Cognitive behavioural therapy for insomnia in inpatient psychiatric care: a systematic review
The objective of this systematic review is to present and discuss studies that have adapted CBT-I for inpatient psychiatric care. PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL) and PsycINFO, were searched until June 2023. A total of 10 studies were identified, with the majority being non-randomised trials without comparison groups and small sample sizes. With preliminary character, studies report feasibility and potential efficacy in inpatient settings. Together, this review identifies a paucity of studies on CBT-I or derivates in inpatient psychiatry. Despite challenging in this setting, studies...

Eur J Intern Med. 2023 Sep 18:S0953-6205(23)00333-3. doi: 10.1016/j.ejim.2023.09.013. Online ahead of print.ABSTRACTThe optimal antimicrobial regimen for adults with respiratory failure due to Legionella pneumonia remains controversial. A systematic review was performed to assess the impact on outcomes comparing quinolones versus macrolides. A literature search was conducted in PubMed, Cochrane Library and Web of Science between 2012 and 2022. It yielded 124 potentially articles and ten observational studies met the inclusion criteria. A total of 4271 patients were included, 2879 (67 %) were male. A total of 1797 (42 %) su...

Reasons, Rewards, and Tips for a More Accessible and Inclusive Web
The following is a guest article by Ben Cash, Founding Partner and CEO at Reason One The end of September marks International Week of the Deaf, followed soon after by Blind Americans Equality Day. Both are reminders of the need to make online resources accessible to all. Supporting this is the Americans with Disabilities Act (ADA), intended to protect the civil rights of persons with disabilities when it comes to areas like employment, government services, public accommodations, transportation, and telecommunications. For any organization – and particularly healthcare entities and systems – accessibility should...

Quality Contract for the Prevention of Postoperative Delirium
Anasthesiol Intensivmed Notfallmed Schmerzther. 2023 Sep;58(9):525-539. doi: 10.1055/a-2065-3806. Epub 2023 Sep 19.ABSTRACTDelirium is one of the most common postoperative complications. Delayed initiation of treatment leads to an increased mortality rate within the first 90 days and to an increased need for post-hospital care. Similarly, neurocognitive disorders (NCDs) occur in a quarter of affected patients over the long-term. The use of evidence-based guideline recommendations can reduce incidence rates of delirium, shorten delirium duration, and prevent complications. Implementing delirium management according to evide...

Lifetime exposure to brominated trihalomethanes in drinking water and swimming pool attendance are associated with chronic lymphocytic leukemia: a Multicase-Control Study in Spain (MCC-Spain)
This study identifies an association of long-term exposure to THMs in drinking water, at concentrations below the regulatory thresholds and WHO guidelines, and swimming pool attendance, with chronic lymphocytic leukemia (CLL). These unprecedented findings are highly relevant since CLL is an incurable cancer with still unknown etiology and because the widespread exposure to chlorination by-products that remain in drinking and recreational water worldwide. Despite the demonstrated carcinogenicity in animals of several chlorination by-products, little is known about their potential risks on human health. This study makes a si...

Non-pharmacological Management of Postoperative Delirium
Anasthesiol Intensivmed Notfallmed Schmerzther. 2023 Sep;58(9):494-512. doi: 10.1055/a-2065-3764. Epub 2023 Sep 19.ABSTRACTPostoperative delirium is common especially in the elderly and is associated with high rates of morbidity and mortality. Non-pharmacological multicomponent interventions are effective in reducing the incidence and to a degree the duration of postoperative delirium and are recommended in international guidelines on postoperative delirium as first line intervention for management of delirium. Non-pharmacological management of postoperative delirium consists of strategies for risk stratification, risk red...

Research for Cytomegalovirus Mutations Associated With Resistance to Antivirals in Kidney Transplant Receptors
Cell Transplant. 2023 Jan-Dec;32:9636897231195245. doi: 10.1177/09636897231195245.ABSTRACTCytomegalovirus (CMV) mutations associated with antiviral resistance have become a major problem related to high mortality in kidney transplant patients. The aim of the study was to investigate mutations in the CMV genes UL97 and UL54 associated with antiviral resistance. A retrospective observational cohort study was carried out at Hospital Ophir Loyola (HOL), a reference in Kidney Transplantation. A total of 81 patients who underwent kidney transplantation were followed up between 2016 and 2018 were monitored for CMV viral load by p...

AHL score for predicting mortality risk in patients with pulmonary tuberculosis
Chest. 2023 Sep 17:S0012-3692(23)05434-X. doi: 10.1016/j.chest.2023.09.008. Online ahead of print.ABSTRACTBACKGROUND: A clinically applicable mortality-risk prediction system for pulmonary tuberculosis may improve treatment outcomes, but no easy-to-calculate and accurate score has yet been reported. We aimed to construct a simple and objective disease-severity score for patients with pulmonary tuberculosis.RESEARCH QUESTION: Does a clinical score consisting of simple objective factors predict the mortality risk of patients with pulmonary tuberculosis?STUDY DESIGN AND METHODS: The dataset from our previous prospective study...

The life and legacy of John Robert Cobb: the man behind the angle
J Neurosurg Spine. 2023 Sep 8:1-8. doi: 10.3171/2023.7.SPINE23146. Online ahead of print.ABSTRACTIn 1934, Dr. John Robert Cobb moved to New York to serve as the Gibney Orthopedic Fellow at the Hospital for the Ruptured and Crippled, now known as the Hospital for Special Surgery. In this role, Cobb was faced with a unique task that would shape not only his personal career but also the fields of scoliosis and spine care: to design the first scoliosis specialty clinic. He critically reviewed the treatment methods for scoliosis outlined by prior pioneers in spine surgery and kept his own meticulous records of diagnoses, treatm...

The objective is to assess cesarean delivery rate disparities at the hospital level in a nationally recognized low risk of cesarean delivery group.METHODS: An observational study using a national population-based database, Nationwide Inpatient Sample, from 2008 to 2011 was conducted. All patients with nulliparous, term, singleton, vertex pregnancies from Black and White patients were included. The primary outcome was delivery mode (cesarean vs vaginal). The primary independent variable was race (Black vs White).RESULTS: A total of 1,064,351 patients were included and the overall nulliparous, term, singleton, and vertex pre...
