Syncope evaluation : Clinical cardiology at it’s new low !
Recently , I came across a young women who underwent the following three tests for one episode of syncope after witnessing her pet dog bleeding with an Injury !
Carotid doppler
Holter monitoring and event monitors
Brain MRI /MR angiogram
This was followed up by Head up tilt(HUT) in a premier hospital
After 1 week of investigation ,a diagnosis of Neurocardiogenic syncope was made and she was reassured and no drugs were prescribed.
(The collective yield of the above three investigation in fixing a specific diagnosis is less than 10 % of all known causes of syncope )

A chill in Cardiologist’s spine : Unnecessary intervention becomes a definite crime !
The irony of modern medical care is 9/10 times medical negligence is defined in terms of acts of omission in required level of care . In reality most medical negligence acts are related to knowingly overdoing a futile diagnostic or therapeutic modality.

Mature cardiologists should never trust BMS !
This study done with over 9000 patients concluded like this . . .
The DES industry was (is) so powerful it could easily shrug the challenge of truth that came out briefly in early 2000s when DES got hit with increased acute complications.
Now, in 2016 NORSTENT study again showed us BMS is as good as DES in all walks of CAD. Let us see what happens , still its very unlikely mature cardiologists do not trust BMS.
*I have a belief (Paranoid or not time will tell !) one of the reasons DES are strongly promoted is to sustain DAPT market alive and kicking for a long haul !

Difficult calls in cardiology : Be ready for DC cardioversion during pregnancy !
