A 40-year-old gravida 0 female presented to her primary care physician with gradual onset right lower quadrant pain. She had an evaluation including CT and ultrasound imaging. Both imaging modalities had findings suspicious for an intra-tubal dermoid cyst (Fig 1). Conventional laparoscopy was performed which confirmed a massively enlarged right fallopian tube, distended with fatty appearing material (Fig 2). She underwent right salpingectomy with contained specimen extraction without intraoperative spill.
