The effect of exercise on vascular health in chronic kidney disease: a systematic review and meta-analysis of randomized controlled trials
Am J Physiol Renal Physiol. 2023 Sep 21. doi: 10.1152/ajprenal.00152.2023. Online ahead of print.ABSTRACTPatients with chronic kidney disease (CKD) are at increased risk of cardiovascular disease. This increased risk cannot be fully explained by traditional risk factors such as hypertension. Endothelial dysfunction and arterial stiffness have been suggested as factors which explain some of the increased risk, and are independently associated with important cardiovascular outcomes in CKD patients. Studies in other disease populations have shown positive effects of exercise on vascular dysfunction. The aim of this review was...

Revising the roles of aldosterone in vascular physiology and pathophysiology: from electocortin to baxdrostat
Can J Cardiol. 2023 Sep 19:S0828-282X(23)01732-4. doi: 10.1016/j.cjca.2023.08.035. Online ahead of print.ABSTRACTAldosterone was initially identified as a hormone primarily related to regulation of fluid and electrolyte homeostasis. However, over the past twenty years there has been an increasing appreciation of its role in regulation of vascular function and pathophysiology in the setting of hypertension, atherosclerosis, and heart failure. The purpose of this review is to highlight recent advances in our understanding the biology of aldosterone as it relates to the pathophysiology and the management of vascular disease-e...

Brain Injury: How Dietary Patterns Impact Long-Term Outcomes
We examined Mediterranean, Dietary Approaches to Stop Hypertension, plant-based, ketogenic, and intermittent fasting dietary patterns through publications of clinical trials and systematic reviews. While many reviews had significant positive findings, some were limited by generalizability.SUMMARY: While there is extensive literature on the immediate nutrition goals in the inpatient setting following an acute TBI exposure, there is limited literature discussing the nature of diet and nutrition in the post-acute setting. Fortunately, most individuals with TBI exposure survive their initial injury and continue into the recove...

Prevalence and risk factors of stroke in high-altitude areas: a systematic review and meta-analysis
Conclusions The findings of this study provide insights into the pooled prevalence of stroke in high-altitude areas, highlighting variations based on geographic regions and sampling type. Moreover, age, gender, hypertension and obesity were found to be associated with the occurrence of stroke. PROSPERO registration number CRD42022381541.

Burden of pulmonary hypertension due to chronic obstructive pulmonary disease: Analysis of exacerbations and healthcare resource utilization in the United States
CONCLUSIONS: Patients with PH-COPD had higher rates of severe exacerbations, hospitalizations, and costs compared to COPD patients without PH, underscoring the need for targeted therapies to prevent and manage PH in patients with COPD.PMID:37729954 | DOI:10.1016/j.rmed.2023.107412

Surgery and Anesthesia in Patients with Pulmonary Hypertension
Semin Respir Crit Care Med. 2023 Sep 20. doi: 10.1055/s-0043-1772753. Online ahead of print.ABSTRACTPulmonary hypertension is characterized by right ventricular impairment and a reduced ability to compensate for hemodynamic insults. Consequently, surgery can be challenging but is increasingly considered in view of available specific therapies and improved longer term survival. Optimal management requires a multidisciplinary patient-centered approach involving surgeons, anesthetists, pulmonary hypertension clinicians, and intensivists. The optimal pathway involves risk:benefit assessment for the proposed operation, optimiza...

The assessment of thoracal approaches in the treatment of aortic coarctation
CONCLUSION: Thoracotomy provided feasible surgical access that led to satisfactory results with a low complication rate, negligible residual gradient, low incidence of hypertension and excellent rate for freedom from re-intervention in the treatment of CoA.PMID:37728294 | DOI:10.5830/CVJA-2023-039

Diagnosis and Treatment of IgA Nephropathy-2023
Wien Klin Wochenschr. 2023 Aug;135(Suppl 5):621-627. doi: 10.1007/s00508-023-02257-6. Epub 2023 Sep 20.ABSTRACTImmunoglobulin A nephropathy (IgAN) is the most common glomerulonephritis. It leads to end-stage kidney disease in about a third of the patients within 10 to 20 years. The pathogenesis of IgAN is incompletely understood. It is believed that a dysregulation of the mucosal immune system leads to undergalactosylation of IgA, followed by formation of IgG autoantibodies against undergalactosylated IgA, circulation of these IgG-IgA immune complexes, deposition of the immune complexes in the mesangium, ultimately resulti...

Real-World Treatment Patterns Among Patients with Connective Tissue Disorder-Related Pulmonary Arterial Hypertension in the United States: A Retrospective Claims-Based Analysis
CONCLUSIONS: Treatment patterns were generally similar between the CTD + PAH and PAH cohorts, although combination therapy was more frequent in the CTD + PAH cohort. Both cohorts may benefit from broader use of all available PAH treatment classes, including combination therapy. Considering the life-threatening nature of PAH, our findings also highlight the need to address the low persistence rates with PAH therapies regardless of etiology.PMID:37728697 | DOI:10.1007/s12325-023-02658-z

Association of participants who screened positive for night eating syndrome with physical health, sleep problems, and weight status in an Australian adult population
CONCLUSIONS: Several physical health problems were found to be significantly associated with the spNES group including sleep problems, increased BMI, increased levels of pain and lower self-reported physical health-related quality of life. Consequently, future research exploring the complex interaction between NES and these medical conditions may provide further insight into the diagnosis, screening tools and management of NES. Additionally, this study highlights the need for future studies which use larger population-based samples.LEVEL OF EVIDENCE: Level III. Evidence obtained from well-designed cohort or case-control an...
