Guidelines for hospital privileges in vascular surgery and endovascular interventions: Recommendations of the Society for Vascular Surgery
The Hospital Privileges Practice Guideline Writing Group of the Society for Vascular Surgery is making the following five recommendations concerning guidelines for hospital privileges for vascular surgery and endovascular therapy. Advanced endovascular procedures are currently entrenched in the everyday practice of specialized vascular interventionalists, including vascular surgeons, but open vascular surgery remains uniquely essential to the specialty. First, we endorse the Residency Review Committee for Surgery recommendations regarding open and endovascular cases during vascular residency and fellowship training.

Incidence of Diagnostic Error in Peripheral Vascular Disease Among Primary Care Physicians
In Canada, vascular surgeons are also vascular specialists, because there does not exist a corresponding medical specialist who manages vascular disease. They are responsible for the diagnosis, medical, surgical and endovascular management of peripheral vascular disease. With an aging population, this places a large burden on the vascular surgeon. It is postulated that many referrals sent to the vascular surgeon are a misdiagnosis from the referring physician. As such, this report sets out to evaluate the diagnostic accuracy of primary care physicians when diagnosing peripheral vascular disease.

Examination of race and infrainguinal bypass conduit use in the Society for Vascular Surgery Vascular Quality Initiative.
CONCLUSION: Racial disparities exist in conduit use for infrainguinal bypass, with black and Hispanic patients less likely to receive vein bypasses. However, the contribution of race to conduit selection is small in adjusted and unadjusted models. Overall, pre-operative variables in the Vascular Quality Initiative poorly predicted vein conduit use for infrainguinal bypass. PMID: 32449478 [PubMed - as supplied by publisher]

Infliximab treatment in refractory vascular Behcet's disease: A single-center experience.
CONCLUSION: Infliximab seems to be an effective and reliable treatment in Behcet's disease with vascular involvement and may also allow reduced dosage or even the discontinuation of corticosteroids. The results of our study suggest that oral anticoagulant use is unnecessary in Behcet's disease with vascular involvement. However, further long-term randomized controlled studies are needed to investigate the length of infliximab regimen, whether or not it should be discontinued, and if so, whether or not immunosuppressants should be given as maintenance after discontinuation. Variation in smoking cessation after vascular operations
Objective: Smoking is the most important modifiable risk factor for patients with vascular disease. The purpose of this study was to examine smoking cessation rates after vascular procedures and delineate factors predictive of postoperative smoking cessation. Methods: The Vascular Study Group of New England registry was used to analyze smoking status preoperatively and at 1 year after carotid endarterectomy, carotid artery stenting, lower extremity bypass, and open and endovascular abdominal aortic aneurysm repair between 2003 and 2009. Of 10,734 surviving patients after one of these procedures, 1755 (16%) were lost to fol...

Approach to the vascular complications of lumbar disc surgery.
The objective of this study was to demonstrate the role of major surgery on patients presenting with vascular complications after lumbar disc surgery. A retrospective analysis of seven cases treated surgically in two tertiary care centers between August 2001 and June 2010 was carried out. The average age of patients (three women and four men) was 35.8 ± 7.2. The most common vessel injured was the left common iliac artery occurring in five patients (71.4%), followed by the left common iliac vein injury detected in two patients (28.5%). Transperitoneal approach was preferred in all cases and primary suturing, graft interpos...

Vascular Training Profiles across Europe
Background: The European Vascular Surgeons in Training (EVST) were appointed by the European Society for Vascular Surgery to review the current status and developments regarding training and certification in vascular surgery (VS) across Europe. Methods: An e-mail-based survey was distributed to EVST representatives in 33 countries. The questionnaire examined the current structure of vascular surgery training and certification (monospecialty, subspecialty, no specialty), as well as the evolving revisions of national curricula. Questionnaires were returned from 31 countries, two of which implement two training models.Results:...

The path to personalized vascular therapy - We are closer than we think.
Abstract The terms "personalized" or "precision" medicine are being used commonly in some branches of medicine but have yet to be widely adopted in vascular surgery. Despite this, personalized vascular therapy occurs on a daily basis in every vascular specialist's office as we strive to make informed recommendations at the individual patient level. The following is a description of several of the areas where advances in personalized vascular care have been achieved, including custom devices, personalized predictions, pharmacogenetics and surgicogenetics. Bio-absorbable antibiotic impregnated beads for the treatment of prosthetic vascular graft infections.
CONCLUSION: This report details the successful use of bio-absorbable antibiotic beads for the treatment prosthetic vascular graft infections in patients at high risk for graft explant or major vascular reconstruction. At early follow-up, we demonstrate successful infection suppression, graft preservation, and limb salvage with the use of these beads in a subset of vascular patients. Direct Admission to a Vascular Referral Center Improves Outcomes for Patients With Acute Vascular Disease
Patients with acute vascular disease frequently need specialized management that may require transfer to a vascular referral center. However, interhospital transfers may delay appropriate care, favoring primary admission to a specialty center. We hypothesized that patients with acute vascular disease requiring transfer to a vascular referral center have worse outcomes than patients directly admitted for specialty acute vascular care.

The impact of urgent computed tomography angiography for gunshot wounds in extremities with concomitant vascular injuries on diagnosis and postoperative outcomes.
Conclusion In the light of our study, we favor the immediate operative exploration of gunshot wounds in extremities with concomitant hard signs of vascular injuries without waiting for preoperative CT angiography, thereby minimizing the shock state and ischemic interval. CT angiography may be required for a more stable patient to avoid unnecessary surgical exploration. PMID: 30041567 [PubMed - as supplied by publisher]

Analysis of intraoperative radiation use in vascular surgery: Catalyst for quality improvement in patient and personnel safety.
Conclusions There were notable trends in the analyzed radiation usage variables both across the surgical field location and type of surgical procedure. Specifically, cases that involve the abdomen/pelvis, embolization and atherectomy have the highest radiation use. These types of cases can be targeted for future improved dose reduction techniques or staged procedures. This data can serve as baseline information for future quality improvement initiatives for patient and personnel radiation exposure safety. PMID: 30336745 [PubMed - as supplied by publisher]

Patient-reported outcomes for peripheral vascular interventions in the vascular quality initiative
The Society for Vascular Surgery Vascular Quality Initiative (VQI) collects outcomes data for quality improvement and clinical research for a range of vascular procedures including open and percutaneous interventions for peripheral arterial disease (PAD). The VQI Peripheral Vascular Intervention (PVI) registry has collected data on endovascular treatment of lower extremity PAD since 2010. Outcome measures have historically included perioperative complications and 1-year outcomes such as symptom status, vessel patency, reintervention, limb salvage, and survival.
