It looks like adult cardiologist should avoid doing pediatric echocardiograms !
A cardiologist  is  a physician who has  trained himself  in a special  way  to deal with any problem of heart.Ironically , it exists only on paper. The field has developed so vast  no one can master everything .There is no such  “Pan or global cardiology expert” .In fact it would be shortly become unethical to try to become one ! Pediatric cardiology  has developed into such a big field , doing a echo in newborn or  infant has become a comprehensive job and  requires  special talent .This unique  and excellent study from Narayana Institute , Bangalore published in the  prestigious Annals of pediat...

Syncope evaluation : Clinical cardiology at it’s new low !
Recently , I came across a   young women  who underwent the following three tests for one episode of syncope after witnessing her pet dog bleeding with  an Injury ! Carotid doppler Holter monitoring and event monitors Brain MRI /MR angiogram This was followed up  by Head up tilt(HUT)  in a premier hospital After 1 week of investigation ,a diagnosis of  Neurocardiogenic syncope was made and she was reassured and no drugs were prescribed. (The collective yield of the above three investigation in fixing  a specific diagnosis is  less than 10 % of all known causes of syncope ) To diagnose  common syncope . . . we...

The Heart of The Matter: Technology In The Future of Cardiology
Sound, rhythm, rate, structure, function – countless features of the heart are measured to keep it healthy for as long as possible. Recently, an army of digital health technologies joined the forces of traditional preventive tools in cardiology to counter stroke, heart attack, heart failure or any other cardiovascular risks. In the future, minuscule sensors, digital twins, and artificial intelligence could strengthen their ranks. Let’s see what the future of cardiology might look like! Fitness trackers, chatbots and A.I. against heart disease Let’s say 36-year-old Maria living in Sao Paulo in 2033 decides one d...

The field of cardiology has seen great men over the centuries. Few women have permanently stamped their presence  in that history .Jane Somerville can be termed mother of pediatric cardiology along with Maude Abbott She has a fascinating life history , having  worked  in Royal Brompton  , Imperial  and Guys London. She was mentored  by  pioneers like  Paul wood , Blalock and others .She is primarily interested in the pediatric cardiology especially congenital heart surgeries .The classification  of pulmonary atresia with VSD  goes with her name. ACE Inhibitors in CKD with CHF : Cardiologist proposes … Nephrologist disposes !
I used to tell my students ,the relationship between the heart and kidney  is so close , it is never justified for  the  two departments of Nephrology and Cardiology  are  physically away by two blocks in our institute . Kidneys are vital to maintain the volume and pressure of body fluids and heart is responsible for keeping this fluid circulating. In clinical setting  it is a well known secret ,most deaths in patients who are on dialysis is cardiac while  most  deaths in patients  with CHF are renal. It remains a mystery  why kidneys were   ever considered as a circulatory organ  , when  our medical pundits ...

A chill in Cardiologist’s spine : Unnecessary intervention becomes a definite crime !
The irony of modern medical care is  9/10 times  medical negligence is  defined in terms of  acts of omission  in  required  level of care . In reality  most medical negligence acts are related to knowingly overdoing a futile diagnostic or therapeutic modality. This irony was never understood by the public, the professionals or  even the judiciary .This remains  the most dangerous issue  facing modern medicine ! Finally some light is appearing in the horizon . A Missouri  Cardiologist is suspended for overdoing things he knows best  . . . namely coronary stenting !  This may bring chills over many cardiologis...

Heros of electrocardiology : Sodi pallares the great Mexican cardiologist !
In the early  20th century , Waller invented the ECG machine. Wilson  created  leads and  methods to record it. Eienthoven  formulated the concepts the electrical theory behind ECG. Between 1940-197os one man ruled supreme in the world of electro cardiography .   He is Dr Sodi pallares from Mexico. His deep insights revolutionised  and helped  us  understand how the cardiac electricity is generated  and propagated  in various pathological states that is the beginning new age electrocardiography! It adds much to his  credit , as in those days scientists  from non American and European countries   were hard to...

Why 90 % of cardiologists suffer from coronary blindness ?
Note :This is a copy of  my earlier blog on coronary micro-circulation  published few years ago.Recently this got numerous hits .Hence I have just reposted it with slight modification. Human coronary circulation stands unique among  others as it is a  life-sustaining circulation. It is indeed a great medical achievement  to visualise  the right and left coronary artery  system by coronary angiogram.  Actually,  what we see is  only a fraction  of  the surface area of coronary circulation .The surface area of  epicardial coronary arteries   constitutes  less than 5 % of entire coronary vascular tree . And  . ...

Mature cardiologists should never trust BMS !
This study done with over 9000 patients  concluded  like this  . . . The DES industry was (is) so powerful it could easily shrug  the challenge of truth that came out briefly  in early 2000s when DES got hit with increased  acute complications. Now, in 2016 NORSTENT study again showed us BMS is as good as DES in all walks of CAD.  Let us see what happens , still  its very unlikely mature cardiologists do not trust BMS. *I have a belief  (Paranoid or not time will tell !) one of the reasons  DES are strongly promoted  is to sustain DAPT market alive and kicking for a long haul ! Scenerio  in India is frighteni...

History of cardiology : Robert F Rushmer, a cardiac scientist par excellence .
Few individual’s works mattered more than others in the field of cardiology. Here was a man born in Utah, studied at Rush university trained in Mayo, settled in Seattle as a pediatrician. But his passion drove him to become a specialist cardiac physiologist with an urge to find the answers to all those lingering queries that arise as a practicing clinical cardiologist.  He built an exclusive animal lab to study the mechanics and physics of circulation and cardiac pumps.   He can be called new age, Harvey. He seemed to always bother, how is it that the 6 liters of blood traverse from he...

Difficult calls in cardiology : Be ready for DC cardioversion during pregnancy !
This report from Taiwan  reassures there is no adverse effect by measuring umbilical artery flow (Yu-Chi Wang European Journal of Obstetrics & Gynecology and Reproductive Biology 126 (2006) 268–274)While we consider DC shock during pregnancy is safe for the fetus, still, shock pads close to the abdomen, amniotic fluid being a good conductor of electricity at least one mother showed a sustained contraction of the uterus and fetal distress. This was possibly attributable to DC shock  Eleanor J. Barnes BJOG 2003 https://doi.org/10.1046/j.1471-0528.2002.02113. Final message Most cardiac arrhythmias in pregnancy ...
