Thalamo-cortical network activity during spontaneous migraine attacks
Conclusions: These results provide evidence for abnormal connectivity between the thalamus and attentional cerebral networks at rest during migraine attacks. This abnormality could subtend the known ictal impairment of cognitive performance and suggests that the latter might worsen with increasing attack frequency.

Comorbid pain and migraine chronicity: The Chronic Migraine Epidemiology and Outcomes Study
Conclusions: These results suggest that noncephalic pain may be a marker for headache chronicity that could be used to identify people with EM at risk of the onset of CM and people with CM at risk of persistent CM.

Randomized study of IV prochlorperazine plus diphenhydramine vs IV hydromorphone for migraine
Conclusions: IV hydromorphone is substantially less effective than IV prochlorperazine for the treatment of acute migraine in the ED and should not be used as first-line therapy. ClinicalTrials.gov identifier: NCT02389829. Classification of evidence: This study provides Class I evidence that for patients in the ED with migraine, IV prochlorperazine + diphenhydramine is superior to IV hydromorphone.

Clinical characteristics of Alice in Wonderland syndrome in a cohort with vestibular migraine
Conclusions This study characterizes the clinical features of AIWS in patients with VM. We observed several rare and highly unusual AIWS misperceptions (frosted-glass vision, underwater vision, dolly zoom effect, sensation of the brain coming out of the head, closed-eye visual hallucinations, and headlight glare–induced marco/microsomatognosia), and resolution or improvement in AIWS and VM with migraine preventive treatment.

Treatment of migraine in patients with CADASIL: A systematic review and meta-analysis
Conclusions Migraine in CADASIL remains a formidable therapeutic challenge, with patients often tried on several medications. Antimigraine prophylaxis with beta-blockers may be contraindicated relative to other common therapies in CADASIL. Controlled studies are needed to rigorously evaluate the safety and efficacy of antimigraine therapies in this population.

Migraine With Aura Increases Women's Heart Attack Risk
Women who have migraine with aura may be at increased risk of heart attacks, and those taking newer contraceptives may be at higher risk of blood clots. These findings came from two new studies that will be presented at the 65th Annual Meeting of the American Academy of Neurology in San Diego, March 16 to 23, 2013. Visual disturbances and other sensory symptoms, such as flashing lights, numbness, tingling of the hands and face, blind spots, or smell distortion, that come before a migraine headache are known as migraine with aura...

Acupressure Wristband Relieves Migraine Nausea
Nausea is one of the most debilitating symptoms of migraine and affects 80 percent of migraine suffers in the United States. Leading headache physician, Dr. Zoltan Medgyessy of the Berolina Clinic in Lohne, Germany demonstrated in a trial that pressure to the P6 antiemetic point on the inner wrist with an acupressure wristband is an effective and quick therapy for relieving nausea of migraine sufferers. He will be presenting his findings to the U.S. for the first time at the International Headache Congress in Boston, MA on June 27 - 30, 2013...

'Dechronifying' chronic migraine: Detox then treat
"Worldwide, headache affects approximately 50% of adults each year. Approximately 11% of cases meet the criteria for migraine," Prof Paolo Martelletti (Rome, Italy) said today, speaking at the Congress of the European Pain Federation EFIC in Florence. "In Europe and America, we estimate the prevalence of migraine to be 6 to 8% in men and 15 to 18% in women." According to WHO data, migraine is more prevalent than other common disorders such as diabetes and asthma, and is in fact the third most common condition in the world...

Musical murmurs on transcranial ultrasound in childhood migraine with aura
A 12-year-old girl presented with headache, nausea, decreased level of consciousness, and diplopia. Brain MRI, arteriography (figure e-1 on the Neurology® Web site at www.neurology.org), vasculitic markers, and CSF were normal, as were EEGs and ECGs. Transcranial color-coded ultrasonography of the basilar artery revealed typical musical murmurs ("seagull cry") indicative of a hemodynamically significant stenosis (video).1 Sumatriptan provided symptomatic relief; basilar flow normalized on ultrasonography (figure e-2). The clinical presentation suggested childhood migraine with aura; the ultrasound and normal arteriogra...

Intravenous paracetamol versus dexketoprofen in acute migraine attack in the emergency department: a randomised clinical trial
Conclusions Intravenous paracetamol and dexketoprofen appear to produce equivalent pain relief for migraine in the emergency department. ClinicalTrials.gov No NCT01730326.

Stress and migraine: Something expected, something unexpected
This could be a statement about clinical practice, particularly completion of documentation; politics; modern life; or here, the pathophysiology of migraine. Only last year, Neurology® published a very challenging study demonstrating that alleged migraine triggers—flickering lights and strenuous exercise—were only about 10% likely to precipitate migraine in apparently susceptible individuals.1 This contrasts with nitroglycerin for which 75% of migraineurs are triggered.2 What of stress: surely, it is a trigger?

Benign paroxysmal vertigo of childhood is a precursor of migraine
Conclusions BPVC is characterised by short and sudden attacks of vertigo and imbalance, with no aura and with nystagmus. Due to its short duration and since children often close their eyes, nystagmus is difficult to establish. Imbalance may persist for few...

Biphasic neurovascular changes in prolonged migraine aura in familial hemiplegic migraine type 2
Conclusions This is the first report showing biphasic CBF changes during the prolonged aura of FHM2. This study suggested that the results of cross-sectional CBF studies should be interpreted carefully. Initial multifocal hypoperfusion is likely due to functional depression of multifocal origin in the affected hemisphere, but the mechanism of persistent hyperperfusion requires further investigation.
