Underwater versus conventional EMR of large nonpedunculated colorectal lesions: no clarity on the role of submucosal injection
We read with interest the article by Sanchez et  al1 comparing the efficacy of underwater EMR (UEMR) and conventional EMR (CEMR) of large nonpedunculated colorectal lesions in a multicenter randomized controlled trial. Prophylactic EUS-guided gallbladder drainage in patients with malignant biliary obstruction and cystic duct orifice involvement: Need more answers!
We read with great interest the randomized controlled trial by Robles-Medranda et  al1 on primary EUS-guided gallbladder drainage (GBD) in preventing acute cholecystitis (AC) in distal malignant biliary obstruction (MBO) and cystic duct orifice (OCD) involvement. AC after self-expandable metal stent (SEMS) placement in a distal MBO associated with tumoral involvement of the OCD is a significant adverse event of palliative endoscopic biliary drainage, and EUS-GBD seems to be an attractive prophylactic strategy to mitigate the risk of AC.

Impact of withdrawal time on adenoma detection rate: some issues to be addressed
Desai et  al1 reaffirmed the effect of longer withdrawal time (WT) on adenoma detection rate (ADR) and concluded that ADR correspondingly increased as WT rose from 6 minutes to 13 minutes, where ADR reached a “ceiling effect.” This result is of a certain degree of clinical relevance; yet, several issues need further clarification.

Combined network analysis and interpretable machine learning reveals the environmental adaptations of more than 10,000 ruminant microbial genomes
ConclusionOur findings indicate that the environment has an impact on the functional features of microbiomes in ruminant. The findings provide a new insight for the future development of microbial resources for the sustainable development in agriculture.
