Translational strategies and systems biology insights for blood-brain barrier opening and delivery in brain tumors and Alzheimer's disease
Biomed Pharmacother. 2023 Sep 11;167:115450. doi: 10.1016/j.biopha.2023.115450. Online ahead of print.ABSTRACTThe blood-brain barrier (BBB) plays a critical role in determining the effectiveness of systemic treatments for brain diseases. Over the years, several innovative approaches in BBB opening and drug delivery have been developed and progressed into clinical testing phases, including focused ultrasound (FUS) with circulating microbubbles, mannitol-facilitated delivery of anti-neoplastic drugs, receptor-mediated transcytosis (RMT) by antibody-drug conjugates (ADCs), and viral vectors for gene therapy. We provided a com...

Liquid Biopsy in Low-Grade Glioma: A Systematic Review and a Proposal for a Clinical Utility Score
We examined each biomarker classifying them by CUS and Level of Evidence (LOE). We identified four classes of biomarkers: (1). Circulating protein-(a) vitronectin discriminates LGG from HGG (Sn:98%, Sp:91%, CUS: 3, LOE: III), (b) CTLA-4 discriminates LGG from HGG, (cutoff: 220.43 pg/ml, Sn: 82%, Sp: 78%, CUS:3, LOE:III), (c) pre-operative TGF b1 predict astrocytoma (cutoff: 2.52 ng/ml, Sn: 94.9%, Sp: 100%, CUS:3, LOE:VI). (2). micro-RNA (miR)-(a) miR-16 discriminates between WHO IV and WHO II and III groups (AUC = 0.98, CUS:3, LOE: III), (b) miR-454-3p is higher in HGG than in LGG (p = 0.013, CUS:3, LOE: III), (c) miR-210 ...

The Prognostic and Functional Impact of Sprouty 2 Expression in Non-small Cell Lung Cancer
CONCLUSION: Spry2 expression was lower in tumor tissues than in normal lung parenchyma. Increased expression of Spry2 is associated with poor prognosis. There were no significant associations between epidermal growth factor receptor, anaplastic lymphoma kinase, or c-ros oncogene 1 rearrangement and Spry2 expression. Despite the absence of KRAS mutational analysis, the clinical and epidemiological features of patients suggested that KRAS mutation might be an underlying determinant factor of the functional role of Spry2 in non-small cell lung cancer.PMID:37700432 | DOI:10.1097/COC.0000000000001044

Innovative Genoceuticals in Human Gene Therapy Solutions: Challenges and Safe Clinical Trials of Orphan Gene Therapy Products
Curr Gene Ther. 2023 Sep 11. doi: 10.2174/1566523223666230911120922. Online ahead of print.ABSTRACTThe success of gene therapy attempts is controversial and inconclusive. Currently, it is popular among the public, the scientific community, and manufacturers of Gene Therapy Medical Products. In the absence of any remedy or treatment options available for untreatable inborn metabolic orphan or genetic diseases, cancer, or brain diseases, gene therapy treatment by genoceuticals and T-cells for gene editing and recovery remains the preferred choice as the last hope. A new concept of "Genoceutical Gene Therapy" by using orphan ...

Web-based psychoeducational interventions for managing cognitive impairment –a systematic review
ConclusionWeb-based cognitive intervention programs are a new approach to rehabilitation and patient education. The evidence, although scarce, shows that web-based interventions are feasible and support subjective cognitive functioning. However, the literature to date is extremely limited and the quality of the studies is weak. More research with high-quality study designs is needed.Systematic review registrationhttps://www.crd.york.ac.uk/prospero/display_record.php?RecordID=257315, identifier: CRD42021257315.

Finding Power in My Panic Attacks
Trouble started in the form of rivulets of sweat dampening the waistband of my underwear. It was a bluebird afternoon in Phoenix in December of 2020, mid 60s, desert dry, and my heart was jackhammering against my ribcage. Breathing felt like I was sucking air through a stir straw. A small ABC News crew was arrayed before me, ready to broadcast the report I’d written that day, but with my vision narrowed to a needle’s eye, I could barely see them. I tried to swallow away the sandiness in my mouth but realized I’d forgotten how. [time-brightcove not-tgx=”true”] “I can’t swallow!...

DE-UFormer: U-shaped dual encoder architectures for brain tumor segmentation
Phys Med Biol. 2023 Sep 12. doi: 10.1088/1361-6560/acf911. Online ahead of print.ABSTRACTOBJECTIVE: In brain tumor segmentation tasks, the CNN or Transformer is usually acted as the encoder since the encoder is necessary to be used. On one hand, the convolution operation of CNN has advantages of extracting local information although its performance of obtaining global expressions is bad. On the other hand, the attention mechanism of the Transformer is good at establishing remote dependencies while it is lacking in the ability to extract high-precision local information. Either high precision local information or global con...

Brain tumor-related epilepsy management: A Society for Neuro-oncology (SNO) consensus review on current management
Neuro Oncol. 2023 Sep 12:noad154. doi: 10.1093/neuonc/noad154. Online ahead of print.ABSTRACTTumor-related epilepsy (TRE) is a frequent and major consequence of brain tumors. Management of TRE is required throughout the course of disease and a deep understanding of diagnosis and treatment is key to improving quality of life. Gross total resection is favored from both an oncologic and epilepsy perspective. Shared mechanisms of tumor growth and epilepsy exist, and emerging data will provide better targeted therapy options. Initial treatment with antiseizure medications (ASM) in conjunction with surgery and/or chemoradiothera...

Machine perfusion in liver transplantation
CONCLUSIONS: In situations where the decision has been made to transplant a liver donated after circulatory death or donated following brain death, end-ischaemic HOPE will provide superior clinically relevant outcomes compared with SCS alone. Specifically, graft survival is improved (high-certainty evidence), serious adverse events are reduced (moderate-certainty evidence), and in donors after circulatory death, clinically relevant ischaemic biliary complications are reduced (high-certainty evidence). There is no good evidence that NMP has the same benefits over SCS in terms of these clinically relevant outcomes. NMP does ...
