allodynia after 21 days of chronic constriction injury (CCI) of the sciatic nerve or Sham-procedure ("false operated"). Electrical neurostimulation of the IFL/mPFC was performed by low-frequency stimuli (20 μA, 100 Hz) applied for 15 s by deep brain stimulation (DBS) device 21 days after CCI. Either cobalt chloride (C...

A Ruptured Cystic Echinococcosis in the Gallbladder and Intra/Extrahepatic Biliary Tract, Radiological and Surgical Imaging Findings
Turkiye Parazitol Derg. 2023 Sep 18;47(3):200-203. doi: 10.4274/tpd.galenos.2023.65265.ABSTRACTCystic echinococcosis is a common parasitic infestation that can still cause serious complications in endemic areas. Intrabiliary rupture is a well-defined complication, but rupture into the gallbladder is rare. The disease may present with cholecystitis and cholangitis. Clinicians and radiologists working in the emergency room will find the management of the disease much easier if they become familiar with the clinical and radiological findings of the cyst. In this article, a 28-year-old male admitted to the emergency department...

Transient left bundle branch block associated with very high coronary artery calcium: a case report
We present a case of very high CAC in a 64-year-old woman with hypertension, type 2 diabetes, and hyperlipidemia presenting with dyspnea. Initial electrocardiogram (ECG) demonstrated normal conduction with a heart rate of 76 beats/min, but new T-wave inversions in V1-V4 and a high-sensitivity troponin-I (hsTnI) value of 6 ng/L (normal < 6 ng/L). Repeat ECG in the emergency department showed normal sinus rhythm (heart rate of 80 beats/min); however, it subsequently demonstrated a left bundle branch block (LBBB) with a repeat hsTnI of 7 ng/L. Stress testing with pharmacologic single-photon emission computerized tomography...

Repair of Distal Fibula Fractures with Minimally Invasive Intramedullary Nailing in the Elderly
J Am Podiatr Med Assoc. 2023 Jul-Aug;113(4):22-121. doi: 10.7547/22-121.ABSTRACTAnkle fractures have been well documented and are common, especially in the elderly, behind hip and distal radius fractures. Open reduction and internal fixation (ORIF) is the standard of treatment for displaced and unstable ankle fractures. Traditionally, ankle fractures in the elderly population have been treated with nonoperative methods; however, nonoperative methods have been associated with increased mortality. Thus, there has been a shift toward operative management for the elderly and less healthy patients. However, these patients often...

Changes in cognitive appraisal in a randomized controlled trial of mindfulness-based cognitive therapy for patients with migraine
CONCLUSION: In this study, pain catastrophizing showed strong promise as a potential mechanism of MBCT-M. Future research should continue to explore cognitive appraisal changes in mindfulness-based interventions.PMID:37723970 | DOI:10.1111/head.14627

The association between labour epidural analgesia and postpartum depression in primiparous patients: a historical cohort study
CONCLUSION: This historical cohort study found an association between LEA and PPD among primiparous patients who subsequently had a second pregnancy in the same province. The findings call into question the hypothesis that LEA decreases the risk for PPD and further illustrates the complexities of PPD.PMID:37726496 | DOI:10.1007/s12630-023-02568-2
