Post-acute hospital healthcare services for people with mild stroke: a scoping review.
CONCLUSION: Further development and assessment of services is warranted. Incorporation of the entire transition period, and research that is mild stroke and location-specific is advised. Attention to maximizing participation in daily life, secondary prevention, emotional well-being, and careful reporting is needed. PMID: 28024459 [PubMed - as supplied by publisher]

Test Accuracy of Cognitive Screening Tests for Diagnosis of Dementia and Multidomain Cognitive Impairment in Stroke Clinical Sciences
Conclusions— Commonly used cognitive screening tools have similar accuracy for detection of dementia/multidomain impairment with no clearly superior test and no evidence that screening tools with longer administration times perform better. MoCA at usual threshold offers short assessment time with high sensitivity but at cost of specificity; adapted cutoffs have improved specificity without sacrificing sensitivity. Our results must be interpreted in the context of modest study numbers: heterogeneity and potential bias.

Organised inpatient (stroke unit) care for stroke.
CONCLUSIONS: Stroke patients who receive organised inpatient care in a stroke unit are more likely to be alive, independent, and living at home one year after the stroke. The benefits were most apparent in units based in a discrete ward. We observed no systematic increase in the length of inpatient stay. PMID: 24026639 [PubMed - as supplied by publisher]

Formation and Function of Acute Stroke-Ready Hospitals Within a Stroke System of Care Recommendations From the Brain Attack Coalition Clinical Sciences
Conclusions— ASRHs will form the foundation for acute stroke care in many settings. Recommended elements of an ASRH build on those proven to improve care and outcomes at Primary Stroke Centers. The ASRH will be a key component for patient care within an evolving stroke system of care.

Depression after stroke and risk of mortality: a systematic review and meta-analysis.
Conclusions. Despite some limitations, this paper confirms the potential role of depression on post stroke mortality. The relationship between depression and mortality after stroke seems to be related to the followup duration. Further research is needed to clarify the nature of the association between depression after stroke and mortality. PMID: 23533964 [PubMed]

Community mobility after stroke: a systematic review.
Conclusion Survivors of stroke may experience a significant decrease in CM compared to people without neurological injury. Rehabilitation addressing motor function, coordination, independence in activities of daily living, balance and endurance may be important for achieving higher levels of CM. Outcome measures directly addressing CM are needed. PMID: 29322861 [PubMed - as supplied by publisher]

Parenting after stroke: a systematic review.
Conclusion Findings from this systematic review of the perspectives of stroke survivors actively parenting suggest that residual stroke impairments lead to both activity limitations and participation restrictions. However, the literature available on parenting after stroke is limited and there is a significant opportunity to advance this area of stroke research. Microbiological Etiologies of Pneumonia Complicating Stroke Clinical Sciences
Conclusions—Although the analysis was limited by small and heterogeneous study populations, limiting determination of microbiological causality, this review suggests aerobic Gram-negative bacilli and Gram-positive cocci are frequently associated with pneumonia complicating stroke. This supports the need for appropriately designed studies to determine microbial cause and a consensus-based approach in antibiotic usage and further targeted antibiotic treatment trials for enhanced antibiotic stewardship.

Prevalence of suicidal ideation among stroke survivors: A systematic review and meta-analysis.
Conclusions: This meta-analysis suggests a high prevalence of SI among stroke survivors. Routine screening of SI after stroke is needed to plan interventions aimed at reducing suicide risk. PMID: 33186090 [PubMed - as supplied by publisher]

Therapeutic interventions for aphasia initiated more than six months post stroke: a review of the evidence.
CONCLUSIONS: There is evidence to support the use of a number of treatments for chronic aphasia post stroke. Further research is required to fully support the use of these interventions and to explore the effectiveness of other aphasia interventions in the chronic stage. PMID: 23192717 [PubMed - indexed for MEDLINE]

Evidence for therapeutic interventions for hemiplegic shoulder pain during the chronic stage of stroke: a review.
CONCLUSIONS: Positive outcomes were noted with the use of corticosteroid injections and electrical stimulation and conflicting results were seen regarding the use of BTx-A. Overall, these targeted therapies provide benefit in the treatment of HSP in individuals who are more than 6 months post stroke. PMID: 23192716 [PubMed - indexed for MEDLINE]

Systematic review and meta-analysis of constraint-induced movement therapy in the hemiparetic upper extremity more than six months post stroke.
CONCLUSIONS: CIMT to improve UE function is an appropriate and beneficial therapy for individuals who have sustained a stroke more than 6 months previously. PMID: 23192715 [PubMed - indexed for MEDLINE]

Carotid Plaque MRI and Stroke Risk: A Systematic Review and Meta-analysis Clinical Sciences
Conclusions— The presence of intraplaque hemorrhage, lipid-rich necrotic core, and thinning/rupture of the fibrous cap on MRI of carotid plaque is associated with increased risk of future stroke or transient ischemic attack in patients with carotid atherosclerotic disease. Dedicated MRI of plaque composition offers stroke risk information beyond measurement of luminal stenosis in carotid atherosclerotic disease.

Systematic review of leisure therapy and its effectiveness in managing functional outcomes in stroke rehabilitation.
Conclusions: There is some evidence that leisure therapy offers an opportunity to enhance short-term treatment gains in community-based stroke rehabilitation. Further controlled research is needed to establish its longer term effects and assist the development of evidence-based guidelines for this treatment. PMID: 24521839 [PubMed - in process]
