Analysis of intraoperative radiation use in vascular surgery: Catalyst for quality improvement in patient and personnel safety.
Conclusions There were notable trends in the analyzed radiation usage variables both across the surgical field location and type of surgical procedure. Specifically, cases that involve the abdomen/pelvis, embolization and atherectomy have the highest radiation use. These types of cases can be targeted for future improved dose reduction techniques or staged procedures. This data can serve as baseline information for future quality improvement initiatives for patient and personnel radiation exposure safety. PMID: 30336745 [PubMed - as supplied by publisher]

Incidence of Diagnostic Error in Peripheral Vascular Disease Among Primary Care Physicians
In Canada, vascular surgeons are also vascular specialists, because there does not exist a corresponding medical specialist who manages vascular disease. They are responsible for the diagnosis, medical, surgical and endovascular management of peripheral vascular disease. With an aging population, this places a large burden on the vascular surgeon. It is postulated that many referrals sent to the vascular surgeon are a misdiagnosis from the referring physician. As such, this report sets out to evaluate the diagnostic accuracy of primary care physicians when diagnosing peripheral vascular disease.

Comparison of mid-term graft patency in common femoral versus superficial femoral artery inflow for infra-geniculate bypass in the vascular quality initiative.
CONCLUSIONS: Within the vascular quality initiative, there is no significant difference in life-table determined three-year primary, primary-assisted, and secondary patency between infra-geniculate bypasses using common femoral artery inflow compared to superficial femoral artery inflow. Small, statistically significant differences exist in primary, primary-assisted, and secondary patency favoring common femoral artery inflow after propensity score matching. Long-term follow-up data are required in the vascular quality initiative to better evaluate bypass graft durability as this study was limited by a mean follow-up of on...

Examination of race and infrainguinal bypass conduit use in the Society for Vascular Surgery Vascular Quality Initiative.
CONCLUSION: Racial disparities exist in conduit use for infrainguinal bypass, with black and Hispanic patients less likely to receive vein bypasses. However, the contribution of race to conduit selection is small in adjusted and unadjusted models. Overall, pre-operative variables in the Vascular Quality Initiative poorly predicted vein conduit use for infrainguinal bypass. PMID: 32449478 [PubMed - as supplied by publisher]

Infliximab treatment in refractory vascular Behcet's disease: A single-center experience.
CONCLUSION: Infliximab seems to be an effective and reliable treatment in Behcet's disease with vascular involvement and may also allow reduced dosage or even the discontinuation of corticosteroids. The results of our study suggest that oral anticoagulant use is unnecessary in Behcet's disease with vascular involvement. However, further long-term randomized controlled studies are needed to investigate the length of infliximab regimen, whether or not it should be discontinued, and if so, whether or not immunosuppressants should be given as maintenance after discontinuation. Post-thrombotic syndrome - a position paper from European Society of Vascular Medicine
Vasa. 2021 Mar 15:1-10. doi: 10.1024/0301-1526/a000946. Online ahead of print.ABSTRACT Post-thrombotic syndrome (PTS) is a chronic venous insufficiency manifestation following an episode of deep-vein thrombosis (DVT). It is an important and frequent long-term adverse event of proximal DVT affecting 20-50% of patients. This position paper integrates data guiding clinicians in deciding PTS diagnosis, treatment and follow-up.PMID:33720757 | DOI:10.1024/0301-1526/a000946

Vascular surgeon wellness and burnout: A report from the Society for Vascular Surgery Wellness Task Force
Physician burnout has been linked to medical errors, decreased patient satisfaction, and decreased career longevity. In light of the increasing prevalence of cardiovascular disease, vascular surgeon burnout presents a legitimate public health concern owing to the impact on the adequacy of the vascular surgery workforce. The aims of this study were to define the prevalence of burnout among practicing vascular surgeons and identify factors that contribute to burnout to facilitate future Society for Vascular Surgery (SVS) initiatives to mitigate this crisis.

Patient-reported outcomes for peripheral vascular interventions in the vascular quality initiative
The Society for Vascular Surgery Vascular Quality Initiative (VQI) collects outcomes data for quality improvement and clinical research for a range of vascular procedures including open and percutaneous interventions for peripheral arterial disease (PAD). The VQI Peripheral Vascular Intervention (PVI) registry has collected data on endovascular treatment of lower extremity PAD since 2010. Outcome measures have historically included perioperative complications and 1-year outcomes such as symptom status, vessel patency, reintervention, limb salvage, and survival.

Arm Composite Autogenous Vascular Access Using the Great Saphenous Vein and the Femoral Vein: Results from a Single-centre Study
Conclusion: With a high rate of postoperative morbidity and re-intervention, the ACAVA is a useful additional technique that should be restricted to difficult cases with limited vascular access options.

Vascular graft thrombosis secondary to activated protein C resistance: a case report and literature review.
Abstract Hypercoagulability is a well-documented and prominent risk factor for venous thromboembolism. The role of thrombophilia in arterial thrombotic events is less well defined. A 52-year-old male patient with multiple atherogenic risk factors was admitted for non-healing pedal ulcer and absent distal pulses. Based on the clinical presentation, Doppler ultrasound and angiography findings, the patient underwent elective in situ bypass arterial reconstruction. The saphenous vein graft was of satisfactory quality and the procedure went routinely. Acute graft thrombosis on postoperative day 0 was recognized immedia...

A vascular laboratory protocol for improving and managing after-hours suspected acute deep venous thrombosis.
This study reviews the clinical and workforce impact of a suggested protocol designed for the management of suspected acute deep venous thrombosis (DVT) in patients seen after standard vascular laboratory business hours. The protocol included the use of Wells score, D-dimer and a single dose of therapeutic anticoagulant to defer venous duplex ultrasound (VDU) testing until routine business hours unless contraindicated. Information was collected on medical history, physical exam and the timing of any diagnostic studies and treatment provided. Over 15% of studies done after-hours were deemed unnecessary by our protocol and i...
