Who's your data? Primary immune deficiency differential diagnosis prediction via machine learning and data mining of the USIDNET registry
DISCUSSION: Clinicians should remember to consider the negative predictive features together with the positives. We are calling this a proof-of-concept study to continue with our explorations. A good performance is encouraging, and feature importance might aid feature selection for future endeavors. In the meantime, we can learn from the rules derived by the model and build a user-friendly decision tree to generate differential diagnoses.PMID:37678719 | DOI:10.1016/j.clim.2023.109759

Italian Orthopaedic and Traumatology Society (SIOT) position statement on the non-surgical management of knee osteoarthritis
ConclusionsFor non-surgical management of knee OA, the recommended first step is to bring about lifestyle changes, particularly management of body weight combined with physical exercise and/or hydrotherapy. For acute symptoms, non-steroidal anti-inflammatory drugs (NSAIDs), topic or oral, can be used. Opioids can only be used as third-line pharmacological treatment. Glucosamine and chondroitin are also suggested as chronic pharmacological treatment. Regarding intra-articular infiltrative therapy, the use of hyaluronic acid is recommended in cases of chronic knee OA [platelet-rich plasma (PRP) as second line), in the absenc...

Trimethylsilyl Chloride Aids in Solubilization of Oxidative Addition Intermediates from Zinc Metal
Angew Chem Int Ed Engl. 2023 Sep 6:e202307787. doi: 10.1002/anie.202307787. Online ahead of print.ABSTRACTTrimethylsilyl chloride (TMSCl) is commonly used to "activate" metal(0) powders toward oxidative addition of organohalides, but knowledge of its mechanism remains limited by the inability to characterize chemical intermediates under reaction conditions. Here, fluorescence lifetime imaging microscopy (FLIM) overcomes these prior limitations and shows that TMSCl aids in solubilization of the organozinc intermediate from zinc(0) metal after oxidative addition, a previously unknown mechanistic role. This mechanistic role i...

Speech With Pauses Sounds Deceptive to Listeners With and Without Hearing Impairment
CONCLUSIONS: Listeners may have only partial awareness of the cues that influence their impression of dishonesty. Listeners with hearing impairment may place greater weight on acoustical cues according to the differing degrees of access provided by hearing aids.SUPPLEMENTAL MATERIAL: https://doi.org/10.23641/asha.24052446.PMID:37672786 | DOI:10.1044/2023_JSLHR-22-00618

Factors Associated With Hearing Aid Outcomes Including Social Networks, Self-Reported Mental Health, and Service Delivery Models
CONCLUSIONS: Novel factors including social network of persons with hearing loss who use hearing aids, self-reported mental health, service delivery model, and work situation are significant contributors to hearing aid outcomes. These newly identified factors can inform public hearing health promotion and individualized audiological care to optimize hearing aid outcomes.SUPPLEMENTAL MATERIAL: https://doi.org/10.23641/asha.24060486.PMID:37669616 | DOI:10.1044/2023_AJA-22-00206

pH-Triggered polymeric Nanoparticles-in-Gel for Preventing Vaginal Transmission of HIV and Unintended Pregnancy
Eur J Pharm Biopharm. 2023 Sep 3:S0939-6411(23)00234-5. doi: 10.1016/j.ejpb.2023.09.001. Online ahead of print.ABSTRACTHuman Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/ AIDS) and unplanned pregnancy affect female reproductive health globally. A single product providing a dual purpose of contraception and HIV prophylaxis may improve adherence to the therapy. Thus, we formulated a female-centric multipurpose prevention technology (MPT) comprising of nanoparticle loaded vaginal gel formulation acting as a contraceptive and microbicide. Eudragit® S100 nanoparticles of Atazanavir sulphate (ATZ; antiviral) a...

HIV/AIDS patients' knowledge, attitude, and practice toward anti-retroviral therapy medications' adverse effects and associated factors in Tikur Anbessa Specialized Hospital
CONCLUSION: Overall, the study participants had poor knowledge, lack of positive attitudes, and non-adherence to the actual recommended practice toward AEs of ART medications. Lack of comorbidity, having prior AEs experience, and lack of counseling regarding ART medication AEs were associated with poor knowledge. Female gender and a lack of counseling on the AEs of ART medications were associated with a negative attitude.PMID:37667777 | PMC:PMC10475229 | DOI:10.1177/20420986231194752

Online ahead of print.ABSTRACTShigella species cause severe disease among travelers to, and children living in, endemic countries. Although significant efforts have been made to improve sanitation, increased antibiotic resistance and other factors suggest an effective vaccine is a critical need. Artificial Invaplex (InvaplexAR) is a subunit vaccine approach complexing Shigella LPS with invasion plasmid antigens. In pre-clinical studies, the InvaplexAR vaccine demonstrated increased immunogenicity as compared to the first generation product and w...
