Fertility and Sexual Health in Young Women with Early-Stage Breast Cancer
Surg Oncol Clin N Am. 2023 Oct;32(4):747-759. doi: 10.1016/j.soc.2023.05.012. Epub 2023 Jun 15.ABSTRACTFertility and sexual health may be impaired by early breast cancer treatment in young women, and these issues should be addressed at diagnosis and through survivorship. Future fertility interest and risk should be considered and communicated, and early referral made to an infertility specialist for those interested. Data regarding safety of fertility preservation options as well as pregnancy after breast cancer are overall reassuring. Patients should be counseled about the impact of systemic therapies and breast surgeries...

Low-Dose Remifentanil in Preterm Cesarean Section with General Anesthesia: A Randomized Controlled Trial
CONCLUSIONS: The use of a low dose of remifentanil before cord clamping for a Cesarean section appears to be safe both for the mother and the preterm newborn, but it does not improve maternal or neonatal outcomes.CLINICAL TRIAL REGISTRATION: ClinicalTrials.gov: NCT02029898.PMID:37713021 | DOI:10.1007/s40272-023-00591-w

Follicular fluid advanced glycation end products in assisted reproduction: A systematic review
Clin Chim Acta. 2023 Sep 14;549:117560. doi: 10.1016/j.cca.2023.117560. Online ahead of print.ABSTRACTFollicular fluid (FF) advanced glycation end products (AGEs) have been associated with low oocyte quality and number, low fertilization rate, impaired embryonic development and low pregnancy rate. These findings are especially relevant in women undergoing in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI), ie, assisted reproductive technology (ART). A systematic literature search was conducted to examine various AGEs including pentosidine, carboxymethyl-lysine (CML), methylglyoxal 5-hydro-5-methylimid...

Xenobiotic metabolites modify immune responses of the cervicovaginal epithelium: potential mechanisms underlying barrier disruption
CONCLUSIONS: Xenobiotic metabolites present in the cervicovaginal space during pregnancy modify immune responses, unveiling potential pathways through which environmental exposures may contribute to the pathogenesis of cervical remodelling preceding preterm birth. Future work identifying xenobiotic sources and routes of exposure offers the potential to modify environmental risks to improve pregnancy outcomes.PMID:37705143 | DOI:10.1111/1471-0528.17654

Care and Monitoring of Pregnant Patients With Left Ventricular Assist Devices
Obstet Gynecol. 2023 Sep 14. doi: 10.1097/AOG.0000000000005351. Online ahead of print.ABSTRACTCardiovascular disease is one of the leading causes of maternal mortality in the United States. Although still rare, pregnancy in patients with left ventricular assist devices (LVADs) is becoming more common. Typical indications for the use of LVADs in reproductive-aged females include ischemic cardiomyopathy, nonischemic (familial) dilated cardiomyopathy, peripartum cardiomyopathy, and some forms of myocarditis. An LVAD drains blood through a cannula placed into the apex of the left ventricle and then returns it to the proximal a...

Utility of Weekly Laboratory Monitoring in the Expectant Management of Hypertensive Disorders of Pregnancy
Obstet Gynecol. 2023 Sep 14. doi: 10.1097/AOG.0000000000005372. Online ahead of print.ABSTRACTOur retrospective cohort study evaluates the diagnostic yield of weekly laboratory surveillance in outpatient management of hypertensive disorders of pregnancy (HDP) based on patient clinical status at the time of laboratory testing. The study included 459 patients and 1,082 laboratory episodes: 356 (32.9%) episodes were performed in the setting of concerning clinical findings and 726 (67.1%) when the patient was asymptomatic. Overall, the diagnostic yield for abnormal laboratory values (n=11) was 1.0% (95% CI 0.4-1.6%) of all ass...

Letrozole During Frozen Embryo Transfer in Women With Polycystic Ovarian Syndrome: A Randomized Controlled Trial
CONCLUSION: There was no difference in pregnancy outcomes between letrozole application compared with artificial cycle for endometrial preparation in women with PCOS who underwent FET. The risk of GDM was higher in the letrozole application group, and the singleton birth weight was lower in the artificial cycle group.CLINICAL TRIAL REGISTRATION: Chinese Clinical Trial Registry, ChiCTR1800014746.PMID:37708500 | DOI:10.1097/AOG.0000000000005367
