Thirty-three vascular residency programs among 13 countries joining forces to improve surgical education in times of COVID-19: A survey-based assessment.
CONCLUSION: Globalization and technology provide an opportunity for international education, with the goal of building well-rounded and academic vascular surgeons. This group is just the beginning of a large collaborative group among Hispanic American countries, hoping that more residency programs will join, with the aim of breaking borders in the education of vascular surgery. Setting high-impact clinical research priorities for the Society for Vascular Surgery
With the overall goal of enhancing the effectiveness and efficiency of vascular care, the Society for Vascular Surgery (SVS) recently completed a process by which it identified its top clinical research priorities to address critical gaps in knowledge guiding practitioners in prevention and treatment of vascular disease. After a survey of the SVS membership, a panel of SVS committee members and opinion leaders considered 53 distinct research questions through a structured process that resulted in identification of nine clinical issues that were felt to merit immediate attention by vascular investigators and external fundin...

Variation in smoking cessation after vascular operations
Objective: Smoking is the most important modifiable risk factor for patients with vascular disease. The purpose of this study was to examine smoking cessation rates after vascular procedures and delineate factors predictive of postoperative smoking cessation. Methods: The Vascular Study Group of New England registry was used to analyze smoking status preoperatively and at 1 year after carotid endarterectomy, carotid artery stenting, lower extremity bypass, and open and endovascular abdominal aortic aneurysm repair between 2003 and 2009. Of 10,734 surviving patients after one of these procedures, 1755 (16%) were lost to fol...

Approach to the vascular complications of lumbar disc surgery.
The objective of this study was to demonstrate the role of major surgery on patients presenting with vascular complications after lumbar disc surgery. A retrospective analysis of seven cases treated surgically in two tertiary care centers between August 2001 and June 2010 was carried out. The average age of patients (three women and four men) was 35.8 ± 7.2. The most common vessel injured was the left common iliac artery occurring in five patients (71.4%), followed by the left common iliac vein injury detected in two patients (28.5%). Transperitoneal approach was preferred in all cases and primary suturing, graft interpos...

Results from the International Silver Graft Registry for high-risk patients treated with a metallic-silver impregnated vascular graft.
Abstract The purpose of this postmarket surveillance registry was to document the efficacy of a vascular prosthesis coated with metallic silver in high-risk patients undergoing vascular reconstructions. Patency (primary endpoint) and freedom from graft infection (secondary endpoint) data were assessed at a minimum of 12 months in patients with significant co-morbidity and/or confirmed graft infections or infected native vessels. Between November 2006 and December 2009, 230 patients with high-risk factors underwent aortic, peripheral and/or extra-anatomic reconstructions with Silver Graft(®) (SG) in six German, on...

Prevalence of Vertebral Fractures, Vascular Calcifications, and Mortality in Warfarin Treated Hemodialysis Patients.
Abstract Warfarin inhibits vitamin-K dependent proteins involved in bone mineralization and the prevention of vascular calcification (bone Gla protein BGP, matrix Gla protein MGP). In this multicenter, cross-sectional study with 3-year follow-up, data from 387 patients on hemodialysis for ≥1 year at 18 dialysis units were analyzed. Patients on warfarin treatment for > 1 year (11.9% of the population) were compared with the remaining cohort for vertebral fractures, vascular calcifications and mortality. Vertebral fractures and vascular calcifications were sought in L-L vertebral X-rays (D4 to L5). Compared wit...

Recent trends in publications of US vascular surgery program directors.
ConclusionsThe decrease in the number of citations/program/year raises concern about the level of academic activity in vascular surgery. Overall, the annual distribution of the topic of these citations represents a continued shift from open to endovascular cases and decreasing basic science citations. Vascular Training Profiles across Europe
Background: The European Vascular Surgeons in Training (EVST) were appointed by the European Society for Vascular Surgery to review the current status and developments regarding training and certification in vascular surgery (VS) across Europe. Methods: An e-mail-based survey was distributed to EVST representatives in 33 countries. The questionnaire examined the current structure of vascular surgery training and certification (monospecialty, subspecialty, no specialty), as well as the evolving revisions of national curricula. Questionnaires were returned from 31 countries, two of which implement two training models.Results:...

Validated Assessment Tool Paves the Way for Standardized Evaluation of Trainees on Anastomotic Models
Conclusions: Performance on an open simulation model evaluated by a standardized global rating scale correlated to trainee experience level. This initial work confirms the ease and applicability of the grading tool among multiple expert observers and different platforms, and supports additional; research into applications translating this performance into the operating room.

The vacuum-assisted closure (V.A.C®) system for surgical site infection with involved vascular grafts.
CONCLUSION: We showed that the V.A.C.® system is valuable for managing specifically surgical site infection with involved vascular grafts. Using the V.A.C.® system, reoperation rates are reduced; 85% of patients avoided graft replacement. PMID: 24879657 [PubMed - as supplied by publisher]

Using 7.5 frames per second reduces radiation exposure in lower extremity peripheral vascular interventions.
CONCLUSION: Using fluoroscopy at the rate of 7.5 fps during peripheral vascular interventions is associated with lower radiation dose compared to the standard 15 fps with comparable success rate without associated increase in the fluoroscopy time or the amount of the contrast used. Therefore, using fluoroscopy at the rate of 7.5 fps should be considered in peripheral vascular interventions. PMID: 25208902 [PubMed - as supplied by publisher]

Transradial approach for challenging vascular access interventions.
CONCLUSION: The transradial approach for vascular access endovascular interventions is technically feasible and safe. It allows simultaneous treatment of peri-anastomotic lesions in fistulas with complex venous anatomy as well as lesions in the arterial inflow and central outflow. Aggressive venous thromboembolism prophylaxis reduces VTE events in vascular surgery patients.
CONCLUSIONS: The overall rate of VTE was reduced by 75% after the initiation of a standard protocol for pharmacologic VTE prophylaxis. These findings justify an aggressive approach to VTE prophylaxis in vascular surgery patients. PMID: 26123057 [PubMed - as supplied by publisher]

Supply and demand: Will we have enough vascular surgeons by 2030?
Abstract The increase in prevalence of certain cardiovascular risk factors increases susceptibility to vascular disease, which may create demand for surgical intervention. In our study, data collected by the American Association of Medical Colleges Physician Specialty Databook of 2012, the United States Census Bureau, and other nationwide organizations were referenced to calculate future changes in vascular surgeon supply and prevalence of people at risk for vascular disease. In 2010, there were 2853 active vascular surgeons. By 2040, the workforce is expected to linearly rise to 3573. There will be an exponential...
