Outcomes after aortic coarctation repair in neonates weighing less than 2000  g
CONCLUSION: Delaying surgery beyond 15 days to gain weight does not appear to decrease the risk of recoarctation and may be deleterious in low-birth-weight infants who are exposed for a longer period to risk factors of prematurity-related complications.PMID:37709606 | DOI:10.1016/j.arcped.2023.08.002

Hormesis Defines The Limits Of Lifespan
Ageing Res Rev. 2023 Sep 12:102074. doi: 10.1016/j.arr.2023.102074. Online ahead of print.ABSTRACTThis commentary provides a novel synthesis of how biological systems adapt to a broad spectrum of environmental and age-related stresses that are underlying causes of numerous degenerative diseases and debilitating effects of aging. It proposes that the most fundamental, evolutionary-based integrative strategy to sustain and protect health is based on the concept of hormesis. This concept integrates anti-oxidant, anti-inflammatory and cellular repair responses at all levels of biological organization (i.e., cell, organ and org...

3D interconnected porous PMMA scaffold integrating with advanced nanostructured CaP-based biomaterials for rapid bone repair and regeneration
J Mech Behav Biomed Mater. 2023 Sep 7;147:106106. doi: 10.1016/j.jmbbm.2023.106106. Online ahead of print.ABSTRACTBioactive scaffolds with polymer and nanostructured bioactive glass-based composites are promising materials for regenerative applications in consequence of close mimics of natural bone composition. Poly methyl methacrylate (PMMA) is a highly preferred thermoplastic polymer for orthopedic applications as it has good biocompatibility. Different kinds of bioactive, biodegradable as well as biocompatible biomaterial composites such as Bioglass (BG), Hydroxyapatite (Hap), and Tricalcium phosphate (TCP) can be integ...

Disrupted HSF1 regulation in normal and exceptional brain aging
Biogerontology. 2023 Sep 14. doi: 10.1007/s10522-023-10063-w. Online ahead of print.ABSTRACTBrain aging is a major risk factor for cognitive diseases such as Alzheimer's disease (AD) and vascular dementia. The rate of aging and age-related pathology are modulated by stress responses and repair pathways that gradually decline with age. However, recent reports indicate that exceptional longevity sustains and may even enhance the stress response. Whether normal and exceptional aging result in either attenuated or enhanced stress responses across all organs is unknown. This question arises from our understanding that biologica...

Parastomal Hernia Repair
Surg Clin North Am. 2023 Oct;103(5):993-1010. doi: 10.1016/j.suc.2023.04.008. Epub 2023 May 12.ABSTRACTParastomal hernias (PHs) are common and contribute to significant patient morbidity. Despite 45 years of evolution, mesh-based PH repairs continue to be challenging to perform and remain associated with high rates of postoperative complications and recurrences. In this article, the authors summarize the critical factors to consider when evaluating a patient for PH repair. The authors provide an overview of the current techniques for repair, including both open and minimally invasive approaches. The authors detail the mesh...

Laparoscopic Ventral Hernia Repair
Surg Clin North Am. 2023 Oct;103(5):947-960. doi: 10.1016/j.suc.2023.05.009. Epub 2023 Jul 12.ABSTRACTThe laparoscopic approach to ventral hernia repair is a safe and effective approach for both elective and emergent repair. The preoperative technical considerations include assessment of incarceration and potential for extensive adhesiolysis, size of defect, and atypical hernia locations. Preoperative considerations include weight loss and lifestyle modification. There are multiple methods of fascial defect closure and mesh fixation that the surgeon may consider via a laparoscopic approach, making it adaptable to varying c...

Ventral Hernia Repair: Does Mesh Position Matter?
Surg Clin North Am. 2023 Oct;103(5):935-945. doi: 10.1016/j.suc.2023.04.005. Epub 2023 May 15.ABSTRACTMesh positioning is a commonly discussed detail in ventral hernia repair and is often cited as a major contributor to the outcome of the operation. However, there is a paucity of data that establishes one plane as superior to others. In this article, we will provide an overview of all potential planes to place prosthetic material and review the relevant literature supporting each option and the complications associated with accessing each anatomic plane.PMID:37709397 | DOI:10.1016/j.suc.2023.04.005

Use of extracorporeal membrane oxygenation and cerebral oximetry in a stage 1 norwood repair for hypoplastic left heart syndrome
We report a case where the use of ECMO was beneficial after stage I Norwood palliation with severe ventricular dysfunction. The systemic-to-pulmonary artery shunt was kept open to balance the systemic and pulmonary circulations. Cerebral oximetry can be useful as an additional monitoring modality to guide management, monitor cerebral perfusion, and help detect cerebral steal.PMID:37706390 | PMC:PMC10284473 | DOI:10.4103/0971-9784.320955

Outcome of obstructed total anamalous pulmonary venous connection (TAPVC) repair patients with milrinone versus milrinone and inhaled nitric oxide (INO): A prospective randomized observational study
CONCLUSION: Most of the patients with obstructed TAPVC had severe PAH. Management of severe PAH with a combination of milrinone with iNO had a better outcome than milrinone alone.PMID:37706383 | PMC:PMC10284474 | DOI:10.4103/aca.aca_56_22

Atrial septal defect occluder device embolization: Experience of a tertiary care cardiac center
CONCLUSIONS: Once embolization occurs, the risks associated increase manifold. Most of the surgical extractions are uneventful; however, there could be certain complications that may need repair of valvular apparatus, the institution of TCA, or the need for the lateral position. Air embolization though very rare can occur which could be fatal.PMID:37706378 | PMC:PMC10284467 | DOI:10.4103/aca.aca_28_22
